Three autopsy cases of pontine and extrapontine myelinolysis are reported; one, a malignant lymphoma in a man of 66 years, the other an alcoholic liver cirrhosis in a man of 54 years, and an esophageal cancer in a woman of 68 years who presented only with extrapontine myelinolysis, but lacked central pontine myelinolysis (CPM). A review is given on symptoms, etiology, pathogenesis and prognosis of central pontine myelinolysis (CPM). Since 1959, 315 cases of CPM have been reported in world literature, 41 per cent of the patients described developed their neurological symptoms in the course of chronic alcoholism. In about 32 per cent CPM was connected with electrolyte disturbance, especially hypo- or hypernatremia. We report a 1 1/2-year-old child with an ependymoma in the posterior cranial fossa. As the computed tomographic scan showed evidence of marked triventricular hydrocephalus, the patient underwent ventriculoperitoneal shunting. Two hours after operation, he developed a fatal intratumoral hemorrhage. The indications and contraindications of ventricular external or internal drainage before posterior cranial fossa tumor removal are briefly discussed.

A 65-year-old woman with known breast carcinoma developed headaches, followed shortly by disequilibrium, vertical diplopia and dysarthria. Cranial computerized tomography (CCT) demonstrated a dense, irregularly enhancing pontine lesion with associated mass effect. She received dexamethasone (Decadron) and radiotherapy and subsequently expired. Postmortem examination disclosed a ruptured brain stem arteriovenous malformation within a massive hemorrhage along with necrotic atypical cells suggestive of metastatic disease.

Central pontine myelinolysis (CPM) occurred in a typical patient with severe alcoholism presenting with severe initial hyponatremia and a rapidly progressive pontomesencephalic syndrome. A review on 141 cases of CPM is presented and the ties between CPM, hyponatremia/hypoosmolality and the syndrome of inadequate secretion of ADH are discussed. Correct interpretation of the patient's history (alcoholism, severe wasting disease) and the clinical picture (initial hypoosmolality with acute or subacute pontomesencephalic syndrome, possibly locked-in syndrome) should permit correct in-vivo diagnosis

Discrete lesions interrupting the fiber tracts that connect the pontine center for conjugate horizontal gaze and the ipsilateral abducens nucleus create the syndrome of Lutz's posterior internuclear ophthalmoplegia. Sleep patterns in two chronically "locked-in" patients were studied. The first patient had sleep activity similar to other quadriparetic patients with reduced physical activity. The second patient experienced no REM sleep and only short bursts of non-REM sleep. Clinical evidence suggested extension of his lesion dorsally into the pontine tegmentum. This correlates well with experimental evidence indicating that structures in the tegmental area are essential for normal sleep.
