The talus is a critical bone in the foot, located between the tibia and calcaneus, playing a key role in weight-bearing and force transmission. OBJECTIVES: To evaluate the use of lag screw only fixation of noncomminuted oblique fractures of the lateral malleolus in patients younger than fifty years of age.
DESIGN: Prospective evaluation.
SETTING: Level I trauma center.
PATIENTS AND PARTICIPANTS: Forty-seven ankle fractures with simple oblique patterns and no comminution that were long enough to accept two lag screws placed at least 1 centimeter apart were prospectively evaluated. All patients were younger than fifty years of age. There were twenty-three ligamentous SE4, eighteen bimalleolar SE4, and six PE4 fractures.

Seventeen transcervical/basal femoral neck fractures in children were treated by free fibular graft and cancellous lag screw. Two cases were failures of a previous surgery, and 15 had been untreated for 3 weeks or more. Four cases had radiological evidence of avascular necrosis of the head and one of the neck preoperatively; five cases had neck resorption. A bony bridge was created in the growth plates of young rabbits. Weekly computed tomography (CT) scanning was done in the axial (transverse) plane for 3 weeks.
