INTRODUCTION: Several concepts have been published for therapy of idiopathic clubfoot. Actually the Cincinnati approach is the "gold-standard" in operative treatment of idiopathic clubfoot. Using this approach delayed wound healing and overcorrection are wellknown complications. During the last 30 years a dorsomedial approach is used in operative treatment of idiopathic clubfoot in our clinic. Using this approach a dorsal, medial and/or lateral release can be performed. Postoperatively a plaster cast is used. The following years a rigid therapy with orthosis and support is requested.

Eosinophilic granuloma (EG) is a rare disease but is more common in adults than children. It's often self-limiting. Spinal involvement is rare. It is the localized and most benign form of Langerhans' cell histiocytosis (previously known as histiocytosis X), characterised by lytic lesions in one or more bones. Spontaneous resolution of vertebral body lesions is very rare. In this case, the patient had one EG in a cervical vertebra and a similar lesion in a lumbar vertebra. This case is important because it featured a symptomatic lesion in the cervical spine accompanied by an asymptomatic lesion

Although in practice since antiquity the nonoperative treatment of spinal deformities is still controversial. Giving preference to orthotic treatment and physiotherapy the historical development of nonoperative treatment will be demonstrated by the examples of idiopathic scoliosis and Scheuermann's kyphosis. It is surprising how early essential and still valuable principles of nonoperative treatment had been acquired and how many of the present-day concepts have remarkable forerunners in the 19th century. In the light of previous knowledge and personal experience the indication, practice and r

Most shoulder problems seen by the primary care physician involve impingement syndrome or problems at the acromioclavicular joint. Despite the complexity of the structures involved, most of these conditions can be diagnosed and treated without difficulty. MRI or other imaging studies are seldom needed but can be used to confirm a questionable diagnosis. Referral to an orthopedic surgeon is appropriate if shoulder problems persist for 3 to 6 months or if there is evidence of a medium or large rotator cuff tear, severe shoulder stiffness, or a complicated fracture.

Fifty-five patients with severe pain from spinal instability secondary to metastatic cancer were referred to Hope Hospital, none being judged to be in a terminal condition. One patient had too extensive disease for surgery so 54 were treated by 55 spinal stabilisations; 49 obtained complete relief of pain and two had partial relief. There were three failures. Twenty-eight of the patients had clinical evidence of spinal cord or cauda equina compression and were decompressed at the time of stabilisation. Of these, 20 had major recovery of neurological function. Patients with pre-operative eviden

BACKGROUND: Spinal orthoses can be used to limit active range of motion in different directions and to support trunk extension; however, their effect on movement and muscle activity remains uncertain.
OBJECTIVE: This study explored the effects of two semi-rigid spinal orthoses on trunk kinematics and muscle activity compared to no brace (NB).
STUDY DESIGN: A pre-clinical, feasibility study.
METHODS: Markerless motion capture and surface electromyography was collected from 20 healthy participants each performing 5 repetitions of a sit-to-stand-to-sit task under three conditions: NB, Brace A

BACKGROUND: The optimal nonoperative treatment for a Boutonniere deformity remains controversial.
PURPOSE: We aimed to evaluate the effect of nonsurgical treatment methods on finger motion for Boutonniere deformities.
STUDY DESIGN: Retrospective cohort study.
METHODS: Conservative treatments included hand therapy, orthotic wear, and home finger exercises. Digit range of motion (ROM) and injury characteristics also were evaluated. Based upon active ROM at the proximal interphalangeal joint, digits were graded as Poor, Good, or Excellent, and then a change in ROM grade was calculated from pre

BACKGROUND: Carbon fiber custom dynamic orthoses have been used to improve gait mechanics after lower limb trauma in military service members, with the goal of restoring function and improving outcomes. However, the effects of commercially available carbon fiber orthoses available to civilians on lower extremity joint kinetics and kinematics are poorly understood.
RESEARCH QUESTION: The aim of this study was to examine the effect of two commercially available orthoses on lower extremity kinematics and kinetics in individuals with lower limb trauma.
METHODS: A total of 23 participants with a

