Tracking funding disparities in global health aid with machine learning.

Reducing the global burden of disease is crucial for improving health outcomes. However, misalignment between health aid and country-level disease burden leaves vulnerable populations without the necessary support for major health challenges, particularly in the least developed countries. Here, we develop a machine learning pipeline using large language models to track flows in official development assistance (ODA) earmarked for health and identify aid-burden misalignment. We classified 3.7 million development aid projects from 2000 to 2022 (USD  ~ 332 billion) into 17 major categories of comm…

LLM-powered prostate cancer staging from PSMA-PET/CT reports using PROMISE v2.

Accurate staging of prostate cancer is essential for guiding therapy and predicting outcomes. Prostate-specific membrane antigen (PSMA) PET/CT has become an established modality for disease assessment, and the PROMISE v2 framework provides standardized criteria for molecular imaging–based TNM (miTNM, molecular imaging TNM) classification. This study investigates the potential of large language models (LLMs) to automatically extract PROMISE v2 staging information from PSMA PET/CT reports. We retrospectively analyzed 1,696 reports from first-diagnosis prostate cancer patients using the open-sour…

Generative AI designs functional thiolation domains for reprogramming non-ribosomal peptide synthetases.

Large language models and generative protein design promise to accelerate biotechnology, but it remains unclear whether they can engineer dynamic megasynth(et)ases whose activity depends on transient, context-specific domain interfaces. Non-ribosomal peptide synthetases (NRPSs) exemplify this challenge and produce many clinically used therapeutics. Here we integrate pretrained generative models (ESM3, ProteinMPNN and EvoDiff) with design-build-test-learn cycles and data-guided prioritization to generate 76 de novo thiolation (T) domains. We build and test 578 recombinant NRPS variants in vivo …

LLM-Powered Dysphagia Screening With Multimodal Physiological Signal Analysis and Medically Informed Prompts.

Dysphagia is a common complication among stroke patients, significantly increasing the risk of aspiration pneumonia, malnutrition, and mortality. Traditional diagnostic techniques, such as bedside screening and videofluoroscopic swallowing studies, are limited by accessibility, reliability, and invasiveness. To address the challenges of limited data and complex multimodal signals, we propose a large language model (LLM)-based framework for dysphagia screening. This framework integrates multimodal physiological signals-including laryngeal vibration, nasal airflow, and swallowing sound-and lever

Med-CoReasoner: Reducing Language Disparities in Medical Reasoning via Language-Informed Co-Reasoning.

While reasoning-enhanced large language models perform strongly on English medical tasks, a persistent multilingual gap remains, with substantially weaker reasoning in local languages, limiting equitable global medical deployment. To bridge this gap, we introduce Med-CoReasoner, a language-informed co-reasoning framework that elicits parallel English and local-language reasoning, abstracts them into structured concepts, and integrates local clinical knowledge into an English logical scaffold via concept-level alignment and retrieval. This design combines the structural robustness of English re…

A multimodal retinal image dataset for diabetic retinopathy detection using foundation models.

Diabetic Retinopathy (DR), a leading cause of preventable blindness worldwide, underscores the urgent need for robust AI-driven diagnostic tools. Although various deep learning models for retinal imaging have emerged, their evaluation remains constrained by limited public available datasets that lack both large-scale coverage and fine-grained annotations, compromising reliable assessments of model generalizability. To bridge this gap, we introduce a comprehensive multimodal dataset that includes three key retinal imaging modalities: color fundus photography (CFP), optical coherence tomography

Content-based indexing of images and video.

By representing image content using probabilistic models of an object's appearance we can obtain semantics-preserving compression of the image data. Such compact representations of an image's salient features allow rapid computer searches of even large image databases. Examples are shown for databases of face images, a video of American sign language (ASL), and a video of facial expressions.

The Differential That Never Gets Generated.

Residents increasingly ask large language models to name a lesion before diagnosing it themselves. The practice contributes a specific harm: never-skilling, a phenomenon in which reasoning is never acquired, and is distinct from 'deskilling' or 'model error'. Forming a differential first is necessary but insufficient. Departments should require documented differentials, supervise disagreement, and examine unassisted reasoning.
