Mediation of the respiratory reflex effects of an exogenous serotonin challenge goes beyond the lung vagi and is suggested to involve the nodose ganglia. In the present experiments the effects of an intravenous serotonin challenge on breathing pattern were studied in 8 pentobarbitone-chloralose anaesthetised cats. Bolus injection of serotonin oxalate (50 microg/kg) into the right femoral vein evoked prompt apnoea of 19.2 (+/- 2.4)-second duration in all 8 cats while intact; the apnoea was much shorter after midcervical vagal section (8.1 +/- 0.9 s, p < 0.001), and was abolished by supranodose

An 11-year-old boy suffered from fever, headache, severe vertigo and unsteady gait. Physical examination showed bilateral vertical nystagmus, mild corneal reflex delay of the right eye and asymmetric facial expression. Laboratory data showed leukopenia, high ESR and normal CSF study. Brain CT showed diffuse brain edema. Electronystagmography showed upbeat nystagmus and central vertigo. EEG revealed diffuse slow wave and mild to moderate cortical dysfunction. MRI of the head showed focal abnormal signal intensity at the ventral portion of the medulla oblongata on both sides. Under suspicion of

OBJECTIVE: To investigate the activation pattern of cerebellar structures during small-field optokinetic stimulation (OKN) by functional MRI (fMRI) and compare it with that obtained during voluntary saccades and fixation suppression of OKN.
METHODS: Functional images were acquired from oblique transverse slices of eight healthy, right-handed volunteers using a radio frequency-spoiled, single-slice, fast low-angle shot (FLASH) pulse sequence with high-spatial resolution.
RESULTS: Horizontal OKN and saccades were associated with bilateral activity in the cerebellar hemispheres (superior semilu

A 73-year-old man with a 16-year history of fasciculations and 15 years of weakness in his right arm was diagnosed with focal motor neuron disease. After 10 years of purely motor symptoms, he developed mild parasthesias although his sensory examination remained normal. Reflexes were reduced or absent in the weak muscles but were normal elsewhere. Nerve conduction was studied in nerves innervating weak muscles and showed severe motor conduction block. Sensory nerve conduction studies were minimally abnormal, showing reduced amplitudes with normal velocities. Based on the clinical picture and th

The responses to rapid, passive, unpredictable, low amplitude (10-20 degrees), high acceleration (3,000-4,000 degrees/s2) head rotations were used to study the human vestibulo-ocular reflex (VOR) in pitch and yaw plane after unilateral posterior semicircular canal occlusion (uPCO) in 10 subjects. The results from these 10 uPCO subjects were compared with those from 18 normal subjects. The VOR gains at a head velocity of 200 degrees/s in the uPCO subjects were: pitch upward = 0.62 +/- 0.06, pitch downward = 0.87 +/- 0.11, yew ipsilesion = 0.78 +/- 0.06, yaw contralesion = 0.79 +/- 0.10 and in n

A 61-year-old male with carcinomatous autonomic and sensory neuropathy was presented. The disease started with numbness in his lower extremities and loss of sensation gradually developed over the upper extremities within two months. Finally he became unable to walk because of orthostatic hypotension and dystaxia due to loss of deep sensation. Physical examination on admission revealed mydriasis with negative light reflex, orthostatic hypotension, sensory loss of below C-4, areflexia, positive Babinski's sign and sexual impotence. Laboratory examinations revealed serum antineural antibody, elev

Obstruction of total anomalous pulmonary venous drainage (TAPVD) is sometimes observed in neonates, usually at the site where the collector drains into the caval system. Stenosis of the pulmonary veins themselves before joining the collector is less common. This was observed in 6 cases of intra- and supracardiac TAPVD; the prognosis is usually very bad due to postoperative pulmonary hypertension (PHT) which is difficult to treat. Three of our cases had TAPVD into the coronary sinus with obstruction presenting at birth. Conventional surgery did not reduce the PHT of the first 2 children because

A 16-year-old girl was admitted to our hospital in August 23, 1986, for headache, nausea and low grade fever. Marked increases in immunoglobulin indices were found in the cerebrospinal fluid. When she was 13, she was diagnosed as having SLE and lupus nephritis. On September 9, 1986, she complained of urinary retention, and pathological reflexes were elicited bilaterally. On September 13, she complained of a sudden loss of vision (count fingers) in the right eye which worsened to a visual acuity of light perception over the next 48 hours. A visual evoked response potentials (VEP) to flash stimu

