Endobronchial mucoepidermoid carcinoma in a child with postinfectious bronchiolitis obliterans.

Endobronchial mucoepidermoid tumors are rare neoplasms. Due to nonspecific symptoms, diagnosis can be challenging, but early diagnosis and treatment are crucial for prognosis. We present the case of a boy, with chronic respiratory insufficiency due to bronchiolitis obliterans, that presented worsening exertional dyspnea at 12 years. Spirometry showed unexpected deterioration of respiratory function and a computed tomography scan revealed an obstructive polypoid mass in the intermediate bronchus. Given the severe basal ventilatory compromise and risk associated with surgical treatment, rigid br

CCL27 Signaling in the Tumor Microenvironment.

Chemokines are a group of small proteins which play an important role in leukocyte migration and invasion. They are also involved in the cellular proliferation and migration of tumor cells.Chemokine CCL27 (cutaneous T cell-attracting chemokine, CTACK) is mainly expressed by keratinocytes of the normal epidermis. It is well known that this chemokine plays an important role in several inflammatory diseases of the skin, such as atopic dermatitis, contact dermatitis, and psoriasis. Moreover, several studies have shown an association between CCL27 expression and a variety of neoplasms including ski

Novel approach using transoral robotic surgery for resection of cervical spine chordoma.

Chordomas are rare, infiltrative neoplasms of notochordal origin that present along the spinal canal; en bloc surgical resection is paramount to successful treatment. Limited visualization and complex anatomy are major challenges to resection of upper cervical spine chordomas and often require invasive surgery. A 27-year-old male presented with an incidentally discovered chordoma of the midline second cervical vertebra of the spine. To obtain en bloc resection of the lesion while both overcoming limitations due to access and without introducing morbidity from traditional anterior approaches, w

Amyloid Pseudo-Dacryolith and Nasolacrimal Obstruction in a 67-Year-Old Male.

A 67-year-old male with a 35-year history of left-sided epiphora presented with a nonpainful, noninflamed, left medial canthal mass and complete left nasolacrimal obstruction. During routine dacryocystorhinostomy, a lesion was present within the lacrimal sac that mimicked a lacrimal stone in appearance but with a consistency concerning for malignancy. Histologically, the lesion displayed apple-green birefringence on polarized light microscopy and Congo red staining. The patient was referred to the hematology service for evaluation, which failed to reveal systemic disease. There is 1 previous r

Adenoid cystic carcinoma of the sublingual gland: A case report.

Adenoid cystic carcinoma (ACC) of the sublingual gland is an extremely rare neoplasm. The clinicopathological characteristics of ACC are slow-growing swelling with or without ulceration, perineural spread, local recurrence, and distant metastasis. This report describes a 58-year-old male who had a slowly growing swelling without ulceration on the right side of the mouth floor that had been present for 1 month. In a radiological examination, the mass showed multilocular cystic features and no bony or tongue muscle invasion. No enlarged cervical lymph nodes were detected. Excisional biopsy and h

Extrathoracic recurrence of type A thymoma.

INTRODUCTION: Neoplasms of the thymus gland are the most common tumors in the anterior mediastinum, and differentiation between benign and malignant forms is rather difficult. Extrathoracic invasions or metastases are extremely rare, and only a few cases have been reported previously. CASE PRESENTATION: We report herein the case of a patient in whom a thymoma type A recurred at skeletal muscle 2-years after surgery. The metastases were completely resected. Adjuvant treatment was started. Actually, no signs of recurrence were seen. CONCLUSION: Our case shows that also thymoma type A, generall

Adenoid cystic carcinoma of the mobile tongue: A rare case.

Adenoid cystic carcinoma (ACC) occurs more commonly in the minor salivary glands of the palate on than the tongue. ACC is a malignant neoplasm that accounts for 1-2% of all head and neck malignancies and 10-15% of all salivary gland malignancies. ACC affects the exocrine glands at any site, but the parotid gland is the most common site in the head and neck region. Many factors should be taken into account in the prognosis of ACC, including the histological and clinical stages of the disease. The most striking feature of ACC is that it is locally aggressive, with a high recurrence level, perine

Cutaneous mast cell tumor with epitheliotropism in 3 dogs.