BACKGROUND: Developmental dysplasia of the hip (DDH) occurs in 3-5 of 1000 live births and is associated with known risk factors. In most countries, formal practice for early detection of DDH entails the combination of risk factor identification and physical examination of the hip, while the golden standard diagnostic instrument is hip ultrasonography (US). This practice is commonly referred to as selective screening. Infants with positive US findings are treated with a Pavlik harness, a dynamic abduction splint. The objective of our study was to evaluate hip US utilization patterns in Maccabi

Stroke is a significant source of mortality and long-term disability in the United States. Of persons who survive a stroke, approximately 50% will have hemiplegia, half of whom will live with a nonfunctional arm. Hemiplegic shoulder pain (HSP), which occurs in most patients with hemiplegia, reduces participation and worsens outcomes in rehabilitation. Management of HSP is challenging because its causes are multifactorial and there is limited, conflicting, or nonspecific evidence in support of most treatments. This article develops an effective approach for diagnosis and treatment using the bes

BACKGROUND: Even though walking can be sustained for great distances, considerable energy is required for plantarflexion around the instant of opposite leg heel contact. Different groups attempted to reduce metabolic cost with exoskeletons but none could achieve a reduction beyond the level of walking without exoskeleton, possibly because there is no consensus on the optimal actuation timing. The main research question of our study was whether it is possible to obtain a higher reduction in metabolic cost by tuning the actuation timing.
METHODOLOGY/PRINCIPAL FINDINGS: We measured metabolic cos

Thoracic outlet syndrome may result from a posterior sternoclavicular (SC) joint subluxation, or an anterior SC joint subluxation after surgical fixation. This case report presents the physical therapy management of a patient with bilateral thoracic outlet syndrome (TOS) secondary to bilateral idiopathic anterior SC joint subluxation. A 16-year-old female presented with a 2-year history of numbness, tingling, and coldness in bilateral upper extremities, and intermittent headaches with occasional vision loss. Ipsilateral upper extremity symptoms were reproduced with cervical rotation and should

PURPOSE: Atlantoaxial rotatory fixation and atlantoaxial rotatory subluxation are the most frequent manifestations of atlantoaxial rotatory dislocation (AARD) in pediatric population and are often treated conservatively. The objective of this study is to correlate the changes highlighted on MRI T2-weighted and STIR sequences with the duration of conservative treatment.
METHODS: We analyzed nine consecutive patients treated surgically between 1 Jan 2006 and 1 Jan 2010 at the Policlinico Umberto I of Rome. All patients underwent cervical X-ray, computed tomography and magnetic resonance imaging

BACKGROUND: Those who experience lower extremity weakness or paralysis following a stroke often exhibit gait deviations caused by the inability to completely lift their foot during swing. An ankle-foot orthosis (AFO) is commonly prescribed for individuals post stroke with this mobility impairment.
STUDY DESIGN: Randomized controlled trial.
OBJECTIVES: To determine whether significant differences could be observed in post-stroke individuals ambulating with an experimental AFO set at three different ankle orientations.
METHODS: Gait analysis was conducted for eight post-stroke individuals amb

OBJECTIVE: People who have extremely high arched feet are subject to substantial foot pain, despite a lack of obvious pathology. Customized foot orthoses improve pain and function and reduce foot pressure, but their specificity is unclear. This study sought to identify predictive variables for improvement and to determine whether changes in foot pressure were correlated to changes in pain and function.
DESIGN: Retrospective analysis of data from a randomized controlled clinical trial.
SETTING: Clinical movement analysis laboratory, School of Physiotherapy, University of Sydney.

This retrospective study of the early work of Arnold Pavlik in the treatment of developmental dysplasia of the hip proves the success of his method in eradicating avascular necrosis (AVN) resulting from other modes of treatment. Authors analyzed some 100 charts of children treated for CDH, as it was known at that time, between 1969 and 1981, and assessed the influence of gender, clinical stability, severity of X-ray pathology and age at which treatment was started, according to duration, outcome of treatment and rate of AVN. Of the 100 children with 134 treated pathological hips, 62 children w