Dynamic characteristics of ventilation, cardiac output, and gas exchange in response to passive limb movements were studied in four healthy men in an upright position. Passive exercise was performed on a motor-driven bicycle ergometer, of which pedaling rate was varied from control (30 rpm) to stimulus (90 rpm) level in a stepwise fashion. Stroke volume (SV), heart rate (HR), and cardiac output (Q) were determined continuously during the exercise by using an automated impedance cardiograph. Minute ventilation (VE), respiratory frequency (f), tidal volume (VT), oxygen consumption (VO2), carbon

The capacity of delayed barbiturate administration to limit brain damage after unilateralcerebral ischemia was examined histologically in gerbils. The right common carotid artery was occluded in 50 animals under brief (3-minute) halothane anesthesia; 18 animals (36%) developed motor abnormalities consistent with stroke. The arterial clasps were removed after 1 hour and the abnormal animals were divided into treatment and placebo groups. Treated gerbils received sodium pentobarbital (70 mg/kg) intarperitoneally 1 hour after clasp removal and a smaller dose (50 mg/kg) 2 hours later; these animal

We report a right-handed 62-year-old man with early onset familial parkinsonism. The patient was well until 24 years of the age when he noted an onset of resting tremor in his right hand. During the next four years, he noted rigidity, bradykinesia, and difficulty in walking. He was seen in another hospital at 28 years of the age, where he received left pallidotomy. Rigidity on the left side showed marked improvement. He received right pallidotomy at age 30 years. He developed right hemiplegia after this surgery. He was admitted to our hospital in March, 1983 when he was 51 years of the age. He

BACKGROUND: Knee pain may induce protective gait patterns that reduce knee loading, but the multi-joint trade-offs that emerge under uneven-terrain demands remain uncertain.
METHODS: Using SCONE/Hyfydy, we generated sagittal-plane predictive simulations over a deterministic ten-tile terrain sequence. A reflex-based controller was optimized under no knee-load limit (C0) or bilateral soft limits of 4, 3, or 2 body weights (C4, C3, and C2). We retained the lowest-objective solution from 24 runs per condition and reanalyzed every complete stance within 1-12 s by limb and terrain.
RESULTS: The C0

Congenital afibrinogenemia is an exceedingly rare autosomal recessive bleeding disorder characterized by the complete absence of circulating fibrinogen, predisposing affected individuals to spontaneous and trauma-related hemorrhage at virtually any anatomical site. Acute scrotal pain, by contrast, is a common urological emergency in which testicular torsion must be excluded promptly, as diagnostic delay risks irreversible testicular loss. We describe a 31-year-old man with known congenital afibrinogenemia, maintained on periodic fibrinogen concentrate replacement, who presented with acute righ

BACKGROUND: Marchiafava-Bignami disease (MBD) is a rare alcohol-related disorder that predominantly affects the corpus callosum. Because MBD frequently demonstrates diffusion restriction on magnetic resonance imaging (MRI), it can closely mimic acute ischemic stroke (AIS), posing a diagnostic challenge in image-driven acute stroke evaluation. Early recognition is crucial, as timely thiamine therapy may result in complete clinical and radiologic recovery.
CASE DESCRIPTION: A 38-year-old man with long-standing daily consumption of shochu, a Japanese distilled spirit, developed vertigo and vomit

INTRODUCTION: Gallbladder distension secondary to malignant biliary obstruction is an under-recognized cause of abdominal pain in advanced pancreatic cancer and may present atypically outside the right upper quadrant. Pain assessment can be further complicated in older adults with cognitive impairment and superimposed delirium.
CASE PRESENTATION: We present an 83-year-old female with locally advanced pancreatic adenocarcinoma and baseline cognitive impairment, admitted to an acute tertiary hospital with painless jaundice. She subsequently developed persistent right iliac fossa pain that was p

Chilaiditi sign is defined as the interposition of the colon or small intestine between the liver and the right diaphragm in the absence of symptoms. Chilaiditi syndrome refers to the condition where the Chilaiditi sign is associated with symptoms including abdominal pain. In this series, we present the cases of two pre-pubescent patients with these rare conditions. A 10-year-old female with a history of autism, IgA deficiency, and constipation presented for gastrointestinal studies due to weight loss and constipation. An abdominal X-ray revealed bowel gas under the right hemidiaphragm and co

Currently there are no reliable biomarkers for early detection of Alzheimer's disease (AD) at the preclinical stage. This study assessed the pupil light reflex (PLR) for focal red and blue light stimuli in central and peripheral retina in 125 cognitively normal middle age subjects (45-71 years old) at high risk for AD due to a family history of the disease (FH+), and 61 age-similar subjects with no family history of AD (FH-) using Chromatic Pupilloperimetry coupled with Machine Learning (ML). All subjects had normal ophthalmic assessment, and normal retinal and optic nerve thickness by optical
