Information control for a general ophthalmic practice.

An integrated ophthalmic office computer system is now functional at five sites. The system automates clerical functions such as appointment scheduling, billing, and preparation of doctors' schedules and insurance forms. It eliminates the need for filing and storage of records. The medical system records the history and physical examination of the eye, offers a broad differential diagnosis, writes prescriptions for glasses and drugs, types referral letters, orders contact lenses from specifications, and catalogues photographic and other patient data vital to the medical record. The accounting

Where are the "rights" in SRHR?

The impetus to write this editorial came from a commentary written by the late Professor Jonathan Mann and Deborah Maine in 19851 , the mentors and advocates of this journal, when they asked the question "where is the M in MCH?" That editorial arose from the global tendency at the time to ignore programming and policies on maternal health but rather to concentrate on child health despite the global joint composition of maternal and child health as a critical determinant outcome. The editorial led to the re-ordering of policies, research, and programs to focus on maternal health, especially in

[Under a pestilent land of lies: misinformation and denialism in the smallpox epidemic in Ceará, Brazil, 1900-1905, and the covid-19 pandemic in Brazil, 2020-2022].

The article draws a parallel between two pathological phenomena with great social impact in different settings: the smallpox epidemic in Cea … vaccination. The analysis makes extensive use of press reports concerning the covid-19 pandemic and writings by the pharmacist Rodolfo Teófilo, a key figure in the smallpox vaccination campaign in Ceará.

[Interpretation of consensus on MRI of endolymphatic hydrops in patients with suspected hydropic ear disease and domestic research progress].

Endolymphatic hydrops(EH) is considered the histological hallmark of Meniere's disease. Visualization of EH has been achieved by special sequences of inner ear magnetic resonance imaging(MRI) with gadolinium-based contrast-agent via intravenous or intratympanic administration. Although it has been applied for more than ten years since 2007, a unified view on this technique has not yet been achieved. In 2022, Yang Jun and Duan Maoli et al led the organization to write the international expert consensus on MRI of EH. This article interprets the main contents and reports related progress.

[Psychopathological aspects of "raptus melancholicus"].

The term "raptus melancholicus" is rarely used in clinical or scientific discussions. There is a lack of data on frequency and attribution to psychiatric disorders. It is found more often in the older literature where authors write of detailed personal experiences with this malicious phenomenon, but the published information is isolated as well. Clinicians and forensic psychiatrists judge raptus melancholicus to represent a risk for suicidal attempts as it appears suddenly and unexpectedly, arising from a state of delayed and inhibited psychomotor motility. This enigma, which sounds quite para

Don't look back in anger: learning from my mistakes with Mark and his family.

The article offers a reflective account of work with Mark and his family. The work concerned a teenage boy who had presented with elective mutism; when this ameliorated he presented later with obsessive compulsive disorder. In the therapy there was a recurrent pattern of disappointment for all concerned that positive change, which at times looked so likely, did not materialize. A description of the work is given, interspersed with reflections about my feelings at the time, and current thoughts and ideas about what might have gone wrong and why. Reflections on the process of writing this piece …

Dr. Arthur Conan Doyle and the case of congenital syphilis.

In 1894, Dr. Arthur Conan Doyle wrote "The Third Generation," a short story involving the transmission of congenital syphilis from generation to generation. Analysts of his writings have interpreted the pathogenetic mechanism involved in modern terms: infection of mother by father and then transplacental infection of the fetus. However, a review of the contemporary literature and the history of the concepts of congenital and "hereditary" syphilis demonstrates that the late 19th-century understanding of the process involved a Lamarckian transmission of paternal infection, via the sperm at the m…

Thomas Annandale and the first report of successful orchiopexy.

PURPOSE: We investigate the surgeon and factors behind the first report of successful orchiopexy. MATERIALS AND METHODS: We reviewed the first reports of orchiopexy, and the work and writings concerning Thomas Annandale and the time in which he practiced. RESULTS: Annandale was a surgeon in Edinburgh, Scotland, in the late 19th century. In 1877 he successfully brought down an ectopic testicle in a 3-year-old boy. This first reported successful orchiopexy was due in large part to Lister's antiseptic technique. CONCLUSIONS: Annandale performed the first successful orchiopexy by integrating th…

William Porterfield (ca. 1696-1771) and his phantom limb: an overlooked first self-report by a man of medicine.

