Functional language performance outcomes on paediatric cochlear implant patients in Malawi.

Objectives: To assess functional language performance on paediatric cochlear implant patients in Malawi. Methods: Sixteen patients who received cochlear implantation before the age of 18 years from a single site were invited for an interview. Parents or guardians of twelve of these participated in the PEACH and BAPP questionnaires, whilst four others who had become adults answered independently. The PEACH scale is a 13-question survey with a maximum score of 52. The BAPP score has four domains with a total score of 16. Results: Most patients acquired profound hearing loss through either mumps,…

Language-based labeling modulates oscillatory neural dynamics of social pain processing.

The neurocognitive mechanisms underlying language-based implicit modulation of social pain remain poorly understood. To examine the neural dynamics associated with this process, we recorded electroencephalography (EEG) from 39 healthy adults completing a three-condition labeling task: social exclusion labeling, affect labeling, and gender labeling. Results showed that social exclusion labeling produced stronger modulation of social pain-related neural responses than affect labeling, as reflected in reduced N400 and P300 amplitudes, suggesting a more efficient allocation of cognitive resources …

Lung cancer research: how natural language processing enhances clinical data extraction.

BACKGROUND: Lung cancer remains one of the leading causes of cancer-related mortality worldwide, with poor survival rates largely due to late diagnosis and the aggressive nature of the disease. Accurate and efficient extraction of clinical data is critical to improving research and patient outcomes. Traditional manual data collection methods, while reliable, are time consuming and prone to error. In this study, we evaluate the effectiveness of natural language processing (NLP) techniques in extracting relevant clinical data from unstructured medical records and compare them to manual data entr…

Benchmarking large language models for cell-free RNA diagnostic biomarker discovery.

Large language models can synthesize biomedical knowledge, parse vast amounts of data, and generate code, positioning them as promising tools for biomarker discovery from high-throughput omics data. Here, we benchmark six models from OpenAI, Anthropic, and Google on plasma cell-free RNA datasets spanning three clinical cohorts: Kawasaki disease versus multisystem inflammatory syndrome in children, active tuberculosis versus symptomatic respiratory controls, and myalgic encephalomyelitis/chronic fatigue syndrome versus sedentary controls. We evaluate literature-guided nomination of diagnostic g…

Large Language Models Versus Multidisciplinary Tumor Board Decisions in Thyroid Cancer.

OBJECTIVES: Large language models (LLMs) are increasingly proposed as clinical decision-support tools; however, their agreement with real-world multidisciplinary tumor board (MDT) decisions remains insufficiently investigated in thyroid oncology. To evaluate the concordance between treatment recommendations generated by ChatGPT 5.2 and Gemini 3.0 and decisions made by a tertiary multidisciplinary thyroid tumor board. METHODS: This study included 59 consecutive patients discussed at a tertiary MDT between August and December 2025. Anonymized clinical data, including demographics, ultrasonograp…

Target sequence-conditioned design of peptide binders using masked language modeling.

The computational design of protein-based binders presents unique opportunities to access 'undruggable' targets, but effective binder design often relies on stable three-dimensional structures or structure-influenced latent spaces. Here we introduce PepMLM, a target sequence-conditioned designer of de novo linear peptide binders. Using a masking strategy that positions cognate peptide sequences at the C terminus of target protein sequences, PepMLM finetunes the ESM-2 protein language model to fully reconstruct the binder region, achieving low perplexities matching or improving upon validated p…

No foreign language effect in Schizotypy: Evidence from German-English bilinguals.

Previous research has suggested that fewer schizophrenic and schizotypal traits are reported in a second language than a mother tongue. Such results make sense in the light of the so-called Foreign Language Effect (FLE), whereby bilinguals make more rational decisions and are less influenced by emotions and biases in a learned second language (L2) than a mother tongue (L1). However, this previous research is to date very limited, and apart from one large-scale quantitative study is based primarily on anecdotal evidence. In the study reported here, we gave German-English bilinguals the Schizoty…

Estimating depression severity in narrative clinical notes using large language models.

BACKGROUND: Depression treatment guidelines emphasize measurement-based care using patient-reported outcome measures, yet their impact on narrative documentation quality remains underexplored. METHODS: We sampled 15,000 narrative clinical outpatient notes from the electronic health record of a large academic medical center, reflecting visits between January 2, 2019 and January 30, 2024, for which a 9-item Patient Health Questionnaire (PHQ-9) was completed at the same time. After censoring PHQ-9 scores from notes, we estimated severity of depressive symptoms with a foundational large language …

Noisy-channel language comprehension in aphasia: A Bayesian mixture modeling approach.

