[Lunocapitate shortening arthrodesis after scaphoid and triquetrum resection: treatment of Watson stage III advanced scaphoid periarthrosis].

PURPOSE OF THE STUDY: Between 1993 and 2000, 16 patients with scaphoid periarthrosis (Watson stage III) were treated by lunocapitate shortening arthrodesis after resection of the scaphoid and triquetrum. These patients were reviewed to evaluate outcome. MATERIAL AND METHODS: Thirteen patients had antecedent trauma with scaphoid nonunion (SNAC, n=9) or scapholunate advanced collapse (SLAC, n=4). The remaining three patients had chondrocalcinosis. RESULTS: Clinical and radiological results were encouraging at a mean follow-up of 3 years (1-7 years). Fourteen patients were very satisfied and tw

Orthopedic pitfalls in the ED: scaphoid fracture.

Wrist injuries are frequently encountered in the emergency department. When a patient presents with such an injury, the possibility of scaphoid fracture must be at the top of the differential for the emergency practitioner. Unfortunately, these injuries can be missed on first presentation, as they are frequently radiographically occult. When left unrecognized and untreated, these injuries lead to a high incidence of long-term functional disability and chronic pain. The emergency physician needs to be vigilant for scaphoid fracture and be aggressive in both its diagnosis and treatment to avoid

Unusual coalition of the carpal bones with simian crease: a case report.

We present a case of scapholunate coalition with absent fifth metacarpal on one hand, and scapholunate triquetral coalition on the other hand. The patient was asymptomatic, though he was aware of the deformity of the little and ring fingers on the right hand. No treatment was offered as the deformity was not incapacitating and the syndactyly did not interfere with hand function.

Mechanical Effects of Wrist Position at the Wrist Joint: A Finite Element Analysis.

PURPOSE: This study aimed to evaluate the effects of wrist joint angles during flexion-extension and radioulnar deviation on stress distribution using finite element analysis. METHODS: Eight fresh-frozen upper limb specimens were analyzed using computed tomography. Finite element models were developed to simulate grip postures in flexion-extension (five positions ranging from 30° flexion to 30° extension) and radioulnar deviation (eight positions from 15° radial deviation to 20° ulnar deviation). Stress distributions (equivalent stress, minimum principal stress, and maximum principal stress)

Painful wrist osteoarthritis can result in major functional impairment. Most cases are related to posttraumatic sequel, metabolic arthropath … ty, and patient's wishes and functional demand. Proximal row carpectomy and four-corner fusion with scaphoid bone excision are the most widely used surgical procedures for stage II wrist osteoarthritis secondary to scapho-lunate advanced collapse (SLAC) or scaphoid non-union advanced collapse (SNAC) wrist. Proximal row carpectomy is not indicated in patients wi …

Flexor Carpi Radialis Tendon Rupture from Scapho-Trapezio-Trapezoid Osteoarthritis: A Case Report and Review of Literature.

Subcutaneous rupture of the flexor carpi radialis (FCR) tendon from scapho-trapezio-trapezoid (STT) osteoarthritis is a rarely reported and diagnosed condition. A 65-year-old female presented in the clinic with wrist pain aggravated with movement. On physical examination, there was a 2-cm soft elastic mass at the radial side of the volar wrist. There was no tenderness, but pain was elicited with range of motion. Computed tomography confirmed an osteoarthritic STT with large spurs. Magnetic resonance imaging (MRI) showed impingement on the tract of the FCR associated with synovitis. Complete ru

Return to Play After Hand and Wrist Fractures.

Wrist and hand injuries are common among athletes, and can lead to considerable disability. Dislocations and soft tissue injuries are common and require prompt recognition and treatment. Accurate diagnosis and early immobilization are often key to getting players back to their sport early. Some injuries require surgery; operative intervention allows the player to return to their sport more quickly or with less long-term disability. This article discusses the spectrum of injury from distal radius fractures to mallet fingers, and offers some general guidelines for the surgeon in how to counsel a

Complications in dorsal percutaneous cannulated screw fixation of nondisplaced scaphoid waist fractures.

