The intra-ocular pressure in 76 patients, undergoing Neodymium: YAG laser surgery was measured just before treatment, and one and two hours afterwards. The fellow eye served as a control. Serious elevation of the intra-ocular pressure was demonstrated, especially in capsulotomy in the aphakic eye, to a lesser extent in pseudophakic eyes. Intra-ocular pressure elevation of more than 20 mm was seen in iridectomies. Timolol 1/2% eyedrops given before Neodymium: YAG laser treatment minimized the elevation of the intra-ocular pressure. Prolonged follow-up of the intra-ocular pressure is recommended

We present an initial report on flexible anterior chamber lens implantation in 14 aphakic or pseudophakic bullous keratopathy patients undergoing penetrating keratoplasty. With a minimum follow-up of 12 months, the improvement in visual acuity and graft clarity is encouraging.

In a prospective clinical study 54 postcataract membrane dissections (51 patients) were performed using a Q-switch Nd-YAG laser. Under standardized conditions all patients were pretreated with prostaglandin inhibitors and received topical steroids and beta-blockers in low concentrations, as well as carbonic anhydrase inhibitors orally. The changes in visual acuity and the response of IOP to total energy input were analyzed. There was no statistically significant correlation between postoperative rise in IOP and total energy input. The clinical relevance of the results is discussed.

The use of intraocular lenses (IOLs) to replace cataractous lenses has raised question of possible retinal damage due to exposure to ultraviolet (UV) radiation. Three groups of cynomolgus monkeys underwent cataract operations and either received normal UV-transparent IOLS or UV-opaque IOLs, or were left aphakic. All eyes were later exposed to UV light for varying periods of time. Exposure to UV light produced severe retinal lesions. A high water content lens (Scanlens 75, +14.5 dioptres) and a low water content lens (Bausch and Lomb Soflens H3, +14.5 dioptres) were compared with respect to corneal thickness response after 6 hours of wear in a provocative test on 12 aphakic eyes. Epinephrine maculopathy is a reversible macular change that occurs in aphakic eyes being treated with topical epinephrine compounds. The first symptom is transient blurred vision, which can be followed by decreased visual acuity, flame-shaped hemorrhages, and cystoid macular edema. A consecutive series of pseudophakic retinal detachments was statistically compared with a matched group of aphakic retinal detachments. Distribution of breaks, anatomic results, and final visual acuities were tabulated. Success in reattachment was equal in the two groups, but final visual acuities were better in the aphakic group. Emphasis was placed on the intraoperative ophthalmoscopic examination of pseudophakic eyes with compromised peripheral fundus visibility.

In patients treated with oral indomethacin for four weeks after intracapsular cataract extraction, five of 57 eyes (9%) had cystoid macular edema four to six weeks postoperatively as shown by fluorescein angiography. In a control group not treated with indomethacin, 15 of 34 eyes (44%) had cystoid mascular edema. Oral indomethacin, therefore, caused a statistically significant decrease in the incidence of aphakic cystoid macular edema.

After hesitation in the indication for the implantation of irisclip-lenses, experience shows that indications which have not yet been planned must be seriously considered: for instance cataract in patients with retinitis pigmentosa. In a series of cataract patients excluding myopic individuals, under age 60 years, and cases in which vitreous loss occurred, retinal detachment was no less frequent after intracapsular cataract extraction and Sputnik iris supported lenses than in controls. Both groups were followed up for a minimum of two years. The detachments predominantly occurred from retinal breaks in areas of the retina that looked normal preoperatively.

A series of 75 eyes underwent pars plana vitrectomy for progressive diabetic traction retinal detachment of the macula. In all cases, the media was sufficiently clear to allow preoperative macular examination. The overall visual improvement was higher in phakic eyes (80%) than aphakic eyes (72%) and the rate of neovascular glaucoma was less in phakic eyes.
