Efficacy of the application of leukocyte and platelet-rich fibrin (L-PRF) on alveolar ridge preservation. A randomized controlled clinical trial.

INTRODUCTION: Alveolar ridge preservation (ARP) is a well-defined treatment performed to reduce bone dimensional changes occurring during the healing of post-extraction sockets to allow for adequate implant placement. Leukocyte and platelet-rich fibrin (L-PRF) has been showing to potentially promote bone and tissue regeneration during wound healing. Therefore, the aim of this study is to evaluate its efficacy for ARP when applied to fresh extraction sockets, in comparison with spontaneous healing. MATERIALS AND METHODS: Twenty-seven patients with hopeless non-molar teeth were treated. After r

Effect of reciprocal headgear forces on the calvarium: A finite element study.

INTRODUCTION: Orthopedic appliances continue to be used for various sagittal, vertical, and transverse corrections of the maxilla. Although cranial growth can continue to adulthood, no study has drawn attention to the effects of headgear forces on the calvarium, in which anchorage is taken. This study focused on the different biomechanical responses on the calvarium of young children wearing a high-pull headgear of varying forces, using a 3-dimensional finite element analysis and the possible implications of such changes on the human skull. METHODS: A 3-dimensional finite element model of a 9

A cost-utility analysis between decompression only and fusion surgery for elderly patients with lumbar spinal stenosis and sagittal imbalance.

Lumbar spinal stenosis (LSS) and sagittal imbalance are relatively common in elderly patients. Although the goals of surgery include both functional and radiological improvements, the criteria of correction may be too strict for elderly patients. If the main symptom of patients is not forward-stooping but neurogenic claudication or pain, lumbar decompression without adding fusion procedure may be a surgical option. We performed cost-utility analysis between lumbar decompression and lumbar fusion surgery for those patients. Elderly patients (age > 60 years) who underwent 1-2 levels lumbar fusio

Premaxillary Repositioning in the Severe Form of Bilateral Cleft Lip and Palate.

Severe forms of bilateral cleft lip and palate remain a challenging issue. Although nasoalveolar molding dramatically improves overall treatment success, the position of the premaxilla often remains dislocated. The authors attempted to relocate the malpositioned premaxilla into the correct position to obtain the correct three-dimensional (3D) maxillary arch structure and growth. Eight patients with severe bilateral cleft lip and palate were treated with premaxillary osteotomy for premaxilla repositioning. The position of the premaxilla was measured directly using cephalometry. Two raters inclu

Significant transaortic gradient after direct flow aortic valve implantation.

A patient with severe aortic valve disease and high surgical risk underwent Direct Flow (DF) valve implantation. Anatomical assessment (Trans-esophageal echocardiography (TEE) and CT scan) revealed a 3-leaflet aortic valve (annulus diameter 23.4 mm) that was functionally bicuspid because of complete and linearly calcified fusion of noncoronary and right cusps. The valve had severe stenosis (peak/mean gradients of 70/45 mm Hg) and moderate to severe regurgitation. A balloon valvuloplasty (semicompliant 23 mm × 45 mm balloon, 2 inflations) was performed with persistence of balloon waist. A 25 DF

A novel method for laryngeal mask airway size selection in paediatric patients.

CONTEXT: The successful use of the laryngeal mask airway in children partly depends on the correct selection of size. Most anaesthesiologists rely on the weight-based table which is often difficult to remember. A simple method allowing an adequate choice of the correct size may be highly desirable. OBJECTIVES: To test the hypothesis that the size of the external ear (pinna) of the child may be used as proxy for the required size of laryngeal mask airway. DESIGN: A descriptive study. SETTING: King Khaled Eye Specialist Hospital, Riyadh, Saudi Arabia. PATIENTS: Two hundred and ten paediatric

Physical properties of root cementum: part 21. Extent of root resorption after the application of 2.5° and 15° tips for 4 weeks: a microcomputed tomography study.

INTRODUCTION: Microcomputed tomography offers a unique opportunity to accurately examine orthodontically induced inflammatory root resorption. The aims of this study were to quantify, in 3 dimensions, the amount of root resorption caused by placing heavy and light distal root tipping forces on premolars and to compare the prevalence of root resorption in different areas of the tooth. METHODS: Thirty maxillary first premolars from 15 patients who were to have these teeth extracted as part of their orthodontic treatment were selected for this study. Each tooth in the same patient was randomly c

Effect of immediate and delayed post space preparation on the apical seal of root canals obturated with different sealers and techniques.

UNLABELLED: During mechanical preparation of the post space, the root canal filling may be twisted or vibrated, depending on several factors associated with the preparation technique and quality of filling. OBJECTIVE: The purpose of this study was to investigate the effect of immediate and delayed post space preparation on the integrity of the apical seal. MATERIAL AND METHODS: Sixty-four extracted human incisors were biomechanically prepared using the step-back technique. Sixty roots were randomly assigned to 6 experimental groups of 10 teeth each and the remaining 4 roots served as positiv

Novel CAD/CAM-splint-based navigation protocol enhances intraoperative maxillary position control in orthognathic surgery: a case control study.

BACKGROUND: Virtual surgical planning for orthognathic surgery typically relies on two methods for intraoperative plan transfer: CAD/CAM occlusal splints and patient-specific implants (PSI). While CAD/CAM splints may offer limited accuracy, particularly in the vertical dimension, PSIs are constrained by higher costs and extended preparation times. Surgical navigation has emerged as a potential alternative, but existing protocols often involve invasive registration or lack transparent evaluation. This study introduces a novel protocol for point-based optical navigation using modified CAD/CAM sp…

A Comparative Evaluation of Enamel Wear Against Different Surface Finished Ceramics: An In Vitro Study.

BACKGROUND: An essential factor in the clinical acceptability of all ceramic restorations is the degree of tooth enamel wear. Wear of human enamel has been shown to produce traumatic occlusion, loss of vertical dimension, supra eruption of opposing teeth, periodontal disintegration, and temporomandibular dysfunction. To assess and compare enamel wear against two different ceramics (IPS Empress and Zirconia) and against three different ceramic surface treatments (autoglazed, overglazed, and polished). METHODS: Zirconia and IPS Empress (Ivoclar Vivadent, Liechtenstein) ceramic samples totaled 3…

Psychopathological profile and sagittal alignment in low-back pain.

INTRODUCTION: Low-back pain requires comprehensive care using a biopsychosocial model. The psychologic dimension plays an important role, but the link between sagittal alignment and a given psychopathological profile is little studied. The aim of this study was to analyze the psychopathological profiles and sagittal parameters of a population with low-back pain and to assess the link. MATERIAL AND METHODS: 205 patients, with a mean age of 49.6 years (range, 18-70 years), presenting chronic common low-back pain without radicular involvement, were included prospectively. Mood scores comprised:
