Donor, recipient and nerve grafts in brachial plexus reconstruction: anatomical and technical features for facilitating the exposure.

Forty three cadavers of adult and five patients were included in our study. Accessory, suprascapular, musculocutaneous and sural nerves were dissected. These widely used nerves in brachial plexus reconstruction have varying anatomy and still have no standard approach for surgery. Dissection of the accessory nerve in the upper part of the posterior neck triangle was quite complicated took a relatively long time and the nerve could easily be injured. It was found that these shortcomings could be diminished starting dissection of this nerve in the lower part of the posterior neck triangle near th

Exercise-related dissection of craniocervical arteries: CT, MR, and angiographic findings.

OBJECTIVE: Our goal was to demonstrate the spectrum of neuroradiologic (CT, MR, and angiographic) findings in craniocervical arterial dissection (CAD) related to exercise or sporting activities and compare the diagnostic utility of CT, MRI, and MR angiography (MRA). MATERIALS AND METHODS: The neuroradiologic examinations of 11 patients with CAD was performed: CT was performed in 10 patients, cranial MRI in 9, cranial and cervical MRA in 4, and contrast angiography in 10. The CT examinations were assessed for the presence of an infarction or a hyperdense artery (consistent with intraluminal th

Transoral closure of a perforation of the hypopharynx from blunt trauma.

Hypopharyngeal perforation secondary to blunt trauma is a rare injury. It can be managed operatively or expectantly without clear criteria f … ransorally using laparoscopic instruments and visualized by a rigid endoscope, followed by anterior neck dissection and drain placement. Postoperatively, he was kept nil per os and received intravenous antibiotics. He was discharged home and returned to the football field the same fall.

[Unusual location of a parathyroid adenoma: the carotid sheath].

We report the imaging features of an occult parathyroid adenoma with unusual location in the carotid sheath. Our patient presented with primary hyperparathyroidism. Following negative neck ultrasound and scintigraphy, exploratory neck dissection with partial thyroidectomy was performed twice over a 2 day period without biological response. Cervical and mediastinal CT and MRI were performed with no result. Digital angiography showed a tumoral blush supplied by the left inferior thyroid artery and located in close contact with the carotid artery. Venous sampling of the neck confirmed the left lo…

Superior mediastinal dissection for papillary thyroid carcinoma: approaches and outcomes.

BACKGROUND: Superior mediastinal surgery for thyroid carcinoma is not a standardized procedure like the neck dissection. The objective of this study was to evaluate the effectiveness of superior mediastinal dissection for mediastinal metastasis of papillary thyroid carcinoma (PTC). METHODS: We conducted a retrospective review of 119 patients who underwent superior mediastinal dissection for the treatment of PTC. The postoperative characteristics and follow-up data were analyzed. Cox regression was performed to identify the factors related to the mediastinal control. RESULTS: No severe compli…

Stratified lymph node yield thresholds after neoadjuvant immunochemotherapy: a surgical benchmark for survival in oral squamous cell carcinoma.

BACKGROUND: The adequacy of neck dissection following neoadjuvant immunotherapy combined with chemotherapy (NICT) for locally advanced oral squamous cell carcinoma (OSCC) lacks evidence-based benchmarks. This study aimed to define and validate extent-specific lymph node dissection (LND) adequacy thresholds associated with survival outcomes. METHODS: In a development and validation study, the training cohort comprised 256 consecutive patients with locally advanced OSCC treated with NICT and surgery at Henan Cancer Hospital (2019-2022). An independent cohort of 199 patients from the First Affil…

Spontaneous carotid dissection presenting lower cranial nerve palsies.

