Chemotherapy in Japanese encephalitis: are we there yet?

Chemotherapy in Japanese encephalitis (JE) is at present entirely supportive and not targeted at the virus. There are no available drugs to effectively counter the viral infection, thereby making the fight against JE a daunting task. With approximately 50,000 reported cases per year, nearly 10,000 deaths and 3 billion people living in endemic regions, it is imperative that the hunt for an effective drug be expedited. Prophylactic measures are effective against JE, but the problem plaguing all is the underdevelopment and inefficiency of medical services in developing countries. Combined to that

Review. Parallel studies of cocaine-related neural and cognitive impairment in humans and monkeys.

Cocaine users display profound impairments in executive function. Of all the components of executive function, inhibition, or the ability to withhold responding, has been studied the most extensively and may be most impaired. Consistent with these deficits, evidence from imaging studies points to dysregulation in medial and ventromedial prefrontal cortices, areas activated during performance of inhibition tasks. Other aspects of executive function including updating, shifting and decision making are also deficient in cocaine users, and these deficits are paralleled by abnormalities in patterns

Early effects of climate change: do they include changes in vector-borne disease?

The world's climate appears now to be changing at an unprecedented rate. Shifts in the distribution and behaviour of insect and bird species indicate that biological systems are already responding to this change. It is well established that climate is an important determinant of the spatial and temporal distribution of vectors and pathogens. In theory, a change in climate would be expected to cause changes in the geographical range, seasonality (intra-annual variability), and in the incidence rate (with or without changes in geographical or seasonal patterns). The detection and then attributio

Attenuated sensorimotor hemodynamic responses and altered cortical dynamics during passive proprioceptive-related stimulation after stroke: a multidimensional fNIRS study.

OBJECTIVE: This study aimed to use functional near-infrared spectroscopy to investigate abnormalities in cortical activity and dynamic network mechanisms associated with proprioceptive processing after stroke. We integrated conventional hemodynamic features, latent-state organization, directed connectivity, and recovery-phase complexity analyses to explore multidimensional changes in cortical dynamics. METHODS: Patients with stroke and healthy controls underwent baseline motor and sensory assessments and completed a block-designed proprioceptive task during fNIRS recording, consisting of six

Evaluating large language models for clinical note processing: local fine-tuning and internal-external validation using electronic health records from South Asia.

OBJECTIVE: Large Language Models (LLMs) hold the potential for clinical task-shifting by processing unstructured clinical text, enabling tasks such as clinical concept extraction and medical question answering from electronic health records. If implemented reliably, such approaches may benefit over-burdened healthcare systems, particularly in resource-limited settings and for traditionally overlooked populations, provided that local fine-tuning is supported by appropriate clinical and technical expertise. However, this powerful technology remains largely understudied in real-world contexts, pa

Heat stress and strain in commercial construction workers in the summer: A pilot study.

Construction workers are 13 times more likely to die from heat-related illnesses than workers in other job industries. This elevated risk is attributed to excessive heat stress from environmental exposure, metabolic heat generated from physical labor, and the insulating effects of protective clothing. Levels of heat stress may vary across job types, reflecting the diversity of tasks performed and the work environment. Despite elevated risks, heat strain assessed by core temperature (Tcore) in construction workers during summertime in the United States has not been evaluated previously. Thirty-

Repeated TTS exposures in monkeys: alterations in hearing, cochlear structure, and single-unit thresholds.

The findings of a number of studies investigating the effects of excessive sound on hearing have indicated that the correspondence between behavioral, physiological, and histological measures of noise-induced hearing loss may be markedly dependent upon the sensitivity of the particular measure. Recent studies demonstrating significant changes in the responses of single auditory neurons following brief exposures to pure tones suggest that single-unit activity may be a sensitive indicator of physiological insult to the organ of Corti's sensory cells. In addition, the long-lasting nature of the c

Fatigue in primary Sjögren's syndrome is associated with an objective decline in physical performance, pain and depression.

BACKGROUND: Exercise capacity (EC) and physical activity (PA) are relevant predictors of mortality in patients with coronary heart disease (CHD) but the CHD-specific long-term trajectories of these outcomes after a cardiac rehabilitation (CR) program are not well known. The main objective of this study was to determine the mean change in EC (6-min walking test (6MWT) distance) in CHD patients at 12 months after a CR program compared to the end of the program. We also performed a series of exploratory analyses: (i) estimating the decile shifts and the typical (median) individual change for EC,

Neuropsychological Assessment Using Portable Automated Rapid Testing in Cognitively Healthy Older Adults: Test-Retest Reliability Study.

BACKGROUND: Remote, scalable cognitive assessment could improve detection and longitudinal monitoring of age-related cognitive change, but few studies have validated comprehensive digital neuropsychological batteries administered entirely at home. OBJECTIVE: This study aims to evaluate the feasibility, acceptability, and short-term test-retest reliability of a remotely delivered digital neuropsychological battery (portable automated rapid testing [PART]) in cognitively healthy older adults. METHODS: We screened 82 English-speaking, cognitively healthy older adults aged 50 to 85 years and mai

Discordance Between Textual Reasoning and Visual Interpretation in Large Language Models for Low Back Pain: Cross-Sectional Quantitative Evaluation and Exploratory Multimodal Stress Test.

