Arthroscopic Repair of Batter's Shoulder.

BACKGROUND: Batter's shoulder is a condition in which the posterior labrum is typically torn during the baseball swinging motion, producing a traumatic tear and posterior instability. The injury commonly occurs in the batter's lead shoulder due to repetitive microtrauma, raising concern for switch-hitters due to the cumulative stress of throwing and swinging on the lead shoulder. Instability is commonly caused by a posterior humeral force and relative shoulder adduction, which is most prevalent during a swing attempt at a low and outside pitch. INDICATIONS: Damage to the labrum during the acu

Posterior shoulder fracture-dislocation: A systematic review of the literature and current aspects of management.

PURPOSE: Posterior fracture-dislocation of shoulder is an infrequent traumatic event; however, most orthopaedic surgeons may face the challenge of treating it. The aim of this study is to review and summarise systematically the current principles of the management of this complex injury, and create a treatment algorithm. METHODS: Both PubMed and Scopus Databases were systematically searched for the terms "posterior shoulder fracture-dislocation" or "posterior glenohumeral fracture-dislocation" or "posterior glenoid fracture-dislocation" for articles written in English and published in the las

Neonatal morbidity associated with shoulder dystocia maneuvers.

OBJECTIVE: We sought to examine neonatal morbidity associated with different maneuvers used among term patients who experience a shoulder dystocia. STUDY DESIGN: We conducted a retrospective cohort study of all women who experienced a clinically diagnosed shoulder dystocia at term requiring obstetric maneuvers at a single tertiary care hospital from 2005 through 2008. We excluded women with major fetal anomaly, intrauterine death, multiple gestation, and preterm. Women exposed to Rubin maneuver, Wood's screw maneuver, or delivery of the posterior arm were compared to women delivered by McRobe

The duration of dislocation is the most important prognostic factor in chronic locked posterior shoulder dislocations treated with the modified McLaughlin surgical procedure: a multicentre study.

BACKGROUND: The diagnosis of posterior shoulder dislocation (PSD) is often overlooked as these injuries are not common, since clinical signs may be subtle and easily overlooked unless specifically evaluated. This study aimed to determine the factors related to clinical outcomes in patients with chronic locked PSD treated with the modified McLaughlin procedure. METHODS: The study included 22 patients from 5 different clinics who were diagnosed with chronic (> 6 weeks) locked PSD and underwent the modified McLaughlin procedure. The pre- and postoperative values of shoulder forward flexion, abdu

Relationship Between Posterior Shoulder Tightness and Lower-Limb Flexibility in College Baseball Players.

CONTEXT: Posterior shoulder tightness evaluated as limited glenohumeral internal rotation and horizontal adduction is a risk factor for throwing-related shoulder and elbow injuries. Given that the throwing motion uses the entire body kinematics, limited lower-limb flexibility might be associated with posterior shoulder tightness. Therefore, we aimed to investigate the relationships between posterior shoulder tightness and lower-limb flexibility in college baseball players. DESIGN: Cross-sectional study. SETTING: University laboratory. PARTICIPANTS: Twenty-two college baseball players (20 ri

Ten years of simulation-based shoulder dystocia training- impact on obstetric outcome, clinical management, staff confidence, and the pedagogical practice - a time series study.

BACKGROUND: To assess the impact of 10 years of simulation-based shoulder dystocia training on clinical outcomes, staff confidence, management, and to scrutinize the characteristics of the pedagogical practice of the simulation training. METHODS: In 2008, a simulation-based team-training program (PROBE) was introduced at a medium sized delivery unit in Linköping, Sweden. Data concerning maternal characteristics, management, and obstetric outcomes was compared between three groups; prePROBE (before PROBE was introduced, 2004-2007), early postPROBE (2008-2011) and late postPROBE (2012-2015). Modified technique for reconstructing reverse Hill-Sachs lesion in locked chronic posterior shoulder dislocation.

BACKGROUND: Among the uncommon and frequently missed orthopedics injuries is the posterior shoulder dislocation, accounting for about 2-4 % of all shoulder dislocations. Commonly, it is associated with the well-known reverse Hill-Sachs lesion. Several surgical treatment modalities such as transfer of subscapularis tendon or lesser tuberosity, humeral rotational osteotomy, osteochondral grafts were used to repair this defect. Hemiarthroplasty or total shoulder arthroplasty was used as salvage procedure in non-constructable defect or neglected old dislocation. PATIENTS AND METHODS: This study i

The floating shoulder: clinical and functional results.

BACKGROUND: Displaced ipsilateral fractures of the clavicle and the glenoid neck are a complex injury pattern that is usually the result of high-energy trauma. The treatment of these injuries is controversial, as good results have been reported with both operative and nonoperative treatment. METHODS: Nineteen patients who had sustained a displaced fracture of the glenoid neck with an ipsilateral clavicular fracture or acromioclavicular separation (floating shoulder) were retrospectively evaluated. The treatment was nonoperative in twelve patients and operative in seven. At the time of final f

[Histological and electron microscopic study of rotator cuff tear].

