[Comparison of blood leptin concentration and colonic mucosa leptin expression in colon adenoma patients and healthy control].

BACKGROUND/AIMS: Obesity increases the risk of colorectal cancer and adenomatous polyp, and one of the underlying mechanisms of this increase is considered to be due to the growth promoting effects of adipokines, such as leptin. In order to investigate this finding, leptin expression in the colonic tissue and blood leptin concentration of the colonic adenoma patients were compared to those of the control group. METHODS: Colonic adenoma tissues were obtained by polypectomy (n=60). In these patients, normal colonic mucosa at remote areas from the polyp was also obtained and blood samples were c

Intestinal bleeding from arteriovenous malformations of the small bowel in a patient with Cowden syndrome: report of a case.

This report presents the case of a patient with Cowden syndrome who had arteriovenous malformations (AVMs) at the jejunum and the ileum and experienced intestinal bleeding. A 54-year-old Japanese male presented with general fatigue and melena. Endoscopic examinations showed gastrointestinal polyposis from the esophagus to the rectum. However, the site of bleeding was not identified. There were some papules on his face and neck. He also had macrocephaly and had multiple papillomas along the gum-line. These findings indicated a clinical diagnosis of Cowden syndrome. Enhanced computed tomography

Cronkhite Canada syndrome.

Cronkhite Canada syndrome is an acquired non-familial syndrome characterised by diffuse gastrointestinal polyposis with alopecia nail dystrophy and hyperpigmentation. There is chronic diarrhoea and protein losing enteropathy. The etiology of this syndrome remains obscure. The rarity of the case prompts this case report.

Sigmoidoscopy in minor lower gastrointestinal bleeding.

The role of sigmoidoscopic examination in the diagnosis, evaluation, and treatment of minor lower gastrointestinal bleeding was investigated. A hundred patients with minor rectal bleeding were examined by rigid sigmoidoscopy under general anaesthesia between January 1989 and July 1996. Patients who had bleeding secondary to infections, anal fissure, or haemorrhoids were excluded from study. Patients were reviewed retrospectively according to their diagnosis and endoscopic and histopathological findings. Twenty nine of these patients were girls and 71 boys; their ages ranged between 8 month and

Utility of flexible sigmoidoscopy as an adjunct to double-contrast barium enema examination.

BACKGROUND: There is controversy regarding the most appropriate investigation for suspected colorectal carcinoma. We offered these patients same-day flexible sigmoidoscopy (FS) and double-contrast barium enema (DCBE). METHODS: We reviewed the results of 117 consecutive adult patients. All patients underwent FS followed by DCBE on the same day. The radiographs were reviewed by two of the authors who were blinded to the clinical information, flexible sigmoidoscopy reports, and the original DCBE report. RESULTS: One hundred seventeen patients made up the study population. Thirty-four of the 117

Superior mesenteric and portal vein thrombosis following laparoscopic-assisted right hemicolectomy. Report of a case.

PURPOSE: This article describes a case of superior mesenteric and portal vein thrombosis following laparoscopic-assisted right hemicolectomy. METHODS: A retrospective case review was performed. RESULTS: Data continue to grow regarding safety and technical feasibility of laparoscopic-assisted colectomy. As this minimally invasive alternative to open colonic resection becomes more popular, it is inevitable that information on benefits and complications associated with it will continue to expand. We report a case of superior mesenteric and portal vein thrombosis following laparoscopic-assisted

[Proposal for a radiological classification of multiple adenomatosis of the colon].

The double contrast enemas of 47 patients with multiple adenomatosis of the colon and rectum were analyzed. According to the density and the distribution of polyps on the mucosal surface, the radiological patterns were classified into three different groups: sporadic, dense and confluent adenomas. Radiographic patterns of each patient were related to the clinical and pathological data collected during the diagnosis and the follow-up. The clinical and pathological features and the evolution of the disease appear to be different in each group of patients (identified by the radiological classific

Colon and anorectal neoplasms in a tropical African population.

At the University College Hospital, Ibadan, Nigeria, over a 30-month period (January 1975 to June 1977) more than 29 cases were fully studied. Nine patients presented with acute large bowel obstruction. In five cases, colonic neoplasms were palpated abdominally before surgery, and four of the patients were initially being treated for "hemorrhoids". All the patients presented at late stages. There was neither socio-economic nor sexual bias in this affliction. Sixty-nine percent of all the tumors occurred in the anorectal area well within the reach of the index finger, and 27.5% occurred in the

Colonic neoplasms complicating ureterosigmoidostomy.

A benign reactive polyp developed at each ureteral implantation in the sigmoid colon following bilateral ureterosigmoidostomy for exstrophy of the bladder. The first polyp was discovered 23 years after the operation. The second polyp occurred 4 years later at the other ureteral orifice. Each polyp contained benign cystic glandular epithelium within the submucosa and the stalk. These submucosal cysts resembled colitis cystica profunda, except that they were confined to the pedunculated polyps. A complete review of the literature of neoplasms complicating ureterosigmoidostomy is presented. It is

Effect of Combination of a Mucosal Exposure Device and Computer-Aided Detection in Diagnostic, Screening, and Surveillance Colonoscopy: An International, Multicenter Study.

INTRODUCTION: Mucosal exposure devices (MEDs) and computer-aided detection (CADe) systems may both improve adenoma detection through distinct mechanisms: expanding mucosal visualization and highlighting lesions, respectively. This study investigated the efficacy of combining CADe-assisted colonoscopy with a MED compared with CADe-assisted colonoscopy alone. METHODS: This international, multicenter, prospective, nonrandomized, single-arm study (NTC05220345) was conducted at 3 centers that also participated in the previous DISCOVERY II randomized controlled trial, comparing CADe-assisted with c

Clinical implications of a machine learning model predicting colorectal polyp recurrence after endoscopic mucosal resection.

The machine learning model developed by Shi et al for predicting colorectal polyp recurrence after endoscopic mucosal resection represents a significant advancement in the field of clinical gastroenterology. By integrating patient-specific factors, such as age, smoking history, and Helicobacter pylori infection, the eXtreme Gradient Boosting algorithm enables precise personalised colonoscopy follow-up planning and risk assessment. This predictive tool offers substantial benefits by optimising surveillance intervals and directing healthcare resources more efficiently toward high-risk individual