PURPOSE: With advances in tools and techniques, percutaneous screw fixation of nondisplaced fractures of the scaphoid waist has gained increasing popularity in recent years as an alternative to prolonged cast immobilization or open reduction and internal fixation. Many reports cite low complication rates, including no complications in some series. The purpose of this study was to evaluate the complications encountered with dorsal percutaneous cannulated screw fixation of nondisplaced scaphoid waist fractures. METHODS: A retrospective chart review was performed for 24 patients who had surgery

Capitolunate arthrodesis maintaining carpal height for the treatment of SNAC wrist.

The clinical and radiological results of a modified midcarpal fusion technique for scaphoid nonunion advance collapse were retrospectively studied in eight patients. All had partial resection of the proximal part of the fractured scaphoid, limited radial styloidectomy, scaphocapitate and lunocapitate arthrodesis, using a block of iliac crest graft to maintain carpal height. All united without complications and wrist motion, grip strength and carpal height were improved postoperatively. The modified Mayo wrist score at follow-up was 70%. Three patients continued to have some pain and one patien

Reliability of Radiographic Parameters in the Evaluation of Scapholunate Dissociation.

PURPOSE: Accurate and reliable radiographic measurement is essential to diagnose and evaluate surgical treatment outcomes for scapholunate dissociation (SLD). Although published standards for measuring radiographic parameters have demonstrated excellent interrater and intrarater reliability in normal patients, we were unable to find reliability data for patients with SLD. We hypothesized that six radiographic parameters for SLD would have good reliability in normal and injured patients. METHODS: Wrist radiographs of patients with SLD and a control group of uninjured patients seen at our insti

Current Trends in Scaphoid Fracture Management: An Update.

Scaphoid fractures are a common but frequently missed wrist injury. Patients classically present following a fall onto an outstretched hand, with pain and tenderness in the anatomical snuffbox. When missed, scaphoid fractures can have serious complications such as avascular necrosis of the scaphoid and scaphoid non-union advanced collapse. The British Society for Surgery of the Hand has recently published guidelines on the management of suspected scaphoid fractures. Initial management involves radiographs and immobilisation with referral to a specialist within 7 days. Further management includ

The onset, progress and factors influencing degenerative arthritis of the wrist following scaphoid fracture non-union.

BACKGROUND/AIMS: Scaphoid non-union causes osteoarthritis but factors associated are poorly understood. We investigated the rate of osteoarthritis after scaphoid fracture non-union, and if duration and fracture location influenced arthritis and its severity. METHODS: This retrospective cross-sectional observational study of 278 consecutive cases with scaphoid fracture non-union retrieved data on demographics, non-union duration, fracture location, dorsal intercalated segment instability (DISI), severity and distribution of wrist arthritis. Patient Evaluation Measure (PEM) and Quality of Life

Assessing the Diagnostic Accuracy of Multimedia Messaging for the Diagnosis of Scaphoid Fractures.

Introduction:Use of smartphone messaging applications, such as WhatsApp, for communication within clinical teams is widespread. Various studies have shown multiple uses for smartphones and multimedia messaging in orthopedic clinical practice for coordinating clinical duties and diagnosing various injuries but none have assessed scaphoid fractures. Our aims were to: (1) assess diagnostic accuracy (sensitivity, specificity, positive predictive value, and negative predictive value) of viewing scaphoid X-rays on smartphones compared with desktop computers by using radiologist reports on the same X

Cost-Effectiveness of Immediate Magnetic Resonance Imaging In the Management of Patients With Suspected Scaphoid Fracture: Results From a Randomized Clinical Trial.

OBJECTIVES: Given the limited diagnostic accuracy of radiographs on presentation to the emergency department (ED), the management of suspected scaphoid fractures remains clinically challenging and poses an unknown economic burden to healthcare systems. We aimed to evaluate the cost-effectiveness of immediate magnetic resonance imaging (MRI) in the management of patients presenting with suspected scaphoid fracture to an ED in England. METHODS: A pragmatic, randomized, single-center trial compared the use of immediate MRI in the ED against standard care with radiographs only. Participants' use