OBJECTIVE: To understand the changes of rotator cuff during tear and observe the pathologic and ultramicroscopic structure of rotator cuff. METHOD: Histological and ultra thin slides of samples of 36 cases of rotator cuff were made. RESULT: The degeneration of rotator cuff happened in synovium side, and then extended to the middle and deep sides of tendon. The deep side of cuff tendon could degenerate without synovium. The repair of tendon could be achieved with two types of healing. CONCLUSION: It would be difficult to achieve full repair when complete tear and deep side tear occurred to t

[Experimental and clinical results of ultrasound imaging in sports orthopedic soft tissue diseases].

Ultrasound scanning of the shoulders, knee joints, Achilles' tendon and muscles is presented as a noninvasive technique for the diagnosis of joint and soft tissue disorders. In experimental ultrasound studies on shoulders, knee joints, tendons and muscles of cadavers the diagnostic value of the method is assessed. The experimental observations were compared with clinical results collected in three years from 610 patients. Particularly characteristic findings in sports-induced soft tissue lesions are reviewed as examples.

EXTENSIVE OSTEOLYSIS AFTER THE USE OF A BIOABSORBABLE SUTURE ANCHOR: CASE REPORT AND LITERATURE REVIEW.

Bioabsorbable implants are very frequently used to treat rotator cuff and shoulder labrum injuries. Many researchers have observed small areas of osteolysis after treating pathological conditions of the shoulder using bioabsorbable anchors. Biological and mechanical theories have been put forward to account for the osteolysis caused by these materials. The case of a patient who was simultaneously treated for a rotator cuff lesion using the double-row technique and a Bankart lesion using bioabsorbable PLLA anchors and Fiber Wire®, and developed extensive osteolysis of the anatomical neck of the

Base of coracoid process fracture with acromioclavicular dislocation in a child.

Fracture of the coracoid process is a rare injury. It can be easily missed when associated with other injuries to the shoulder girdle, for instance, acromioclavicular joint (ACJ) dislocation. Clinical attention is easily drawn to the more obvious ACJ dislocation, hence, the need for further radiological evaluation. We report an unusual case of fracture of the base of coracoid process associated with a true acromioclavicular joint dislocation in a 12 year old boy, with no separation of the epiphyseal plate, as one might expect. Treatment also remains controversial. Our patient underwent open re

MR imaging of the shoulder: first dislocation versus chronic instability.

Traumatic dislocation of the shoulder is a frequent injury in the young and active population. An acute shoulder dislocation often denotes a onetime traumatic episode, whereas chronic shoulder instability indicates multiple recurrent dislocations. Imaging, in particular MRI, is a useful tool that can accurately demonstrate the typical soft tissue and osseous markers of shoulder dislocation. However, the ability to differentiate between first time versus recurrent dislocation based on imaging remains in question. In this article, we describe the underlying biomechanics of glenohumeral stability

Bilateral locked posterior shoulder dislocation overlooked for 15 months treated with the modified McLaughlin procedure: A case report.

Neglected bilateral posterior shoulder dislocation is a rare injury caused primarily by an epileptic seizure. The injury is usually associated with a reverse Hill-Sachs lesion in the anteromedial aspect of the humeral head. The modified McLaughlin technique may avoid instability and osteoarthritis when 20 to 40% of the articular surface is affected by reverse Hill-Sachs. In this article, we present the clinical results of a case overlooked in the literature for the longest time, i.e., for 15 months. A 46-year-old male patient was receiving treatment for epilepsy for five years. There was no fa

Increasing Trend in Pediatric Brazilian Jiu Jitsu Injuries Presenting to US Emergency Departments and an Injury Profile - A 10-Year Analysis.

Brazilian Jiu Jitsu (BJJ) is an increasingly popular martial arts discipline. Yet, BJJ injuries remain understudied in light of increasing popularity. In particular, pediatric injuries have a dearth of literature examining epidemiologic trends and injury profiles. As such, this study's purpose was to evaluate trends in pediatric Brazilian Jiu Jitsu (BJJ) injuries over a 10-year period and formulate an injury profile. The US Consumer Product Safety Commission's (CPSC) National Electronic Injury Surveillance System (NEISS) database was queried for martial arts related injuries from January 1, 20

Imaging the Injured Pediatric Athlete: Upper Extremity.

Increased physical activity in childhood has resulted in a large number of sports-related injuries. Although there is overlap between the sports-related injuries seen in pediatric and adult patients, important differences exist in the injury patterns of pediatric patients. These differences are related to the continuous changes in the developing skeleton and its relationship with adjacent soft tissues. The imbalance in strength between the growing bones and the nearby tendons and ligaments makes the bones prone to acute and chronic injuries. Acute injuries are more common in contact sports suc
