First experience with ATOMS system implant in neurogenic stress urinary incontinence.

AIM: To evaluate efficacy and safety of ATOMS implant in neurogenic patients with stress incontinence performing clean intermittent catheterization (CIC). METHODS: We included all patients with neurogenic sacral/subsacral lesion and stress urinary incontinence, treated with ATOMS implant between January 2018 and March 2019. All patients received anamnesis, 24-hour pad test and pad count, physical examination, video urodynamic evaluation, Qualiveen questionnaire. All patients were followed up at 12 months after implantation. Patients were considered "continent" when dry or when wearing a secur

Tight filum terminale syndrome in children: analysis based on positioning of the conus and absence or presence of lumbosacral lipoma.

BACKGROUND: Tight filum terminale syndrome (TFTS) characterized by findings consistent with a tethered cord but with the conus ending in a normal position has only recently been observed in children. In this situation, diagnosis may prove difficult and sectioning of the filum terminale is questionable. MATERIALS AND METHODS: Sixty cases of pediatric TFTS were analyzed by methods including spinal X-ray and magnetic resonance imaging (MRI). Twenty-one patients exhibited a normally positioned conus, 18 a low-lying conus, and 21 a low-lying conus with accompanying lumbosacral lipoma. These three

Patients with urinary incontinence often benefit from surgical detethering of tight filum terminale.

We retrospectively reviewed 77 patients with a tethered spinal cord syndrome to evaluate the results of neurosurgical treatment. The patients were divided into two groups: in group 1 there were 17 patients with primary tethered cord who had normal level conus medullaris (NLCM) and normal thickness filum terminale (NTFT) with urinary incontinence, and group 2 was made up of 60 patients with secondary spinal cord tethering after a previous closure of a midline fusion defect who had a low-lying conus medullaris. Neurological examination, radiography, urodynamic tests and electrophysiological find

Intravascular papillary endothelial hyperplasia causing cauda equina compression: case report.

OBJECTIVE AND IMPORTANCE: Intravascular papillary endothelial hyperplasia (Masson's vegetant hemangioendothelioma) is a rare condition affecting the neuraxis. In the literature, only one case of this lesion involving the vertebral canal with spinal cord compression has been reported. We present a case of cauda equina compression due to this abnormality. CLINICAL PRESENTATION: A 17-year-old boy was admitted at our hospital with pain, numbness, paresis of the left lower extremity, and bladder dysfunction of approximately 1 month's duration. Computed tomography and magnetic resonance imaging of

Clinical analysis of two-level compression of the cauda equina and the nerve roots in lumbar spinal canal stenosis.

STUDY DESIGN: This study is a prospective, clinical study assessing the efficacy of selective decompression of the responsible level in two-level stenosis in accordance with neurologic findings defined by the gait load test, and functional diagnosis based on selective nerve root block. OBJECTIVE: To clarify the clinical features of two-level stenosis regarding the neurologic level responsible for the symptoms, neurogenic intermittent claudication, and the outcome of selective decompression. SUMMARY OF BACKGROUND DATA: Experimental studies have indicated that double-level compression of the c

Surgical treatment of degenerative spondylolisthesis in the lumbar spine: no reposition without prior decompression.

In the present note, the cause of root compression in cases with degenerative lumbar spondylolisthesis is discussed in order to clarify the best method of surgery and to avoid iatrogenic damage of the cauda equina if a transpedicular fusion is going to be performed. The root compression is due to osteophytes growing out from the upper articular facets and from the upper ridge of the lamina of the lower vertebra. Therefore, the repositioning of the anteriorly displaced vertebra by the instrumentation could lead to the squeezing of the cauda equina between the withdrawn vertebral body and the de

[Venous pressure in the vertebral venous plexus and its role in cauda equina claudication].

The venous pressure in the ascending lumbar vein was measured by catheter and pressure transducer to document venous stasis in degenerative lumbar spinal canal stenosis. Measurement of the pressure in the ascending lumbar vein at rest revealed a progressive rise of the mean pressure from the control group to the lumbar intervertebral disc herniation group, and the lumbar spinal canal stenosis (LSCS) group in ascending order. Pressure curve tracings showed a plateau accompanying increased abdominal pressure during the Valsalva maneuver. Phase 3 was defined as the recovery period of decreasing p

Traumatic Isolated Lumbosacral Spinal Subdural Hematoma in a Pediatric Patient: A Case Report and Literature Review.

Spinal subdural hematomas (sSDHs) are rare, particularly in the pediatric population and in the context of trauma without coexisting intracr … rauma, the patient presented with back pain, limiting ambulation without focal deficits or signs of cauda equina syndrome. Magnetic resonance imaging demonstrated a subdural fluid collection from L1-S2 spinal levels and "inverted Mercedes-Benz" sign consistent with sSDH. The patient was managed conservatively with steroids and close observation. At her three-month foll …

Comparative Analysis of Artificial Intelligence Platforms: ChatGPT-3.5 and GoogleBard in Identifying Red Flags of Low Back Pain.

