Traumatic perilymphatic fistula secondary to stapes luxation into the vestibule: a case report.

A penetrating ear injury with a perilymphatic fistula is not an uncommon occurrence in otolaryngologic practice, but stapes luxation is rare. We report the case of an 11-year-old boy who developed a traumatic perilymphatic fistula secondary to an atypical stapes luxation into the vestibule. After sustaining a penetrating injury to the right ear, the patient presented with otalgia, vertigo, vomiting, gait unsteadiness, and hearing loss. High-resolution computed tomography (HRCT) of the temporal bone detected pneumolabyrinth, indicating a perilymphatic fistula. The stapes had pivoted on the foot

Vestibular perceptual thresholds to angular rotation in acute unilateral vestibular paresis and with galvanic stimulation.

Studies of compensation of injury to the human vestibular system have, in the main, focused on the vestibular-ocular reflex. Probing vestibular perception allows more of the sensory pathway to be assessed. We present a novel paradigm for simultaneously testing vestibular perceptual and nystagmic thresholds to angular acceleration around an earth vertical axis. The perceptual thresholds can be modulated asymmetrically in normal subjects by DC galvanic stimulation with the head flexed in the roll plane, as expected from the main torsional plane of action of the galvanic stimulus. The perceptual

Increased vestibular contribution to posture control in individuals with chronic headache.

OBJECTIVE: Numerous studies have identified an association between headache disorders and vestibular symptoms, such as dizziness, vertigo, and motion sensitivity. Using bipolar, binaural galvanic vestibular stimulation (GVS), our objectives were to (1) determine the degree of vestibular sensitivity in borderline headache sufferers, and (2) characterize the postural response to vestibular perturbation in these individuals. METHODS: Fourteen volunteers participated in this study: 8 individuals with chronic headache (6 with migraine and 2 with tension type headache (TTH)), and 6 healthy control

[Gaze-evoked suppression of the vestibular-ocular reflex in patients with posterior cervical sympathetic syndrome].

This vestibulometric (sinusoidal rotation) study included patients with posterior cervical sympathetic syndrome (n=40) and with vestibular neuronitis (n=20) as well as 20 healthy subjects of the control group. Vestibular dysfunction was induced by the method proposed by de Klein. The patients with posterior cervical sympathetic syndrome in the decompensation phase displayed abnormal gaze-evoked suppression of the vestibular-ocular reflex when their head movements lined up with sinusoidal rotation. This symptom is considered to be of primary value for the diagnosis of vestibular dysfunction in

Lmx1a maintains proper neurogenic, sensory, and non-sensory domains in the mammalian inner ear.

Lmx1a is a LIM homeodomain-containing transcription factor, which is required for the formation of multiple organs. Lmx1a is broadly expressed in early stages of the developing inner ear, but its expression is soon restricted to the non-sensory regions of the developing ear. In an Lmx1a functional null mutant, dreher (dr(J)/dr(J)), the inner ears lack a non-sensory structure, the endolymphatic duct, and the membranous labyrinth is poorly developed. These phenotypes are consistent with Lmx1a's role as a selector gene. More importantly, while all three primary fates of the inner ear - neural, se

Vestibulo-ocular reflex and motion sickness in figure skaters.

In order to determine the effect of figure skating on the functional plasticity of the vestibular system, we quantified vestibulo-ocular reflex (VOR) and motion sickness (MS) intensity in 11 female figure skaters and 11 matched control subjects. Vestibular stimulation consisted of three cycles of sinusoidal rotation (0.025 Hz, +/-60 degrees /s) and two velocity steps of 60 degrees /s (acceleration 60 degrees /s(2)). Nauseogenic stimulation consisted of a constant velocity (60 degrees /s) off vertical axis rotation (OVAR) using a 15 degrees tilt angle. Subjective sickness symptoms were rated im

Spontaneous nystagmus and canalithiasis preceding a loss of vestibular function.

The patient, a 45-year-old female with Ménière's disease, was submitted to endolymphatic sac surgery because of frequent spells of vertigo. The day after surgery she became dizzy and lost the hearing in the operated ear. She presented with a brisk nystagmus towards the healthy ear; however, a remaining vestibular function on the operated side was still present. The head impulse test was normal and in the positioning test, when turning to the operated ear, the spontaneous nystagmus was replaced by a transient horizontal nystagmus in the opposite direction. It is likely that this geotropic nysta

Dynamic posturography findings predict balance status in vestibular schwannoma patients.

OBJECTIVE: To test whether condition 5 score (C5S) and condition 6 score (C6S) of the sensory organization test of computerized dynamic platform posturography (CDPP) differ between vestibular schwannoma (VS) patients with and without vestibular symptoms. STUDY DESIGN: Retrospective review of prospectively collected data. SETTING: Tertiary academic referral center. PATIENTS: Two hundred and sixteen consecutive patients with a histological diagnosis of a VS (103 women; 113 men; age range, 18-78 years; median, 54 years) who had been preoperatively evaluated by CDPP. A hundred and twelve patien

Specific influences of cerebellar dysfunctions on gait.

