BACKGROUND: Abdominal computed tomographic (CT) scan is accepted as the primary diagnostic modality in stable patients with blunt abdominal trauma. A recent survey of 328 trauma surgeons demonstrated marked variation in the management of patients with head injuries and the finding of free intra-abdominal fluid without solid organ injury on CT scan. This study was undertaken to attempt to determine what to do when free fluid without solid organ injury is seen on abdominal CT scan in patients with blunt trauma.

UNLABELLED: From 1992 to 1995, 1565 laparoscopic operations were performed in Roubaix. We report on six complications.
OBJECTIVE: To compare this figure with these of the French collaborative study done from 1987 to 1991 by seven departments known for their laparoscopic activity, including Roubaix.
RESULTS: The trends are good. The rate of injuries was stable in spite of increasingly complex procedures. The largest portion of visceral and even vascular injuries were repaired by laparoscopy reducing by the same amount use of laparotomy.

Besides blunt and penetrating trauma, many diseases spontaneously produce abdominal hemorrhage. Here the term "spontaneous" implies the lack of observable injury, but the likelihood that unrecognized trauma initiates blood loss which continues unabated when clotting factors are absent or depleted. Survival of the patient often depends on rapid and accurate diagnosis.