EARLY REPORTS OF phantom limbs by Ambroise Paré and René Descartes were based on second- or third-hand descriptions provided by amputees. William Porterfield (ca. 1696-1771) was a prominent Scottish physician and was possibly the first man of medicine to write about his experiences after having a leg amputated. Porterfield was an authority on vision; he devised the first optometer and examined accommodation after cataract operations. Rather than using the phenomenon of a phantom limb to question the veracity of the senses (as Descartes had done), Porterfield integrated his phantom limb experie

A Three-Year Longitudinal Study of Reading and Spelling Difficulty in Chinese Developmental Dyslexia: The Matter of Morphological Awareness.

In the present study, we used a three-time point longitudinal design to investigate the associations of morphological awareness to word reading and spelling in a small group of those with and without dyslexia taken from a larger sample of 164 Hong Kong Chinese children who remained in a longitudinal study across ages 6, 7 and 8. Among those 164 children, 15 had been diagnosed as having dyslexia by professional psychologists, and 15 other children manifested average reading ability and had been randomly selected from the sample for comparison. All children were administered a battery of tasks i

Beyond phonological and morphological processing: pure copying as a marker of dyslexia in Chinese but not poor reading of English.

To examine cognitive correlates of dyslexia in Chinese and reading difficulties in English as a foreign language, a total of 14 Chinese dyslexic children (DG), 16 poor readers of English (PE), and 17 poor readers of both Chinese and English (PB) were compared to a control sample (C) of 17 children, drawn from a statistically representative sample of 177 second graders. Children were tested on pure copying of unfamiliar stimuli, rapid automatized naming (RAN), phoneme deletion, syllable deletion, and morphological awareness. With children's ages and Raven's nonverbal reasoning statistically con

In contrast, this was the case in even 20 % of the children with difficulties in reading/writing and of those with low IQ. Compared to girls, boys in the control group had a 150% higher risk for matching the criteria of one of the ADHD subtypes in parents’ ratings, whereas boys with learning difficulties and those with low IQ had an even 200% t …

The orthographic sensitivity to written Chinese in the occipital-temporal cortex.

Previous studies have identified an area in the left lateral fusiform cortex that is highly responsive to written words and has been named the visual word form area (VWFA). However, there is disagreement on the specific functional role of this area in word recognition. Chinese characters, which are dramatically different from Roman alphabets in the visual form and in the form to phonological mapping, provide a unique opportunity to investigate the properties of the VWFA. Specifically, to clarify the orthographic sensitivity in the mid-fusiform cortex, we compared fMRI response amplitudes (Exp.

Integrating job, Jesus' passion, and Buddhist Metta to bring meaning to the suffering and recovery from breast cancer.

Coming face-to-face with death was a spiritual crisis. My family and I suffered individually and collectively during my treatment and recovery for locally advanced breast cancer. Like Job, I learned that it takes tremendous energy to ruminate about the causes of suffering and to protest innocence with little gains in wisdom. Wisdom came as I deeply experienced a passion narrative based on the life of Jesus with reference to the psychological benefits as extolled by Wilkes. The grueling experience of treatment for locally advanced breast cancer broke my body and forced me to experience Easter S

Computer speech-based remediation for reading disabilities: the size of spelling-to-sound unit in a transparent orthography.

This study was designed to assess the effects of four reading-training procedures for children with reading disabilities (RD) in a transparent orthography, with the aim of examining the effects of different spelling-to-sound units in computer speech-based reading. We selected a sample of 83 Spanish children aged between 7 years 1 month and 10 years 6 months (M = 105.2, SD = 7.8) whose pseudoword reading performance was below the 25th percentile and IQ > 90. The participants were randomly assigned to five groups: (a) the whole-word training group (WW) (n = 17), (b) the syllable training group (

[Kanji-predominant alexia with agraphia in opticospinal multiple sclerosis].

Alexia with agraphia is very rare symptom in multiple sclerosis. We present a patient of opticospinal multiple sclerosis with kanji-predominant alexia with agraphia. A 55-year-old, right-handed man was admitted to our hospital because of difficulty in reading and writing in August 2001. The patient had been diagnosed as having relapsing-remitting opticospinal multiple sclerosis eight years prior to admission. Language examination showed alexia with agraphia predominantly affecting kanji and also mild naming difficulties, but a good comprehension and a normal repetition. T2-weighted MRI demonst…
