Variation in the Use of Chemoradiotherapy for Stage II and III Anal Cancer: Analysis of the National Cancer Data Base.

BACKGROUND: Treatment for anal canal cancer has evolved from radical operations to definitive chemoradiotherapy (CRT), which allows for sphincter preservation in most patients. OBJECTIVE: The aim of this study was to examine the use of CRT for patients with stage II and III anal cancer, among different patient demographics, geographic regions, and facility types. METHODS: Utilizing the National Cancer Data Base, we examined patients with stage II and III anal canal squamous cell carcinoma from 2003 to 2010. Via univariate analysis, we examined patterns of treatment by patient demographics, t

Body image and sexual function in women after treatment for anal and rectal cancer.

OBJECTIVE: Treatment for anal and rectal cancer (ARCa) often results in side effects that directly impact sexual functioning; however, ARCa survivors are an understudied group, and factors contributing to the sexual sequelae are not well understood. Body image problems are distressing and may further exacerbate sexual difficulties, particularly for women. This preliminary study sought to (1) describe body image problems, including sociodemographic and disease/treatment correlates, and (2) examine relations between body image and sexual function. METHODS: For the baseline assessment of a large

Anogenital malignancies in women after renal transplantation over 40 years in a single center.

BACKGROUND: Renal transplant recipients have an increased risk to develop human papillomavirus (HPV)-related anogenital malignancies. A clinical overview of female anogenital posttransplantation malignancies and possible multifocal premalignancies over a period of 40 years renal transplantation is presented. Additionally, the genotype-specific prevalence of HPV in these (pre)malignancies was investigated. METHODS: Data of 1023 women, who underwent a renal transplantation between 1968 and 2008, were collected. Clinical data of all female renal transplant recipients who developed anogenital mal

Amifostine and curative intent chemoradiation for compromised cancer patients.

Concurrent chemotherapy and radiation are usually indicated for locally advanced carcinomas such as head and neck and anal malignancies. Because of the toxicity of the treatment, patient selection plays an important role in recommendations to treat with a curative intent. Elderly patients (> 70 years), those with underlying medical conditions or acquired immunological disorders (AIDS) are commonly excluded from combined modality therapy because of their perceived inability to tolerate the aggressive treatment. They may thus be deprived of a potentially curative therapy. In an attempt to improv

Cancer of the Anal Canal: Diagnosis, Staging and Follow-Up with MRI.

Although a rare disease, anal cancer is increasingly being diagnosed in patients with risk factors, mainly anal infection with the human papilloma virus. Magnetic resonance imaging (MRI) with external phased-array coils is recommended as the imaging modality of choice to grade anal cancers and to evaluate the response assessment after chemoradiotherapy, with a high contrast and good anatomic resolution of the anal canal. MRI provides a performant evaluation of size, extent and signal characteristics of the anal tumor before and after treatment, as well as lymph node involvement and extension t

Human papillomavirus-associated cancers - United States, 2004-2008.

Oncogenic human papillomavirus (HPV) has a causal role in nearly all cervical cancers and in many vulvar, vaginal, penile, anal, and oropharyngeal cancers. Most HPV infections clear within 1-2 years, but those that persist can progress to precancer or cancer. In the United States, public health prevention of cervical cancer includes both secondary prevention through cervical cancer screening and primary prevention through HPV vaccination. Transmission of HPV also can be reduced through condom use and limiting the number of sexual partners. Two vaccines (bivalent and quadrivalent) are available

Melanoma among southwestern American Indians.

All forms of skin cancer are uncommon among southwestern American Indians. The estimated average annual incidence of melanoma is approximately 1 per hundred thousand population or less than one tenth of that currently reported for Anglos (non-Hispanic whites) residing in the same region. This study correlates the clinical and pathologic features of melanoma in 18 American Indian patients. A marked predilection for palms, soles, and subungual locations was displayed. Additionally, three patients presented with mucous membrane primaries and two with ocular melanomas. Advanced disease stage at di

Estimating the clinical benefits of vaccinating boys and girls against HPV-related diseases in Europe.