Individuals with "agrammatic" receptive aphasia have long been known to rely on semantic plausibility rather than syntactic cues when interpreting sentences. In contrast to early interpretations of this pattern as indicative of a deficit in syntactic knowledge, a recent proposal views agrammatic comprehension as a case of "noisy-channel" language processing with an increased expectation of noise in the input relative to healthy adults. Here, we investigate the nature of the noise model in aphasia and whether it is adapted to the statistics of the environment. We first replicate findings that a…

Large language model-based biological age prediction in large-scale populations.

Accurate and convenient assessment of individual aging is crucial for identifying health risks and preventing aging-related diseases. Nonetheless, current aging proxies often face challenges such as methodological limitations, weak associations with adverse outcomes and limited generalizability. Here we propose a framework that leverages large language models (LLMs) to estimate individual overall and organ-specific aging using only health examination reports. We validated this approach across six population-based cohorts, encompassing over 10 million participants and demonstrated effectiveness…

XBusNet: Text-Guided Breast Ultrasound Segmentation via Multimodal Vision-Language Learning.

Background/Objectives: Precise breast ultrasound (BUS) segmentation supports reliable measurement, quantitative analysis, and downstream classification yet remains difficult for small or low-contrast lesions with fuzzy margins and speckle noise. Text prompts can add clinical context, but directly applying weakly localized text-image cues (e.g., CAM/CLIP-derived signals) tends to produce coarse, blob-like responses that smear boundaries unless additional mechanisms recover fine edges. Methods: We propose XBusNet, a novel dual-prompt, dual-branch multimodal model that combines image features wit

Specialized curricula for training vision language models in retinal image analysis.

Clinicians spend significant time reviewing medical images and transcribing findings. By integrating visual and textual data, foundation models have the potential to reduce workloads and boost efficiency, yet their practical clinical value remains uncertain. In this study, we find that OpenAI's ChatGPT-4o and two medical vision-language models (VLMs) significantly underperform ophthalmologists in key tasks for age-related macular degeneration (AMD). To address this, we developed a dedicated training curriculum, designed by domain specialists, to optimize VLMs for tasks related to clinical deci…

Automated cardiac magnetic resonance interpretation derived from prompted large language models.

BACKGROUND: The versatility of cardiac magnetic resonance (CMR) leads to complex and time-consuming interpretation. Large language models (LLMs) present transformative potential for automated CMR interpretations. We explored the ability of LLMs in the automated classification and diagnosis of CMR reports for three common cardiac diseases: myocardial infarction (MI), dilated cardiomyopathy (DCM), and hypertrophic cardiomyopathy (HCM). METHODS: This retrospective study enrolled CMR reports of consecutive patients from January 2015 to July 2024, including reports from three types of cardiac dise…

Language experience and reading ability modulate word recognition in deaf readers.

For most deaf readers, learning to read is a challenging task. Visual word recognition is crucial during reading; however, little is known about the cognitive mechanism of Chinese deaf readers during visual word recognition. In the present study, two experiments explored the activation of orthographic, phonological, and sign language representations during Chinese word recognition. Eye movements were recorded as participants read sentences containing orthographically similar words, homophones, sign language-related words, or unrelated words. All deaf readers showed shorter reading times for or…

Generation of antigen-specific paired-chain antibodies using large language models.

The traditional process of antibody discovery is limited by inefficiency, high costs, and low success rates. Recent approaches employing artificial intelligence (AI) have been developed to optimize existing antibodies and generate antibody sequences in a target-agnostic manner. In this work, we present MAGE (monoclonal antibody generator), a sequence-based protein language model (PLM) fine-tuned for the task of generating paired human variable heavy- and light-chain antibody sequences against targets of interest. We show that MAGE can generate novel and diverse antibody sequences with experime…

Enhancing substance use detection in clinical notes with large language models.

Identifying substance use behaviors in electronic health records (EHRs) is challenging because critical details are often buried in unstructured notes that use varied terminology and negation, requiring careful contextual interpretation to distinguish relevant use from historical mentions or denials. Using MIMIC-III/IV discharge summaries, we created a large, annotated drug detection dataset to tackle this problem and support future systemic substance use surveillance. We then investigated the performance of multiple large language models (LLMs) for detecting eight substance use categories wit…

Large language models accurately identify immunosuppression in intensive care unit patients.

OBJECTIVE: Rule-based structured data algorithms and natural language processing (NLP) approaches applied to unstructured clinical notes have limited accuracy and poor generalizability for identifying immunosuppression. Large language models (LLMs) may effectively identify patients with heterogenous types of immunosuppression from unstructured clinical notes. We compared the performance of LLMs applied to unstructured notes for identifying patients with immunosuppressive conditions or immunosuppressive medication use against 2 baselines: (1) structured data algorithms using diagnosis codes and…