BACKGROUND: Large language models (LLMs) are rapidly evolving from text-based agents to multimodal systems capable of interpreting medical images. While their textual reasoning has improved, the safety implications of this shift remain underexplored, specifically regarding the alignment between visual interpretation and textual advice in low back pain (LBP) management. OBJECTIVE: This study aims to evaluate the performance of 10 commonly used and highly representative LLMs, including paid models such as ChatGPT 5 Plus (OpenAI) and reasoning-oriented models like Qwen3-Thinking (Alibaba/Qwen te

Inter-isolate similarity networks reveal task-dependent graph signal for antimicrobial resistance prediction from MALDI-TOF mass spectra.

Antimicrobial resistance (AMR) prediction from matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectra is typically formulated as instance-wise classification, even though clinical isolates are naturally related by spectral similarity. We ask whether isolate-level similarity carries AMR information beyond per-isolate spectral classifiers, and what breaks first when we move such a model to another clinical site. We construct k-nearest-neighbor graphs from train-normalized spectra and introduce Hybrid-GNN, which combines a per-isolate multilayer perceptron encoder wit

State of clinical AI in 2026.

Clinical artificial intelligence (AI) has advanced rapidly, with frontier large language models now matching or exceeding physician performance on simulated diagnostic reasoning and clinical decision-support tasks. Yet adoption has outpaced the evidence base: fewer than 5% of cleared U.S. Food and Drug Administration (FDA) AI/machine learning (ML) devices have undergone peer-reviewed evaluation and prospective randomised trials remain scarce. This narrative review synthesises evidence from ARISE's State of Clinical AI Report 2026 across six domains: model performance and human benchmarking, ev

The mPFC-PAG in defensive coping: A correlational but not causal role in shifting strategy.

Defensive behaviors under threat are fundamental for survival, and their dysregulation contributes to psychiatric disorders. The medial prefrontal cortex (mPFC) and periaqueductal gray (PAG) pathway are implicated in regulating defensive behaviors, but their specific role in choosing between coping strategies remains unclear. Here, we investigated these brain regions using the two-way active avoidance (TWAA) task in male rats. Anatomical tracing confirmed a robust unidirectional projection from layer V neurons of mPFC subregions (including ACC, PL, IL) to the PAG. Using c-Fos immunohistochemis

Decomposing response inhibition: A POMDP model.

Inhibitory control is a core cognitive function whose competence varies across the population, with impairments often observed in psychiatric conditions such as attention deficit hyperactivity disorder (ADHD). The Stop Signal Task (SST) is a widely used paradigm for assessing this ability. However, conventional formalizations of SST performance, such as the independent race model, rely on assumptions that are frequently violated in modern experimental designs. Furthermore, they typically fit only mean reaction times, overlooking crucial trial-by-trial dynamics. To address these limitations, we

Meta-learned dynamic hierarchical fusion for robust multi-scale object classification.

A key limitation of existing feature fusion methods is their static nature: they learn dataset-specific integration policies during training that remain fixed at inference. This restricts adaptability to new domains and input characteristics, often causing brittle generalization, hierarchical information decay, and inefficient relational modeling. We propose Meta-Learned Dynamic Hierarchical Fusion (MDHF), which reframes feature fusion as context-conditioned policy generation. MDHF uses a bi-level meta-learning objective to learn a transferable prior over fusion strategies, enabling sample-spe

Adaptive concept rule learning for interpretable and generalizable medical image classification.

The pursuit of decision safety in clinical applications calls for medical image classification models that are not only accurate but also transparent enough for clinicians to audit the visual evidence and decision logic behind each diagnosis. Existing concept-based models ground predictions in human-interpretable concepts (i.e., diagnostic visual signs), but they typically provide only instance-level explanations without exposing the global classification logic, and their soft concept representations are prone to concept leakage, where unintended information undermines both interpretability an

The Cortical Source of the P300 ERP Generator in the Associative Learning Task: A Consensus of Four Inversion Algorithms.

Background: Associative learning, the process of binding stimuli, responses, and outcomes, is critical for behavioral adaptation. While event-related potentials (ERPs) like the P300 effectively index the cognitive effort and context updating required during trial-and-error learning, the precise cortical generators of these signals and their shift across development remain difficult to isolate due to the electroencephalography (EEG) inverse problem. The aim of the study was to examine the differences in P300 localization depending on the associative learning task stage and participants' age. Me

From Monoamines to Systems Psychiatry: Rewiring Depression Science and Care (1960s-2025).

Major depressive disorder (MDD) was long framed as a single clinical entity arising from a linear stress-monoamine-hypothalamic-pituitary-adrenal (HPA) axis cascade. This view was shaped by forced swim and learned helplessness tests in animals and by short-term symptom-based trials using scales such as the Hamilton Depression Rating Scale (HAM-D) and the Montgomery-Åsberg Depression Rating Scale (MADRS). This "unitary cascade" view has been dismantled by advances in neuroimaging, immune-metabolic profiling, sleep phenotyping, and plasticity markers, which reveal divergent circuit-level, inflam

Differences in the neuropsychological profiles of high-functioning TSC adults with and without epilepsy.

BACKGROUND: Tuberous sclerosis complex (TSC) is a multisystem genetic disorder often accompanied by neuropsychiatric symptoms, including epilepsy and cognitive impairments. While the cognitive impact of epilepsy in TSC is well documented in children, there is a paucity of studies examining neuropsychological functioning in high-functioning adults, particularly in relation to epilepsy status. OBJECTIVE: This study aimed to investigate cognitive functioning in adults with clinically or molecularly confirmed TSC, without intellectual disability or autism spectrum disorder (ASD), with a specific
