The effects of long-term amiodarone therapy on the defibrillation thresholds and the rate of shocks were evaluated in 62 patients who had implantation of an automatic cardioverter-defibrillator (n = 53) or prophylactic implantation of patch electrodes (n = 9) who were survivors of sudden cardiac death (n = 34) or had refractory rapid ventricular tachycardia (n = 28). UNLABELLED: The clinical course of 140 patients with Wolff-Parkinson-White syndrome who had their initial episode of supraventricular tachycardia before 18 years of age was reviewed. Among those whose tachycardia began at age 0 to 2 months, it disappeared in 93% and persisted in 7%. In 31%, it disappeared and reappeared at an average age of 8 years. Among patients whose tachycardia was present after age 5 years, it was persistent in 78% at a mean follow-up period of 7 years. Patients with supraventricular reciprocating tachycardias or atrial fibrillation with rapid ventricular response mediated over an accessory pathway may be difficult to treat. The records of 90 patients with Wolff-Parkinson-White syndrome who presented with supraventricular tachycardia in the first 4 months of life were reviewed. Among these, 63% were male. Structural heart disease was present in 20%, most commonly Ebstein's anomaly. All patients presented with a regular narrow QRS tachycardia, and pre-excitation became evident only when normal sinus rhythm was established. In some cases undergoing radiofrequency catheter ablation (RFCA) of accessory pathway (AP), a Mahaim-like right-sided atrioventricular AP (M-AP) showing slow and decremental conduction is observed. We have discussed the evolution of selected areas of echocardiography in an attempt to illustrate the capabilities and limitations of this method of study. As information has accumulated, some concepts of the specificity of certain echocardiographic patterns have had to be revised.
