Assessment of patient function after limb-sparing surgery.

Cancer rehabilitation is becoming more of a focus for the field of physiatry due to increased longevity and the side effects of treatment. In order to investigate the rehabilitation needs of patients undergoing limb-sparing procedures, chart analysis was conducted on 17 children treated for primary bone tumors by resection and an expandable endoprosthetic replacement. Each patient underwent a course of postoperative inpatient and outpatient physical therapy and was followed over an average of 2.5 years. Gait training was relatively straightforward and in seven patients required neither orthosi

Small cell tumours in childhood: a review.

The past decade has seen significant advances in the treatment of childhood malignancies accompanied by appreciable improvement in survival rates. Treatment programmes have been largely formulated to meet the specific characteristics of individual tumours, as well as being based on the extent of disease presented at diagnosis. In selecting the most appropriate treatment protocol, accurate histological categorization of resected or biopsied tumour is thus of paramount importance. In the paediatric age range in which so many tumours lack differentiation as to present as, or mimic, other small ce

Reversal of protein-energy malnutrition in children during treatment of advanced neoplastic disease.

The effectiveness of enteral and parenteral feeding in supporting a satisfactory nutritional status and/or reversing protein-energy malnutrition was evaluated in 28 children, ages 1-19 (14 female) with advanced malignant disease (21 solid tumors, 7 leukemia-lymphoma). At the onset of treatment, 21 patients received intensive nutritional counseling (INC) and oral supplementation while seven received total parenteral nutrition (TPN). Sixteen of 21 patients who received INC had a decreased intake (x 48 +/- 24%) Recommended Dietary Allowances (RDA) for kilocalories and dramatic weight loss (x 16.4

Intraoperative pathologic consultation (IOC) for tumors of the bone.

When combining clinical examination, laboratory information and noninvasive imaging studies the differential diagnosis of bone lesions is narrowed. For those who are not experts in the field the major purpose of the IOC is to insure that adequate tissue has been obtained and to triage the tissue in the process preparing imprints, whenever possible; fixing some tissue for possible electron microscopic review; placing some tissue in B5 fixative for better cytologic detail; and to save some undecalcified tumor tissue in formalin in case immunostaining procedures are required. Most community patho

Socioeconomic Predictors of Surgical Resection and Survival for Patients With Osseous Spinal Neoplasms.

OF BACKGROUND DATA: Primary osseous spinal neoplasms (POSNs) include locally aggressive tumors such as osteosarcoma, chondrosarcoma, Ewing sarcoma, and chordoma. For such tumors, surgical resection is associated with improved survival for patients. Socioeconomic predictors of receiving surgery, however, have not been studied. OBJECTIVE: To examine the independent effect of race on receiving surgery and survival probability in patients with POSN. STUDY DESIGN: A total of 1904 patients from the SEER program at the National Cancer Institute database, all diagnosed with POSN of the spinal cord, …

[Bilateral fibula graft. Biological reconstruction following resection of malignant bone tumors].

Bilateral vascularized fibula graft (BVFG) is actually not a satisfying method for the replacement of metadiaphyseal defects of the femur and tibia in young patients suffering from malignant bone tumors. This reconstruction was used in five patients (two female, three male, average age 15.2 years, femur n=3, tibia n=2) undergoing metadiaphyseal resection of malignant bone tumors between November 2000 and August 2003. The median length of the defect to be bridged was 16.4 cm (range 11.5-23). In the two cases of tibia reconstruction, the ipsilateral fibula was transposed into the osseous defect

Phase I study of fludarabine (2-fluoro-ara-AMP).

Fludarabine phosphate is a derivative of adenosine arabinoside. The compound is an antimetabolite which resists deamination by the addition of a phosphate moiety. A phase I trial was conducted and showed the safe dose to good-risk patients to be 20 mg/m2 in 125 ml of 5% dextrose given over 30 min every 12 hr for 6 doses. The cycle can be repeated every 21 days. The major side-effect is myelosuppression, which can be severe at higher doses.

Malignancies in children who initially present with rheumatic complaints.