Epitheliotropism is an important diagnostic feature of cutaneous epitheliotropic lymphoma and canine cutaneous histiocytoma; however, although noted in certain feline mastocytic diseases, it has not been considered a feature of canine cutaneous mast cell tumor. In this study, 3 canine cutaneous mast cell tumors had epitheliotropic invasion of neoplastic mast cells into the epidermis and follicular epithelium. This unusual histologic finding was characterized by infiltrates of individual and clusters of neoplastic mast cells in the stratum basale and stratum spinosum. The mast cell origin of th

Odontogenic myxoma presenting as localized inflammatory gingival enlargement: A diagnostic dilemma.

Odontogenic myxoma (OM) is a rare and locally invasive benign neoplasm found exclusively in the maxillofacial region. The radiographic and clinical features are variable, and the diagnosis is therefore not easy. A case of OM of the maxilla is described in a 19-year-old female, previously diagnosed as inflammatory gingival enlargement. Clinical, histological, radiographic, and computed tomographic (CT) scan assessments were done, which were confirmatory for OM. Surgical excision of the lesion was done. The patient was put on periodic recall, as the recurrence rate of the lesion was high. The bi

Recurrent renal leiomyosarcoma mimicking a colonic submucosal tumor: a case report.

A primary leiomyosarcoma of the kidney is a rare, but highly aggressive, neoplasm, accounting for only 0.1% of all invasive renal tumors. Local or systemic recurrence is common, but a leiomyosarcoma is difficult to diagnose preoperatively. We recently encountered an interesting case of an unusual recurrence of a renal leiomyosarcoma. A 57-year-old woman visited our hospital complaining of lower abdominal pain. Four years previously, she had undergone a left nephrectomy. She had a primary leiomyosarcoma of the kidney that had been misdiagnosed as a renal cell carcinoma. Colonoscopy revealed the

[Biliary-duodenal fistula following radiofrequency ablation therapy for hepatocellular carcinoma].

Hepatocellular carcinoma (HCC) is one of the most common malignant neoplasms occurring worldwide. Surgical resection currently provides the best chance of long-term tumor free survival, but the most HCCs are not candidates for surgical excision due to poor liver function or poor medical background. Numerous noninvasive alternatives to surgical resection have been introduced to treat liver cancers. Radiofrequency thermal ablation has begun to receive much attention as an effective and minimally invasive technique for the local control of HCC. The biliary system related complications after radio

Navigated dorsolateral suboccipital transcondylar (NADOSTA) approach for treatment of hypoglossal schwannoma. Case report and review of the literature.

Hypoglossal schwannoma is a rare skull base neoplasm, which lies in the midst of extremely complex anatomical structures. We report a 39-year-old man who presented with a history of right glossal hemiatrophy, dysesthesia and weakness of the left extremities. These symptoms were caused by a unilateral hypoglossal schwannoma located at the cranial base. This lesion was exposed through a dorsolateral suboccipital transcondylar approach, which was navigated with the frameless stereotaxy (NADOSTA). We also describe the epidemiology, symptomatology and the diagnosis of hypoglossal schwannoma as well

Paraganglioma of the bladder: report of a case.

Paragangliomas are uncommon tumours that comprise less than 0.1% of all primary bladder neoplasms. Approximately half of the patients present the clinical triad of hypertension, haematuria and attacks associated with micturition. About 10% of vesical paragangliomas behave malignantly. This may be manifested as local invasion or metastatic spread, particularly to lymph nodes and lung. The best therapy remains total excision and partial cystectomy is considered adequate treatment for most cases. We report a case of paraganglioma of the bladder with extramural appearance in a 39-year-old man. Cli

Multiple colorectal neoplasms in a young adult with hypogammaglobulinemia. Report of a case.