Cranial nerve palsy in internal carotid artery (ICA) dissection occurs in 3--12% of all patients, but in 3% of these a syndrome of hemicranias and ipsilateral cranial nerve palsy is the sole manifestation of ICA dissection, and in 0.5% of cases there is only cranial nerve palsy without headache. We present two cases of lower cranial nerve palsy. The first patient, a 49-year-old woman, developed left eleventh and twelfth cranial nerve palsies and ipsilateral neck pain. The angio-RM showed an ICA dissection with stenosis of 50%, beginning about 2 cm before the carotid channel. The patient was tr

Spontaneous Internal Carotid Dissection Presenting With Pulsatile Tinnitus and Hearing Loss.

Internal carotid artery (ICA) dissection often presents with headache and neck pain, along with symptoms related to the anterior circulation if stroke occurs. Less commonly, it may cause Horner's syndrome and involvement of the lower cranial nerves (IX, X, XI, and XII). However, it is rare for ICA dissection to present with pulsatile tinnitus and hearing loss, which is typically linked to vertebral artery dissection. We report a rare case of a man in his 50s who initially presented to the Otolaryngology Department with a brief episode of high-pitched pulsatile tinnitus and hearing loss, preced

Hyo neck lift: Preliminary report.

BACKGROUND: The aim of neck lift is to recreate an acute cervicomandibular angle. Surgical neck rejuvenation typically is associated with posterosuperior traction on the platysma (PLA) with an anterior vertical PLA corset or a digastric corset. Medium-term instability can exist with these procedures. OBJECTIVES: A novel platysmaplasty technique called the hyo neck lift is described and its surgical efficacy is compared with those of previously described techniques. METHODS: Ten patients underwent hyo neck lift and were evaluated in a preliminary prospective study. Hyo neck lift involved hori

[Operative technique aiming at en bloc dissection in esophageal surgery].

The lymph node metastases of esophageal cancer occur over a wide area. It is essential for radical surgery of such metastases to aim at en bloc dissection. Otherwise, it can easily become a combination of blunt esophagectomy and lymph node sampling through a right thoracotomy. In the intrathoracic procedure, all the nodes to be dissected can be harvested while attached to the esophagus together with the surrounding connective tissue, except for the pretracheal nodes in front of the cardiac branches of the right vagus nerve and the subaortic arch nodes. It is important to dissect the left parat

Standard Diffusion-Weighted Imaging in the Brain Can Detect Cervical Internal Carotid Artery Dissections.

BACKGROUND AND PURPOSE: The ICA is the most common site of cervical artery dissection. Prompt and reliable identification of the mural hematoma is warranted when a dissection is clinically suspected. The purpose of this study was to assess to capacity of a standard DWI sequence acquired routinely on the brain to detect dissecting hematoma related to cervical ICA dissections. MATERIALS AND METHODS: This was a retrospective study of a cohort of 110 patients younger than 55 years of age (40 women; mean age, 46.79 years) admitted at the acute phase of a neurologic deficit, headache, or neck pain

Bilateral Vertebral Artery Dissection: A Case Report with Literature Review.

Vertebral artery dissection (VAD) is a rare cause of ischemic stroke in young patients. The largely nonspecific symptoms and delayed presentation pose a serious diagnostic challenge. Medical management with either anticoagulation or antiplatelet therapy is recommended, but there are no reports of successful dual therapy. We report a case of spontaneous bilateral vertebral artery dissections (VADs) treated with both anticoagulation and antiplatelet therapy and a literature review on clinical presentation and the current medical and surgical management options. A 37-year-old healthy female prese

[A case of spontaneous dissection of the cervical carotid artery with pseudoaneurysm].

We experienced the case of a dissecting carotid artery presenting with a pulsatile neck mass. This 60-year-old man was admitted to our department due to a progressive, painful left neck mass. The lesion was diagnosed as a dissecting left internal carotid artery with the subsequent formation of a giant pseudoaneurysm. First, the patient underwent an endovascular treatment using self-expandable stent and Guglielmi detachable coils (GDCs). However, four months after the treatment, recurrence of the pseudoaneurysm happened to him, and he was readmitted to our department. Angiographies revealed an
