Fluctuating corticosteroid-responsive auditory neuropathy/dyssynchrony is suggestive of central nervous system pathology.

OBJECTIVE: To highlight diagnostic and management features of auditory neuropathy/auditory dyssynchrony (AN/DS) due to central demyelinating disorder. PATIENTS: A child with AN/DS due to central nervous system pathologic findings. INTERVENTIONS: Audiometry, auditory brainstem response (ABR) test, otoacoustic emission test, magnetic resonance imaging (MRI) with gadolinium contrast, intravenous corticosteroid treatment, antiviral treatment, stereotactic biopsy, and cyclophosphamide immunomodulation. MAIN OUTCOME MEASURES: Pure-tone audiometry, speech discrimination testing, ABR, and MRI. RES

Glutathione-dependent antioxidant systems in the mammalian inner ear: effects of aging, ototoxic drugs and noise.

Glutathione and glutathione-related enzymes protect against oxidative (free radical) cell injury. This study presents basic information on this antioxidant system in inner ear tissues and preliminary results of the influence of age, ototoxic drugs and noise. These conditions affect inner ear function, possibly through free radicals, and are therefore expected to affect cellular defense mechanisms. In 24-month old Fischer 344 rats, a standard model for aging, glutathione levels were significantly decreased in the auditory nerve by 86% as compared to 3-month old rats but remained unchanged in ot

[Auditory brainstem responses in the diagnosis of retrocochlear hearing loss: selected case reports].

In this study, four cases of patients with retrocochlear hearing loss are presented (acoustic neuroma, cerebellopontine angle tumour, the fourth ventricle region tumour, inflammatory or vascular lesions in the fourth ventricle region). Standard audiometric tests, auditory brainstem response and radiological examinations such as CT scan and MRI were performed in all patients. The abnormal ABR observed in all cases suggested retrocochlear hearing loss. The presence of retrocochlear disorders was confirmed by CT or MRI. The analysis of presented cases showed that the abnormal ABR results should b

The localization and specificity of guinea pig inner ear antigenic epitopes.

In this study, we investigated the relative localization of some antigenic epitopes in the inner ear. The inner ear protein antigens were extracted from various parts of the guinea pig inner ear. Brain, kidney, lung, heart and liver extracts were also obtained. We found by SDS-polyacrylamide gel electrophoresis that total inner ear extracts separated into three high concentration polypeptide bands with molecular weights of approximately 30, 42, 58 kd and three low density bands of 20, 25 and 35 kd. The 30 kd band was found mainly in the extract of the spiral ganglion and the acoustic nerve in

Phase II study of mTORC1 inhibition by everolimus in neurofibromatosis type 2 patients with growing vestibular schwannomas.

Neurofibromatosis type 2 (NF2) is a genetic disorder with bilateral vestibular schwannomas (VS) as the most frequent manifestation. Merlin, the NF2 tumor suppressor, was identified as a negative regulator of mammalian target of rapamycin complex 1. Pre-clinical data in mice showed that mTORC1 inhibition delayed growth of NF2-schwannomas. We conducted a prospective single-institution open-label phase II study to evaluate the effects of everolimus in ten NF2 patients with progressive VS. Drug activity was monitored every 3 months. Everolimus was administered orally for 12 months and, if the decr

[Otoacoustic emissions, auditory evoked potentials, pure tone thresholds and speech intelligibility in cases of auditory neuropathy].

The combined findings of "normal evoked otoacoustic emissions (EOAE) and absent or markedly disturbed auditory evoked potentials from the brainstem and/or cortex" has been named auditory neuropathy in the English literature. The pure tone thresholds in these patients range from mild to severe loss. The loss of speech comprehension is out of proportion in comparison to the pure tone threshold elevation. This combination of findings suggests that cochlear function and in particular outer hair cell function, is normal in these patients but also suggests that the inner hair cell / VIII nerve funct

Auditory Brainstem Response Altered in Humans With Noise Exposure Despite Normal Outer Hair Cell Function.

OBJECTIVES: Recent animal studies demonstrated that cochlear synaptopathy, a partial loss of inner hair cell-auditory nerve fiber synapses, can occur in response to noise exposure without any permanent auditory threshold shift. In animal models, this synaptopathy is associated with a reduction in the amplitude of wave I of the auditory brainstem response (ABR). The goal of this study was to determine whether higher lifetime noise exposure histories in young people with clinically normal pure-tone thresholds are associated with lower ABR wave I amplitudes. DESIGN: Twenty-nine young military Ve

Vestibular function in children with auditory neuropathy spectrum disorder.

OBJECTIVE: Children with auditory neuropathy spectrum disorder (ANSD) account for about 10% of paediatric patients referred for cochlear implantation. Vestibulopathy may be associated with ANSD, and may have implications when formulating management plans in this patient group. We wanted to determine the incidence and predictive factors for vestibulopathy in this patient group to guide vestibular testing in this patient population, and give insight to the aetiology of ANSD. METHODS: We reviewed the outcomes of vestibular function testing in a cohort of paediatric patients with ANSD. RESULTS:

Tobramycin sulfate: a summary of worldwide experience from clinical trials.

Tobramycin sulfate, a new bactericidal aminoglycoside antibiotic, has been evaluated in 3,506 patients. Its in vitro activity is similar to that of other aminoglycoside antibiotics, except that it is more active against Pseudomonas aeruginosa. Clincal studies have shown that tobramycin sulfate is well tolerated and effective in the treatment of the infections listed below when they are caused by susceptible gram-negative organisms and by staphylococci: infections of the respiratory tract (including cystic fibrosis), central nervous system, skin, soft tissue, and bone (including burns); urinary
