Improving affirming care for sexual and gender minority veterans.

Lesbian, gay, bisexual, and transgender (LGBT) populations face a range of health disparities that all health care systems must address. In response to known health disparities, the LGBT Health Program of the Veterans Health Administration (VHA) developed policies supporting the provision of affirming care to LGBT veterans. To support policy implementation, the program launched the LGBT Veteran Care Coordinator (LGBT VCC) Program in 2016, requiring every VHA facility to appoint at least one clinical staff member to serve as an LGBT VCC. This quality improvement project reports on LGBT VCCs' pe

Perceived Community Support, Users' Interactions, and Value Co-Creation in Online Health Community: The Moderating Effect of Social Exclusion.

Online health communities (OHCs) face the same problem as other social media platforms in terms of decreasing activity and user attrition. Drawing upon organizational support theory, this study explores how perceived community support affects user interactions and value co-creation which in turn influence their continuous participation. OHCs act as both health knowledge-sharing platforms and important social media for patients, and thus, interpersonal interactions in OHCs are categorized into health-related and general topic interactions. Considering the identity of patients, this study also e

Computational Analysis Supports IL-17A as a Central Driver of Neutrophil Extracellular Trap-Mediated Injury in Liver Ischemia Reperfusion.

Hepatic ischemia reperfusion (I/R) is a clinically relevant model of acute sterile inflammation leading to a reverberating, self-sustaining inflammatory response with resultant necrosis. We hypothesized that computerized dynamic network analysis (DyNA) of 20 inflammatory mediators could help dissect the sequence of post-I/R mediator interactions that induce injury. Although the majority of measured inflammatory mediators become elevated in the first 24 h, we predicted that only a few would be secreted early in the process and serve as organizational centers of downstream intermediator complexi

Differential Representation of Articulatory Gestures and Phonemes in Precentral and Inferior Frontal Gyri.

Speech is a critical form of human communication and is central to our daily lives. Yet, despite decades of study, an understanding of the fundamental neural control of speech production remains incomplete. Current theories model speech production as a hierarchy from sentences and phrases down to words, syllables, speech sounds (phonemes), and the actions of vocal tract articulators used to produce speech sounds (articulatory gestures). Here, we investigate the cortical representation of articulatory gestures and phonemes in ventral precentral and inferior frontal gyri in men and women. Our re

Implementing a cornerstone of culture change: Consistent staff assignment in VHA community living centers.

In 2011, the Veterans Health Administration (VHA) began implementing consistent staff assignment in its nursing homes (called Community Living Centers or CLCs). Consistent assignment, a cornerstone of culture change, minimizes the number of staff who provide a resident's care. The present research assessed the level and stability of consistent assignment in units within VHA CLCs and identified unit characteristics related to implementation of this staff assignment model. Schedulers in 185 of 335 organizational units that make up VHA CLCs completed a Staffing Practices Survey. For the month pri

Effect of scapular dyskinesis on supraspinatus repair healing in a rat model.

BACKGROUND: Rotator cuff tears are common conditions that often require surgical repair to improve function and to relieve pain. Unfortunately, repair failure remains a common problem after rotator cuff repair surgery. Several factors may contribute to repair failure, including age, tear size, and time from injury. However, the mechanical mechanisms resulting in repair failure are not well understood, making clinical management difficult. Specifically, altered scapular motion (termed scapular dyskinesis) may be one important and modifiable factor contributing to the risk of repair failure. The

Cross-sector collaborations in Aboriginal and Torres Strait Islander childhood disability: a systematic integrative review and theory-based synthesis.

INTRODUCTION: Aboriginal and Torres Strait Islander children in Australia experience a higher prevalence of disability and socio-economic disadvantage than other Australian children. Early intervention is vital for improved health outcomes, but complex and fragmented service provision impedes access. There have been international and national policy shifts towards inter-sector collaborative responses to disability, but more needs to be known about how collaboration works in practice. METHODS: A systematic integrative literature review using a narrative synthesis of peer-reviewed and grey lite

Assessment of medical reserve corps volunteers' emergency response willingness using a threat- and efficacy-based model.

The goal of this study was to investigate the willingness of Medical Reserve Corps (MRC) volunteers to participate in public health emergency-related activities by assessing their attitudes and beliefs. MRC volunteers responded to an online survey organized around the Extended Parallel Process Model (EPPM). Respondents reported agreement with attitude/belief statements representing perceived threat, perceived efficacy, and personal/organizational preparedness in 4 scenarios: a weather-related disaster, a pandemic influenza emergency, a radiological ("dirty bomb") emergency, and an inhalational

Heterogeneity in aripiprazole diffusion for bipolar disorder treatment in the Veterans Health Administration.

