Surrogates 20 years on: long-term psychological health, contact with surrogacy families, and thoughts and feelings about post-birth contact.

STUDY QUESTION: What were the psychological health outcomes for surrogates 20 years after the surrogacy pregnancy? SUMMARY ANSWER: Most surrogates did not experience psychological problems 20 years after the pregnancy with many showing positive psychological wellbeing. WHAT IS KNOWN ALREADY: Studies of surrogates from the global north have found that although some surrogates may experience psychological difficulties in the weeks following the birth of the child, this decreases over time, with most surrogates not experiencing psychological problems at 6 months, 1 year or 10 years following th

An Aboriginal and Torres Strait Islander Cardiac Rehabilitation program delivered in a non-Indigenous health service (Yeddung Gauar): a mixed methods feasibility study.

BACKGROUND: There is limited evidence of Aboriginal and Torres Strait Islander people attending cardiac rehabilitation (CR) programs despite high levels of heart disease. One key enabler for CR attendance is a culturally safe program. This study evaluates improving access for Aboriginal and Torres Strait Islander women to attend a CR program in a non-Indigenous health service, alongside improving health workforce cultural safety. METHODS: An 18-week mixed-methods feasibility study was conducted, with weekly flexible CR sessions delivered by a multidisciplinary team and an Aboriginal and/or To

Proxy decision making in Alzheimer disease research: durable powers of attorney, guardianship, and other alternatives.

Individuals in advanced stages of Alzheimer disease are incapable of giving or refusing legally or ethically valid consent to participation in biomedical and behavioral research protocols. Yet, many research protocols aimed at better treatment of persons with this disease can be conducted usefully only if such individuals are themselves involved as human subjects. This dilemma raises difficult issues of personal autonomy and beneficent motives. This article discusses several potential proxy decision-making mechanisms regarding research participation in light of the relevant legal and ethical c

Life-terminating acts without explicit request of patient.

In the Dutch nationwide study on medical decisions concerning the end of life (MDEL) life-terminating acts without the explicit request of the patient (LAWER) were noted in 0.8% of all deaths. We present here quantitative information and a discussion of the main issues raised by LAWER. In 59% of LAWER the physician had some information about the patient's wish; in 41% discussion on the decision would no longer have been possible. In LAWER patients tend to be younger and more likely to be male and to have cancer than in non-acute deaths generally. The physician (specialist or general practition

COVID-19 and risk of first-time major depressive disorder in patients without history of psychiatric disorders.

BACKGROUND: Several studies have shown that patients who survived hospitalization with COVID-19 are at higher risk of psychiatric illness including major depressive disorder (MDD) compared to COVID− controls. However, few prior studies excluded patients with prior psychiatric illness from their analyses, and therefore the post-COVID risk of new-onset MDD among those without psychiatric vulnerabilities remains unclear. METHODS: This observational study analyzed patients with at least one COVID-19 test, no history of prior psychiatric diagnosis, and at least one follow-up visit 30 days post-COVI

Changes in cognitive performance following combined CogSMART-SA and BrainHQ interventions: a pilot study.

BACKGROUND: Despite effective antiretroviral treatment, neurocognitive impairment (NCI) and functional disability remain concerningly prevalent among people living with HIV (PLWH). Despite this, there are currently no adequate evidence-based treatments targeting NCI in PLWH who reside in low- and middle -income countries. This is a major public health concern given that most the global population of PLWH is concentrated in those countries. Studies emerging from high-income countries of the global north suggest that different forms of cognitive remediation therapy (e.g., compensatory cognitive

Global Perspectives on Physician-Assisted Death: A Cross-Sectional Survey of Doctors' Opinions.

Introduction Physician-assisted death (PAD) remains ethically and legally contested worldwide. PAD encompasses both euthanasia and physician-assisted suicide, and is used interchangeably with assisted dying. While some countries have legalized PAD, many others continue to deliberate, some remain silent, and many don't engage on this issue. Physicians' perspectives are central to these discussions, yet cross-national data remain limited. This study seeks to explore these perspectives and identify the factors that influence them. Methods We conducted an anonymous, online cross-sectional survey o

Perspectives on Self-Management and Meditation: A Qualitative Study of Adolescents With Type 1 Diabetes Mellitus and Their Parents.