BACKGROUND: Low back pain (LBP) is a prevalent healthcare concern that is frequently responsive to conservative treatment. However, it can also stem from severe conditions, marked by 'red flags' (RF) such as malignancy, cauda equina syndrome, fractures, infections, spondyloarthropathies, and aneurysm rupture, which physicians should be vigilant about. Given the increasing reliance on online health information, this study assessed ChatGPT-3.5's (OpenAI, San Francisco, CA, USA) and GoogleBard's (Google, Mountain View, CA, USA) accuracy in responding to RF-related LBP questions and their capacity…

Risk factors for spinal subdural hematoma after minimally invasive transforaminal lumbar Interbody Fusion (MI-TLIF): a multivariate analysis.

BACKGROUND: Spinal subdural hematoma (SSH) is a rare cause of compression of the neutral elements of the spinal cord. However, little is kno … ne of the patients presented with serious neurologic symptoms, such as lower extremity paralysis or cauda equina syndrome that required emergency hematoma debridement. SSH was found in 17 (7.9%) patients and non-SSH in 198 (92.1%). Factors affecting SSH were presence of hypertension, presence of diabetes and postoperative anticoagulant therapy. The significantly indepe …

The Effect of Thunder-Fire Moxibustion on Lumbar Disc Herniation: Study Protocol for a Randomized Controlled Trial.

BACKGROUND: Lumbar disc herniation (LDH) is a common disease seen in orthopedics; it is caused by nucleus pulposus herniation. Its clinical manifestations are low back pain, radiating pain of the lower limbs, and cauda equina symptoms that seriously affect patients' quality of life. At present, oral analgesics are commonly used in the treatment of LDH; but they can produce gastrointestinal reactions and other side effects. Thunder-fire moxibustion is a method that is widely used in China to treat pain syndromes. This study aimed to design a randomized controlled trial to explore the effectiven

Incidence and Skeletal Features of Developmental Cervical and Lumbar Spinal Stenosis.

STUDY DESIGN: Cross-sectional study. PURPOSE: To report the prevalence and relationship of developmental cervical and lumbar spinal stenosis (DCSS and DLSS) with the bone parameters of the skull and extremities. OVERVIEW OF LITERATURE: DCSS and DLSS are found occasionally in daily practice. DCSS and DLSS can cause compressive myelopathy, radiculopathy, and cauda equina syndrome; however, data on their prevalence and correlation with skull and skeletal extremity profiles is scarce. METHODS: A cross-sectional measurement study of 293 whole-body dried-bone samples was conducted. We measured th…

Compliance With the Cauda Equina Pathway: Results of a Closed-Loop Audit.

Introduction The British Association of Spine Surgeons (BASS) and Society of British Neurological Surgeons (SBNS) recommend urgent MRI imaging and operative intervention in patients with suspected cauda equina syndrome (CES). Due to the lack of a 24-hour MRI service and the centralisation of neurosurgery to large tertiary centres, there is a need for an evidence-based protocol for the referral of patients presenting with back pain, with red flags to specialist tertiary neurosurgical centres. Methods The standard operating procedure (SOP) at our local hospital outlines steps in the assessment, …

Lumbosacral intrathecal nerve roots: an anatomical study.

BACKGROUND: The lumbosacral intrathecal anatomy is complex because of the density of nerve roots in the cauda equina. Space-occupying lesions, including disc herniation, trauma and tumor, within the spinal canal may compromise the nerve roots, causing severe clinical syndromes. The goal of this study is to provide spinal surgeons with a detailed anatomical description of the intrathecal nerve roots and to emphasize their clinical importance. METHOD: Ten formalin-fixed male cadavers were studied. They were dissected with the aid of a surgical microscope, and measurements were performed. RESUL

Pelvic Nerve Injury Causes a Rapid Decrease in Expression of Choline Acetyltransferase and Upregulation of c-Jun and ATF-3 in a Distinct Population of Sacral Preganglionic Neurons.

Autonomic regulation of the urogenital organs is impaired by injuries sustained during pelvic surgery or compression of lumbosacral spinal nerves (e.g., cauda equina syndrome). To understand the impact of injury on both sympathetic and parasympathetic components of this nerve supply, we performed an experimental surgical and immunohistochemical study on adult male rats, where the structure of this complex part of the nervous system has been well defined. We performed unilateral transection of pelvic or hypogastric nerves and analyzed relevant regions of lumbar and sacral spinal cord, up to 4 w…