OBJECTIVES: The objectives of this study were to ascertain the relative importance of scientific "approval" versus U.S. Food and Drug Administration (FDA) regulatory approval regarding changes in aripiprazole prescribing rates for treating bipolar disorder and to identify system-level covariates associated with faster regional uptake of aripiprazole. METHODS: Medication use data for 2002–2009 were obtained from the Veterans Health Administration (VHA) National Psychosis Registry for 106,547 patients with diagnoses of bipolar disorder, aggregated at the level of the Veterans Integrated Service

[The Global Fund to fight HIV/AIDS, TB and malaria policy issues].

The five-year evaluation of the Global Fund to fight HIV/AIDS, TB and malaria (GFATM) was carried out by a Consortium of several Universities and institutions, led by a consulting firm based in Washington DC. Evaluation focused on three study areas: (i) organizational efficiency and effectiveness of the Global Fund, (ii) effectiveness of the Global Fund partner environment, (iii) system effects of the Global Fund and impact of increased funding on the level of response to the three diseases. Findings can be summed up as follows: the Global Fund has been successful in mobilizing additional fund

Ethanol exposure during neurogenesis induces persistent effects on neural maturation: evidence from an ex vivo model of fetal cerebral cortical neuroepithelial progenitor maturation.

Ethanol is a significant neuroteratogen. We previously used fetal cortical-derived neurosphere cultures as an ex vivo model of the second trimester ventricular neuroepithelium, and showed that ethanol directly induced fetal stem and progenitor cell proliferation and maturation without inducing death. However, ethanol is defined as a teratogen because of its capacity to persistently disrupt neural maturation beyond a specific exposure period. We therefore utilized a simplified neuronal maturation paradigm to examine the immediate and persistent changes in neuronal migration following ethanol ex

The influence of protein-calorie malnutrition on quality of life in nursing homes.

BACKGROUND: Up to 85% of the older adults living in our nation's nursing homes suffer from protein-calorie malnutrition (PCM). Early identification and treatment of PCM can reduce or prevent hospital stays, reduce complications, and decrease mortality. We describe the influence of PCM on quality of life in nursing homes, using archived data from the Minimum Data Set. METHODS: The study was guided by the Quality Nutrition Outcomes-Long Term Care Model, which posits a pathway whereby organizational issues influence nutritional status, consisting of body mass index (BMI), serum albumin levels, a

Environmental and economic impact of validated endoscope storage systems: a life cycle assessment and scenario analysis of avoided morning reprocessing.

BACKGROUND AND AIMS: Validated storage systems allow prolonged microbiologically safe storage of thermosensitive endoscopes and may avoid repeated morning reprocessing in countries where an additional disinfection cycle is required before first use. We performed a life cycle assessment (LCA) comparing validated endoscope storage systems and modeled the environmental and economic consequences of avoiding repeated morning reprocessing. METHODS: An ISO 14040/44-compliant LCA compared three validated storage technologies: two drying and storage cabinets (steris endodry and wassenburg dry 300D) an

Booked model: radiological performance and the role of the General Practitioner in Friuli-Venezia Giulia.

INTRODUCTION: Waiting lists are a structural challenge for healthcare systems, affecting equity of access, diagnostic timeliness, and perceived quality of care. The Italian National Plan for the Governance of Waiting Lists (PNGLA) 2019-2021 promoted a shift from a performance model based on delivered services to one based on booked services, in order to better capture the gap between demand and supply. In this framework, the General Practitioner (GP) is involved not only in prescribing, but also in managing patient expectations. The aim of this study was to assess the impact of this transition

Racial Discrimination as a Traumatic Bedrock of Healthcare Avoidance: A Pathway Through Healthcare Institutional Betrayal and Mistrust.

Objectives: Experiences of racial discrimination within the healthcare system are potentially traumatic events (PTEs) that have been associated with lowered perceived trust in healthcare providers, ongoing symptoms of PTSD and depression, and anticipated healthcare avoidance. Based on the BITTEN trauma impact model, we test a pathway such that greater past healthcare discrimination would be associated with anticipated future healthcare avoidance among BIPOC college students. We posited that this direct relationship would be sequentially mediated by healthcare institutional betrayal (HIB) durin

Jail-to-community treatment continuum for adults with co-occurring substance use and mental disorders: study protocol for a pilot randomized controlled trial.

BACKGROUND: Adults with co-occurring mental and substance use disorders (CODs) are overrepresented in jails. In-custody barriers to treatment, including a lack of evidence-based treatment options and the often short periods of incarceration, and limited communication between jails and community-based treatment agencies that can hinder immediate enrollment into community care once released have contributed to a cycle of limited treatment engagement, unaddressed criminogenic risks, and (re)arrest among this vulnerable and high-risk population. This paper describes a study that will develop resea