Cerebellar ataxic gait is characterized by unsteady movements and variable gait patterns. Previous studies have successfully identified pathological changes of balance-related gait parameters. However, it has been difficult to demonstrate deficits of joint coordination and the control of limb dynamics. This has motivated the hypothesis that cerebellar ataxic gait might be affected predominantly by balance impairments. We investigated the influences of different types of cerebellar dysfunction on the gait patterns of patients suffering from degenerative cerebellar disease (13 patients, five fem

Vestibular-evoked myogenic potentials and caloric stimulation in infants with congenital cytomegalovirus infection.

BACKGROUND: The influence of congenital cytomegalovirus infection on cochlear function has been well recognised; however, its impact on the vestibular system in infants has not been examined. The purpose of the present study was to evaluate vestibular function in a group of infants, using caloric stimulation tests and vestibular-evoked myogenic potential measurements. MATERIALS AND METHODS: Vestibular-evoked myogenic potentials and auditory brainstem responses were recorded and caloric stimulation was performed in 66 infants aged three months, comprising 40 healthy controls and 26 infants wit

The systemic application of diazepam facilitates the reacquisition of a well-balanced vestibular function in a unilateral vestibular re-input model with intracochlear tetrodotoxin infusion using an osmotic pump.

Diazepam is a popular medicine used in the treatment of acute vertigo. In the past, many studies investigating the effect of diazepam in peripheral vestibular destruction have been reported. However, no previous study has yet investigated the effect of diazepam on a model with a transient and reversible vestibular function similar to recurrent vertigo as seen in Meniere's disease. We thus made a peripheral vestibular re-input model by the unilateral intracochlear administration of tetrodotoxin (TTX) using an osmotic pump and then examined the influence of diazepam on the vestibular system in t

Comparison of tone burst and tapping evocation of myogenic potentials in patients with chronic otitis media.

OBJECTIVE: Vestibular evoked myogenic potential (VEMP) has recently been broadly studied in cochleo-vestibular disorders to elucidate its mechanism. Because it is evoked by loud sound stimulation, impairment of the sound transmission through the middle ear may affect VEMP results. This study aims to compare the response rate of VEMPs using the tone burst method and the tapping method in patients with chronic otitis media (COM). DESIGN: Fourteen patients (22 ears) with conductive hearing loss due to COM were subjected to VEMP tests using both the tone burst method and the tapping method. Each

[Vertigo and dizziness associated with cerebral hemispheric lesions].

To investigate the clinical significance of vertigo and dizziness in diagnosing cerebral cortical lesion, we analyzed 12 patients with supratentorial lesions whose chief complaint was rotational vertigo or non-rotational dizziness. There were 8 patients with right, and 4 with left hemispheric lesions. The lesion was localized in the vicinity of the central sulcus in 3 patients, temporal and/or parietal regions in 6, and occipital region in 3. Vertigo was seen in three patients, two of them with lesions in the vicinity of the central sulcus, and one in the temporo-parietal junction. The other 9

Scala vestibuli cochlear implantation in patients with partially ossified cochleas.

Partial cochlear obstruction is a relatively common finding in candidates for cochlear implants and frequently involves the inferior segment of the scala tympani in the basal turn of the cochlea. In such patients, the scala vestibuli is often patent and offers an alternative site for implantation. The current report describes two patients with such partial obstruction of the inferior segment of the basal cochlear turn, caused in one case by systemic vasculitis (Takayasu's disease) and in the other by obliterative otosclerosis. A scala vestibuli implantation allowed for complete insertion of th

Vestibular projections in the human cortex.

There is considerable evidence from studies on cats and monkeys that several cortical areas such as area 2v at the tip of the intraparietal sulcus, area 3av in the sulcus centralis, the parietoinsular vestibular cortex adjacent to the posterior insula (PIVC) and area 7 in the inferior parietal lobule are involved in the processing of vestibular information. Microelectrode recordings from these areas have shown that: (1) most of these cortical neurons are connected trisynaptically to the labyrinthine endorgans and (2) they receive converging vestibular, visual and somatosensory inputs. These da

Selective chemical vestibulectomy. Preliminary results.

In an attempt to destroy the peripheral vestibular labyrinth selectively in patients suffering from intractable vertigo as a result of end-stage Ménière's disease, 25 patients underwent selective chemical vestibulectomy between 1989 and 1994. As part of the procedure a certain quantity of streptomycin was placed between the bony and the membranous part of the lateral semicircular canal. The dosage of streptomycin used for successful ablation of vertigo varied between 250 and 100 micrograms. Within 12 months following the procedure all patients experienced a tremendous improvement in their over

[The role of cervical input in vestibular compensation].

The role of proprioceptive input from cervical receptors in vestibular compensation was investigated. Vibratory stimulation to the dorsal neck muscle was given to patients with unilateral vestibular lesions. The length of the displacement of the center of gravity, maximum sway length, and sway area increased markedly during vibratory stimulation to the dorsal neck. In patients in whom compensation had been achieved, a position vector to the side of the lesion increased and the center of gravity moved to the side of the lesion. From these results, it can be speculated that cervical input plays