OBJECTIVE: Children ultimately diagnosed with malignancy are referred to pediatric rheumatology clinics with provisional rheumatic diagnoses. We aimed to distinguish the features in these patients that lead to the correct diagnosis of malignancy. STUDY DESIGN: A retrospective review of the case records of 29 children (19 boys and 10 girls, aged 1 to 15.5 years) with malignancy who were referred to 2 pediatric rheumatology centers between 1983 and 1997. RESULTS: The suspected diagnoses on referral were: juvenile rheumatoid arthritis (12), nonspecific connective tissue disease (4), discitis (3

The role of cytogenetics in the classification of soft tissue tumours.

Soft tissue tumours represent a heterogeneous group of mesenchymal lesions, and their classification is the subject of continuous debate. Chromosome analysis, molecular cytogenetics and molecular assays may become increasingly useful in diagnosis, and this review summarises advances in the cytogenetic characterisation and classification of soft tissue tumours. Among the group of fibrous lesions, superficial fibromatosis exhibits trisomy 8. This genomic change is also observed in desmoid fibromatosis in association with trisomy 20. Trisomy 11 is the most frequently observed chromosomal aberrati

Challenges and Clinical Relevance in Diagnosing Metastatic Cells From Non-Hematopoietic Malignancies in Bone Marrow Aspirates.

INTRODUCTION: The causes of cytopenias are numerous, and the bone marrow aspirate helps to identify them. In rare cases, these cytopenias are due to bone marrow metastases from solid cancers. The techniques used in hematology laboratories are limited in characterizing these cells. Interaction with the cytopathology laboratory becomes critical for characterizing tumor cells and completing a comprehensive diagnosis from the bone marrow aspirate. METHODS: This article describes a series of 38 bone marrow aspirates from 36 patients with bicytopenias who underwent bone marrow aspiration and for wh

Axillary lesion in a young adult ends up to a peculiar diagnosis: primitive neuro-ectodermal tumor (PNET) of ulnar nerve: a rare case report.

INTRODUCTION AND IMPORTANCE: Primitive neuro-ectodermal tumor (PNET) is a highly aggressive tumor composed of small round blue cells, mostly developing in children and young adults. Being a member of Ewing's Sarcoma Family of Tumors (ESFT); it has been discussed in two subcategories of central and peripheral PNET. PNETs of peripheral nerves are very uncommon pathologic findings, as to the best of our knowledge only 12 well-documented cases have been yet reported. CASE PRESENTATION: A 30-year-old male presented with progressive paresthesia of his right hand's little finger and painless swellin

CSF Involvement by Nonhematolymphoid Malignancies: A Descriptive Study with Emphasis on Cytomorphological Clues.

INTRODUCTION: Detection of malignant cells in cerebrospinal fluid (CSF) samples in suspected cases of malignancy is critical for the management of patients. CSF involvement by nonhaematolymphoid malignancies is less common. We aimed to study the cytomorphologic characteristics of various nonhaematolymphoid malignancies in CSF. METHODS: A retrospective cytomorphological analysis of 27 CSF cytology smears reported as positive or suspicious for nonhematolymphoid malignancies from January 2010 to April 2020 over 10 years was carried out. Smears in all cases were prepared by cytospin technique and

Neuroendocrine carcinomas: Cytological mimics and diagnostic dilemmas.

BACKGROUND: Neuroendocrine carcinomas (NEC) are rare tumours with multiple morphologic mimics. The cytologist may miss these cases if not aware. AIMS AND OBJECTS: We focused upon the clinical presentation, cytological features and problems in the diagnosis of NEC. MATERIALS AND METHODS: We studied the detailed cytomorphology of 19 cases of NEC (histopathology proved). RESULTS: Of these 19 cases of NEC, 15 cases constituted fine needle aspiration (FNA) specimens, one transbronchial needle aspiration specimen, two pleural fluid specimens and a single biliary brushing specimen. All of the FNA

Transient osteomyelitis of the distal radius in a three-year-old patient.

Aim Reporting on an atypical case of a three-year-old patient affected by osteomyelitis of the distal radius, completely healed without antibiotic therapy. Methods The clinical case is related to a three-year-old patient, whose clinical picture began with fever, pain and swelling of the right wrist. After three days, fever and swelling disappeared, but pain persisted. Plain x-rays of the right wrist, hematologic analyses, magnetic resonance imaging (MRI) with contrast medium and biopsy were performed in order to make a differential diagnosis between acute osteomyelitis and a malignant neoplasm
