Plasma fructosamine, total proteins and albumin were estimated in a group of 77 sickle cell anaemia and 32 glucose-6-phosphate dehydrogenase (G6PD)-deficient Saudi Arabs and the results compared with those obtained in a group of 30 normal controls. The mean values of fructosamine in the sickle cell anaemia and G6PD-deficient patients were not statistically different from those of the controls. The mean values of the total proteins and albumin were also not significantly different from those of the normal group. It is concluded that fructosamine level is not affected by sickle cell anaemia and

Proteins C and S are vitamin K-dependent proteins with an essential anti-coagulant function. Protein C exists in an inactive form and is activated by a thrombin-thrombomodulin complex. Protein S combines with protein C and forms a stoichiometric complex which regulates coagulation in the presence of calcium. As patients with sickle cell disease (SCD) bear a high risk of developing thrombo-embolic disorders, we studied the coagulation derangement in 100 patients and 40 normal age- and sex-matched controls. The patients were clinically assessed and classified into sickle cell homozygotes (Hb SS)

Although large vessel thrombi are occasionally reported in patients with homozygous sickle cell disease, the role of intravascular coagulation in typical pain crises is controversial. Therefore, we studied 24 sickle cell patients during and between episodes of pain crisis, using several sensitive tests of hemostasis. Fibrinogen was measured by a clotting assay, beta-thromboglobulin (beta-TG) and fibrinopeptide A (FPA) were quantitated by radioimmunoassay, and protein C was determined by absorbing the zymogen from test plasma, activating it with thrombin-thrombomodulin complex, and measuring ac

Hand films of 100 patients with sickle cell disease were analyzed in order to establish the type, distribution, and incidence of terminal phalangeal sclerosis. Fifty hand films from patients with no known sickle cell disease matched for age and race were also studied. Twenty-four of 100 patients with sickle cell disease, with 14 less than 20 years old, showed clear evidence of terminal phalangeal sclerosis, while only 10% of the controls were positive. The incidence was significantly different from the control group (chi 2 = 4.1949, d.f. = 1, p less than 0.05) suggesting that terminal phalange

A simple technique is presented for determining the volume of blood to be exchanged in an isovolumetric partial exchange transfusion in order to achieve a desired final hematocrit or desired final percentage of normal hemoglobin. Experience from 16 exchange transfusions in 10 obstetric patients is used to improve the estimate of specific blood volume and, hence, the accuracy of the technique. A detailed protocol for this simple outpatient procedure is presented.

The sickle cell trait (Hgb-AS) and G-6-PD deficiency are two genetic defects which increase the hemolytic susceptibility of erythrocytes. As these two traits are common in Nigeria, blood transfusions with such defective cells are frequently given. In this study, the immediate and long-term effects of using either normal blood or blood with either or these defects for exchange transfusions in 115 neonates have been examined. Infants transfused with G-6-PD deficient blood were compared with those transfused with G-6-PD normal blood. Similarly, neonates transfused with Hgb-AS blood were compared …

The relationship between D-glucose-6-phosphate: NADP oxido-reductase (E.C.1.1.1.49; glucose-6-phosphate dehydrogenase; G6PD) deficiency and homozygous sickle cell (SS) disease was examined in 120 patients. The proportions of hemizygotes (22.6%) was slightly more than that observed, and the combined proportions of heterozygotes and homozygotes (28.3%) were slightly less than would be expected, in the general population, but the differences were not significant. However, the proportion of patients of abnormal G6PD status in the 10-19 years age group was 41.7%, significantly more than that found

BACKGROUND: Non-traumatic fat embolism syndrome (NT-FES) is a rare but frequently fatal complication of sickle cell disease (SCD) that is often associated with bone marrow necrosis. A delayed diagnosis or lack of standard criteria leads to a higher mortality rate. Firstline therapy in NT-FES is red cell exchange transfusion (RCE); however, therapeutic plasma exchange (TPE) is considered to be an adjunct or potential alternative in recent times.
CASE PRESENTATION: We report a case of a middle-aged male with HbSS sickle cell disease and α-thalassemia trait who presented with a vaso-occlusive cr

A 24-year-old Senegalese woman without remarkable history except anemia and iron deficiency related to excessive menstrual bleeding and sickle cell trait was admitted to our internal medicine department with 4-month fever, weight loss (-13 kg), dyspnea for limited efforts, intermittent productive cough, and bilateral metacarpophalangeal (MCP) and interphalangeal arthralgia. She was born and lived in France. She traveled previously to Senegal in 2015. She had no history of tobacco, alcohol, or drug use nor proximity with animals. She was taking no medication.

Unexplained cytopenias can be related to bone marrow necrosis (BMN) in critically ill patients as it can be encountered in several diseases or life-threatening conditions. We present the case of a 39-year-old woman with pancytopenia revealing a BMN in the setting of an acute pyelonephritis with septic shock, multiple organ failure and sickle cell trait. After a short review on the subject, we suggest that various haematological abnormalities occurring in critically ill patients may be related to a mild to severe marrow necrosis.

Renal acidification was evaluated in patients with sickle-cell disease (HvSS) with both oral NH4CI and NaHC03 and the results were compared to those of subjects with sickle-cell trait (HbAS) and controls. The pH of arterial blood was normal in HbSS subjects but their PC02 and [HC03] were lower than those of controls. In response to NH4CI, six of 20 HbSS subjects had an abnormal minimal urine pH (greater than 5.3) and the entire HbSS group had a higher mean value than did either controls or HbAS subjects. Since none of the six HbSS subjects had evidence of proximal tubular abnormalities, it was

Within the very large group of sickle cell hemoglobinopathies several phenocopy clusters exist. These can be conveniently classified as: 1) conditions in which HbA and S are present, 2) disorders where HbS is the predominant hemoglobin, and 3) the HbSF phenotype. This review will focus on distinguishing the genotype of these common phenocopies as well as new potential approaches to therapy.

PURPOSE: To evaluate the association of sickle-cell disease (SCD) and sickle-cell trait (SCT) disease with diabetic retinopathy (DR) in patients with diabetes mellitus (DM).
DESIGN: Population-based, retrospective cohort study utilizing data from the TriNetX Research Network, including 119 million patients across 80 health care organizations worldwide.
PARTICIPANTS: Diabetes mellitus patients (type 1 [T1DM] or 2 [T2DM]), with or without SCD and SCT, were included. Three cohorts were analyzed, including (1) DM patients without SCD, SCT, or sickle-cell/hemoglobin-C; (2) DM with SCD; and (3) DM
