Prolongation of monophasic action potential duration and the refractory period in the human heart by tedisamil, a new potassium-blocking agent.

The effect of intravenous tedisamil (0.3 mg.kg-1), a newly developed potassium-blocking agent, on ventricular repolarization was studied in 10 patients (three women, seven men; mean age 53 +/- 8 years) with coronary artery disease (stenoses < or = 60%). Left ventricular monophasic action potentials, effective refractory periods and surface electrocardiograms were recorded during sinus rhythm and during constant atrial pacing at cycle lengths of 600, 500 and 400 ms. Under tedisamil there was a 12% reduction of heart rate and in parallel a prolongation of QTc interval (+10%) and left ventricular

Solvent-free engineering of a co-amorphous efavirenz-ritonavir system by hot-melt extrusion: Solid-state stabilisation and improved bioavailability.

Combination antiretroviral therapy remains the most successful approach for effective HIV management, as the simultaneous targeting of multiple stages of the viral replication cycle improves therapeutic efficacy and limits the emergence of drug resistance. Efavirenz (EFV) and ritonavir (RTV) form a mechanistically complementary drug pair; however, both exhibit poor aqueous solubility, leading to dissolution-limited oral absorption. In this work, a dose- and mechanism-informed co-amorphous formulation strategy was developed using hot-melt extrusion (HME) to improve the biopharmaceutical perform

Real-world safety profile of roflumilast: a pharmacovigilance analysis using FDA adverse event reporting system and Canada vigilance database.

BACKGROUND: Roflumilast, a highly selective phosphodiesterase 4 inhibitor, is used to treat with chronic obstructive pulmonary disease and psoriasis. We aim to determine potential roflumilast-associated adverse events (AEs) and the differences in AE signals among diverse populations. METHODS: Roflumilast's AE reports between the first quarter of 2011 and the fourth quarter of 2024 were obtained from the FDA Adverse Event Reporting System (FAERS) and Canada Vigilance Adverse Reaction Database (CVARD). The signal strength was measured by four disproportionality analysis methods, including Repor

Beyond corticosteroids: A systematic review and meta-analysis of novel therapies for hypersensitivity pneumonitis.

INTRODUCTION: Hypersensitivity pneumonitis (HP) is a complex immune-mediated interstitial lung disease triggered by inhaled antigens. While corticosteroids remain the mainstay of therapy, their benefit is limited in fibrotic HP. Several novel agents-including pirfenidone, nintedanib, rituximab, azathioprine, mycophenolate mofetil (MMF), leflunomide, and roflumilast-have been investigated as alternatives. METHODS: This meta-analysis followed PRISMA guidelines and was registered on PROSPERO (CRD42023428070). Eligible studies included clinical and non-clinical trials assessing both traditional a

Roflumilast inhibits neuronal ferroptosis via AMPK/Nrf2/HO-1 signaling and promotes motor function recovery after spinal cord injury in rats.

Spinal cord injury (SCI) is a serious central nervous system disease. Ferroptosis is one of the major causes of spinal cord neurological loss, and targeting ferroptosis is a promising therapeutic strategy. Roflumilast has shown promising applications in the treatment of neurological diseases due to its potent anti-inflammatory and anti-oxidative stress effects. This study aimed to investigate whether roflumilast could inhibit neuronal ferroptosis to improve motor function after SCI in rats. In vitro experiments, we found that roflumilast significantly increased cell survival in an in vitro fer

A single-arm phase IIIb study of 8-week glecaprevir/pibrentasvir treatment in adults with acute hepatitis C.

BACKGROUND & AIMS: With over 1 million new HCV infections in 2022, treating acute HCV is essential to achieve WHO's HCV elimination goal. Although guidelines recommend prompt treatment, no direct-acting antiviral is approved for acute HCV outside of the US, leading to loss of patients to care while waiting for chronicity, and increased onward transmission. This single-arm, phase IIIb study evaluated the efficacy and safety of an 8-week glecaprevir/pibrentasvir regimen in treatment-naïve adults with acute HCV. METHODS: Analyzed populations included intention-to-treat (ITT; all patients receivi

Montelukast improves disease outcome in SOD1G93A female mice by counteracting oligodendrocyte dysfunction and aberrant glial reactivity.

BACKGROUND AND PURPOSE: Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disorder characterized by progressive motor neuron (MN) loss and consequent muscle atrophy, for which no effective therapies are available. Recent findings reveal that disease progression is fuelled by early aberrant neuroinflammation and the loss of oligodendrocytes with neuroprotective and remyelinating properties. On this basis, pharmacological interventions capable of restoring a pro-regenerative local milieu and re-establish proper oligodendrocyte functions may be beneficial. EXPERIMENTAL APPROACH: H

Pharmacokinetics and Bioequivalence of Two Formulations of Montelukast Sodium Tablets in Healthy Chinese Volunteers Under Fasting and Fed Conditions.

Montelukast sodium is a leukotriene type 1 receptor antagonist that can be used for the prophylaxis and treatment of asthma. However, the pharmacokinetics of montelukast sodium tablets (10 mg) remain unclear in healthy Chinese subjects. Here, a single-dose randomized, open-label, 2-sequence, and 2-period crossover (7-day washout period between treatments) study was performed to compare the pharmacokinetics and bioequivalence between the test products and the reference at a single dose of 10 mg among healthy Chinese subjects under fasting and fed conditions. Blood samples were collected at spec

Glucocorticoid Receptor α Mediates Roflumilast's Ability to Restore Dexamethasone Sensitivity in COPD.

The angiotensin-converting enzyme (ACE)/Angiotensin II (Ang II) and angiotensin-converting enzyme 2 (ACE2)/angiotensin-(1-7) (Ang-(1-7)) pathways are coexpressed in most tissues. The balance between these pathways determines, at least in part, whether tissue damage will occur in response to pathological stimuli. The present study tested the hypothesis that male sex and high blood pressure are associated with ACE/ACE2 imbalance in the lungs. Experiments were conducted in male and female Wistar rats and spontaneously hypertensive rats (SHRs). Lung ACE and ACE2 gene expression was also evaluated

Effects of anti-allergic drugs on T cell-mediated nasal hyperresponsiveness in a murine model of allergic rhinitis.

BACKGROUND: We have recently demonstrated that T cell-mediated nasal hyperresponsiveness (NHR) is a representative pathophysiological feature of allergic rhinitis (AR). Although several anti-allergic drugs are used for the treatment of AR, the efficacy of these drugs on T cell-mediated NHR have not been elucidated. In these studies we investigated the effects of dexamethasone (Dex), montelukast (Mk), and chlorpheniramine (Chl) on NHR in antigen-immunized and antigen-specific Th2 cell-transferred mice. METHODS: OVA-immunized BALB/c mice were treated with Dex, Mk, or Chl and challenged intranas