BACKGROUND: One in three adolescents with type 1 diabetes mellitus (T1DM) experiences diabetes distress, which predicts poor self-management and glycemic control. Mindfulness-based interventions such as meditation have been associated with reduced psychological distress and health outcomes in different populations. This study explores the psychosocial barriers and facilitators of diabetes self-management and beliefs about meditation practices. METHODS: Eight adolescents aged 15-19 who had been diagnosed with T1DM for more than a year were invited to participate in a 40-60-minute semi-structur

The impact of the COVID-19 pandemic on nursing students' navigation of their nursing programmes and experiences of resilience. A qualitative study.

INTRODUCTION: High-quality pre-registration student nurse training and development is integral to developing a sustainable and competent global nursing workforce. Internationally, student nurse recruitment rates have increased since the onset of the COVID-19 pandemic; however, attrition rates for student nurses are high. During the pandemic, many student nurses considered leaving the programme due to academic concerns, feeling overwhelmed, and doubting their clinical skills. Little was known about the extent to which nursing education prior to COVID-19 had prepared students for their role in m

The role of zinc in gastrointestinal and liver disease.

Zinc is essential for many metabolic and enzymatic functions in man. Deficiency of zinc in man has now been recognized to occur not only as a result of nutritional factors, but also in various disease states, including malabsorption syndromes, acrodermatitis enteropathica, Crohn's disease, alcoholism and cirrhosis of the liver. The deficiency state in human subjects exists as a spectrum extending from mild to severe degree. The clinical manifestations of mild zinc deficiency include oligospermia, weight loss and hyperammonaemia. Moderate zinc deficiency is characterized clinically by growth re

Implementation Outcomes and Lessons From a Health-Department-Driven Quality-Based Financing Approach to HIV Prevention Programmes in New York City, United States: A Mixed Methods Analysis.

INTRODUCTION: In 2021, the New York City Health Department (NYCHD) became one of the first jurisdictions in the United States to adopt a quality-based financing (QBF) approach to HIV prevention programming, shifting away from the traditional fee-for-service model. NYCHD used a multi-level approach that defined quality through three programme-level implementation strategies (PLS): universal screening; holistic care provision; and sex-positive, "risk-free" HIV prevention conversations. By focusing their QBF programme on implementation strategies in addition to service delivery outcomes, NYCHD he

Adapting the HealthMpowerment (HMP) mobile application for youth living with or at risk of HIV in Africa using the ADAPT-ITT model.

BACKGROUND: Adaptation plays a critical role in implementation science (IS) by optimizing the fit of evidence-based interventions (EBIs) to local contexts and populations. Adaptation is especially relevant in digital health, where technologies, user preferences, and needs evolve rapidly. Yet, methodological approaches to guide and document adaptation processes are seldom included. Grounded in an IS framework, we describe a multi-phase process and results for the adaptation of a digital health intervention (DHI) for HIV prevention and treatment for sexual and gender minority (SGM) youth in Mala

'But … Would I Be Able to Toast With Friends?' When Service Users Ask for New Care Pathways.

INTRODUCTION: The WHO European Mental Health Action Plan (2013-2030) emphasises the need to generate services that are more inclusive and attentive to the co-construction of care practices. This exploratory research investigates the needs of young substance abusers shown during their stay in residential communities; in particular, it explores the idea that treatment may include a new phase focused on how to manage moderate or controlled alcohol intake during residential care. Interviews with young ex-users open up critical reflections on complete abstinence programmes from all substances, incl

[What are the principles for measurement and estimation of accident sequelae? : Development and function of the reduction in earning capacity (MdE) tables in the statutory accident insurance].

The evaluation of permanent dysfunction after injuries to the musculoskeletal system is one of the core activities of a trauma surgeon and orthopedists. Based on the knowledge of the injury and an accurate description of the dysfunction, the medical expert then makes a proposal regarding the amount of the reduction in earning capacity (Minderung der Erwerbsfähigkeit, MdE). The amount is based on the MdE tables, which have been derived from a decade-long harmonization and coordination between administrative authorities, courts and the medical profession. They have been published in the fundamen
