BACKGROUND: Clinical outcomes of terrible triad injuries (TTIs) of the elbow are historically poor. To date, it is still debatable whether the coronoid needs to be fixed and if so, how and in which sequence.
METHODOLOGY: Between 2010 and 2013, 13 patients were treated surgically for acute TTIs of the elbow at a Tertiary Level 1 Trauma Centre by a single surgeon, using a standardized protocol, which included coronoid-brachialis complex fixation via pull-through trans-osseous sutures, radial head fixation or prosthetic replacement and a repair of the lateral ulnar collateral ligament. Repair of

BACKGROUND: We wished to determine the correlation between supraspinatus muscle occupation ratio and reparability of rotator cuff muscles and that of each occupation ratio with 1-muscle, 2-muscle, and 3-muscle rotator cuff injury.
METHODS: We evaluated 170 patients (average age, 62.3 [40-83] years) with complete (type II) or incomplete (type III) rotator cuff repair by arthroscopy. Type II repair was defined as complete repair but less optimal coverage of the entire medial-lateral footprint. Type III repair was defined as incomplete repair; a small portion (<10 mm) of the humeral head is expo…

BACKGROUND: Understanding the mechanisms of shoulder impingement created by clinical tests is crucial to accurately evaluate the condition. The objective of this study was to relate mechanisms of subacromial and coracoid impingement occurring in positions of the shoulder during clinical examination, in quantitative and qualitative terms.
METHODS: A 1.0T open magnetic resonance imaging system was used in 18 female and 19 male subjects, to determine the distances between the humeral head and the acromion or coracoid, and contact with the rotator cuff (RC). Measurements were taken with the shoul…

OBJECTIVE: To investigate the effectiveness of open reduction and internal fixation with locking anatomical plate for treating valgus impact … eduction and no loosening of internal fixation were observed. At 10-12 months, osteonecrosis of the humeral head occurred in 3 cases (7.9%), including 2 cases of Cruess stage III and 1 case of Cruess stage IV. At last follow-up, the Constant shoulder joint scores were 56-95 (mean, 82.6); the results were excellent in 10 cases, good in 15 cases, fair in 9 cases …

BACKGROUND: Coracoid bone graft osteolysis and fibrous union are the principal causes of failure in patients treated with the Latarjet proce … al technique were the same for both groups, we believe that the mechanotransduction effect from the humeral head on the graft influences its remodeling.
CONCLUSION: The results of this study suggest that the bone graft part of the Latarjet procedure plays a role in patients with significant coracoid bone loss but much less so when there is no bone loss.

BACKGROUND: Adult literature cites an axillary nerve danger zone of 5 to 7 cm distal to the acromion tip for open or percutaneous shoulder s … tifiable intraoperative surface and radiographic landmarks, including the acromion tip, apex of the humeral head, lateral physis, and central apex of the physis. The physeal apex height (i.e., 1 "mountain") was defined as the vertical distance between the most lateral point of the humeral physis (LPHP) and the central intraosseous apex of the physis.

Pathology of the long head of the biceps tendon (LHBT) is a common source of anterior shoulder pain and is frequently encountered during arthroscopic shoulder surgery. Although biceps tenodesis is widely performed, many existing techniques rely on implants that increase cost and may be associated with bone- or fixation-related complications. This paper describes a novel arthroscopic intracapsular soft-tissue biceps tenodesis technique emphasizing efficiency, reproducibility, and implant minimization. The LHBT is pierced percutaneously using a spinal needle approximately 1 cm proximal and later

Quadrilateral Space Syndrome (QSS) is a rare clinical entity characterised by neurovascular compression within the quadrilateral space, a confined anatomical region bounded by the teres minor, teres major, long head of the triceps brachii, and the surgical neck of the humerus. The syndrome primarily affects the axillary nerve and posterior circumflex humeral artery (PCHA), presenting with nonspecific symptoms such as shoulder pain, paresthesia, and muscle weakness. Imaging plays a pivotal role in diagnosing QSS, with magnetic resonance imaging (MRI) serving as the gold standard. Ultrasound com

Fractures occurring in the elbow region of children and adolescents pose a notable challenge for orthopedic surgeons due to the complex anatomy and developmental stages of the humerus. Capitulum fractures, a rare form of elbow injury, are classified into three types according to the Bryan and Morrey classification system. Monteggia fractures, a rare occurrence in children, involve a combination of ulna break and radial head dislocation, with Monteggia equivalent fractures expanding upon traditional classifications. The distinction lies in Monteggia equivalent fractures (MEFs) not always involv

Rotator cuff tears stand as the most prevalent shoulder condition prompting medical intervention. Failure rates as high as 60% have been rep … s. Using those implants in addition to sutures improved the macroscopic attachment of tendon to the humeral head at early time points (2 and 4 weeks), led to a faster improvement of the tendons' mechanical properties (p < 0.05), and inhibited the development of heterotopic ossification (p < 0.01). Rotator cuff regeneration using CS-FIIa-PRP implants is, theref …

The natural history of midsubstance capsular tears (MCTs) is unclear. We herein describe a case of MCT observed using serial magnetic resona … ded to have surgery. Arthroscopy revealed a large capsular defect extending from the glenoid to the humeral head in the anterior inferior glenohumeral ligamentous complex. The MCT was repaired with the placement of nonabsorbable sutures in a side-to-side fashion. At the final follow-up, three years postoperatively, the patient had no anterior shoulder instabil …

INTRODUCTION: Shoulder dislocation is very common. However, neglected or chronic shoulder dislocations are extremely rare. The position of the humeral head determines the classification of shoulder dislocation. Anterior shoulder dislocation accounts for most cases, while inferior dislocation is rare. Negligence from the patient and misdiagnosis are the leading causes of chronic shoulder dislocation. Treatment of the condition poses a significant challenge for surgeons since there is no widely accepted treatment protocol.

The ideal treatment approach to massive, irreparable rotator cuff tears remains a hot topic of debate. In addition to joint-preserving metho … ween the remaining anterior and posterior portions of rotator cuff and as a static depressor of the humeral head during shoulder motion. With adequate graft quality, compatibility (i.e., fascia lata autograft), and thickness (minimum 5 mm), SCR may reliably maintain these functions. Tendon transfers are best reserved for the limited subgroup of patients where …

Fractures of the lesser tuberosity of the humerus are typically traumatic in nature, most commonly occurring in association with multi-part fractures of the humeral head, often in the setting of a posterior shoulder dislocation. Isolated fractures of the lesser tuberosity are considerably more rare and are difficult to diagnose on standard shoulder radiographs without an axillary view. These fractures have been associated with 3 main types of injury: acute abduction/external rotation injury, acute injury or repetitive stress injury in adolescent overhead or throwing athletes, and rarely as a s…