BACKGROUND: HPV is related to a number of cancer types, causing a considerable burden in both genders in Europe. Female vaccination programs can substantially reduce the incidence of HPV-related diseases in women and, to some extent, men through herd immunity. The objective was to estimate the incremental benefit of vaccinating boys and girls using the quadrivalent HPV vaccine in Europe versus girls-only vaccination. Incremental benefits in terms of reduction in the incidence of HPV 6, 11, 16 and 18-related diseases (including cervical, vaginal, vulvar, anal, penile, and head and neck carcinom

Missed opportunities for anal cancer (AC) screening in women living with HIV: results from a survey across the European region.

OBJECTIVE: The incidence of anal cancer (AC) is higher in women with HIV than in women without HIV due to immunosuppression and persistence of human papilloma virus (HPV). Since 2024, the International Anal Neoplasia Society's and European AIDS Clinical Society (EACS) guidelines recommend annual AC screening of cisgender women (CW) of ≥45 years old, transgender women (TW) of ≥35 years old and women with previous vulvar high-grade squamous intraepithelial lesion (HSIL)/cancer regardless of age. This study describes current clinical practices and protocols for AC screening in women with HIV with

Perianal lesions in men who have sex with men living with HIV: Risk of precancer and cancer.

OBJECTIVES: Men who have sex with men living with HIV (MSMLWH) are at highest risk for human papillomavirus (HPV)-associated anal cancer, which may originate from the anal canal, verge or perianal skin. Perianal lesions are frequently overlooked during examination, and their (pre)malignant burden in this population remains poorly characterized. METHODS: A total of 308 MSMLWH who underwent high-resolution anoscopy (HRA)-guided perianal biopsy between 2018 and 2024 were analysed. Demographics, clinical HIV parameters, smoking history, HPV vaccination status, anal cytology, high-risk HPV test re

Five-Year Evaluation of an Anal Cancer Screening Program in Men Who Have Sex With Men With HIV at 2 Academic Center Clinics.

BACKGROUND: Guidelines recommend annual anal cytology-based squamous cell carcinoma of anus (SCCA) screening for men who have sex with men with HIV aged ≥35 years (eligible population). The recommended threshold for high-resolution anoscopy (HRA) depends on its availability: low-threshold (any abnormal cytology) if availability is high and high-threshold (high-grade squamous intraepithelial lesion [HSIL] on cytology) if availability is low. METHODS: This was a retrospective chart review (2018-2022) at academic HIV clinics. We evaluated (1) 5-year uptake of cytology-based SCCA screening in the

Molecular Insights Into the Natural History of Anal HSIL.

Anal squamous cell carcinoma is commonly associated with human papillomavirus (HPV) infection and preceded by low- and high-grade anal lesions (LSIL; HSIL). We performed a molecular comparison on paired LSIL- and HSIL-lesions collected in a longitudinal fashion to assess their relationship. Fifty biopsies from 22 men diagnosed with LSIL at baseline (T0) who developed HSIL during follow-up (T1) were subjected to a comprehensive molecular analysis: HPV-typing and HPV16 variant, cellular DNA methylation levels, and copy number aberrations (CNA). After histopathological revision, 23 biopsies were

Impact of race, ethnicity, and human immunodeficiency virus status on anal high-risk HPV subtypes: Preliminary insights from a diverse urban population.

INTRODUCTION: Racial differences have been identified in the distribution of cervical high-risk human papillomavirus (hrHPV) subtypes; however, there is limited understanding of hrHPV subtypes in anal specimens based on patient race/ethnicity. This knowledge gap limits possible vaccination and/or treatment efforts and may not provide optimal coverage in diverse populations. MATERIALS AND METHODS: This preliminary study evaluates anal hrHPV subtype distribution and cytological outcomes in a diverse population accessing care at a large, urban, publicly funded hospital over a 2-year period. Detection of PIK3CA Mutations in Anal Dysplasia.

INTRODUCTION: The incidence of anal cancer is increasing despite screening and treatment options for anal dysplasia, the precursor to anal cancer. Once anal dysplasia is identified, predicting which patients are at the highest risk of progressing to anal cancer remains challenging, as there are no molecular biomarkers for risk stratification. The most common mutation in anal cancer affects the catalytic subunit of Phosphatidylinositol (3, 4, 5)-trisphosphate Kinase (PI3K). We sought to determine if PIK3CA mutations are detectable in precancerous anal lesions. METHODS: DNA was extracted from f