We report on a 22-year-old man with congenital hypogammaglobulinemia who developed multiple colorectal neoplasms. An immunodeficiency had been diagnosed in the patient since two years of age, and this time many tumors of the sigmoid colon and rectum were detected by barium enema and fiberscopy. Abdominoperineal resection was performed, and the resected specimen revealed 29 polyps, including 9 adenocarcinomas and 20 adenomas. The carcinomas, measuring 0.8 to 11.0 cm in size, showed various depths of invasion, and the adenomas, measuring 0.2 to 1.5 cm in size, showed various degrees of epithelia

Thymoma--report of 71 cases and a review.

Of the 71 cases of thymoma reviewed, 31 were benign and 40 were malignant. Associated syndromes were common--myasthenia gravis 51% (36/71), red cell aplasia 3%, hypogammaglobulinemia 4%, mucocutaneous candidiasis 1%, and a number of other autoimmune diseases and neoplasms. These were almost equally distributed among benign and malignant tumors. A plain radiograph showing diffuse involvement of the mediastinum always indicates invasive (malignant) thymoma, while a localized pattern usually suggests a benign lesion (25 of 39 cases). Computerized tomography or MRI often gives additional informati

Neurotropic malignant melanoma of right temple with orbital metastasis: a clinicopathological case report.

A case is reported of neurotropic melanoma developed from a superficial spreading melanoma with minimal cytological deviation, situated in the right temple. The nine-year course was clinically characterised by local recurrences, involvement of the orbit and the parotid region via neurogenic invasion, and systemic metastases to lung, seventh rib, and the brain. The histopathology was characterised by fascicles of dysplastic spindle cells, neuroid arrangement in loose fibrillary matrix, and peri- and intraneural permeation of the nerve trunks. Despite minimal atypism the neoplasm metastasised an

A practical critique of antifungal treatment guidelines for haemato-oncologists.

The management of invasive fungal disease (IFD) in the haemato-oncology setting remains a challenge. This article reviews recent guidelines relating to IFD for their similarities and differences, as well as applying the Appraisal of Guidelines Research and Evaluation (AGREE) criteria. The guidelines' recommendations on antifungal prophylaxis, empirical and definitive treatment of candidiasis and aspergillosis are summarized; also, minimum standards for diagnosis and follow-up are discussed. This critique of the reviewed guidelines is a practical guide to physicians and commissioners in making

Comparing oncological outcomes and safety between photodynamic diagnosis-assisted and white-light transurethral resection in elderly patients with non-muscle invasive bladder cancer.

OBJECTIVES: This study aimed to assess the prognostic outcomes and risk of adverse events in elderly non-muscle invasive bladder cancer (NMIBC) patients receiving photodynamic diagnosis-assisted transurethral resection of bladder cancer (PDD-TURBT). METHODS: This study retrospectively included 326 patients who were over 70 years old and received either PDD-TURBT (n = 114, PDD group) or white-light TURBT (n = 212, WL group). Oncological outcomes, namely recurrence-free survival (RFS) and progression-free survival (PFS), and adverse event profiles were compared between the two groups. RESULTS:

[Efficacy and Adverse Events of Low-Dose Intravesical BCG Therapy for Non-Muscle-Invasive Bladder Cancer].

We examined the efficacy and adverse effects of low-dose intravesical Bacillus Calmette-Guérin (BCG) therapy in patients with non-muscle-invasive bladder cancer. Patients who underwent intravesical BCG therapy (n=176 ; 198 courses) at our hospital between April 2012 and December 2022 were enrolled. After assigning patients to either the low-dose or regular-dose (40 or 80 mg of BCG Tokyo 172 strain) groups, treatment efficacy and incidence of adverse events were compared. The mean number of BCG infusions in the low-dose and regular-dose groups was 6. 8 and 7. 8,respectively,although the differe

Invasive Front and Peritumoral Prognostic Factors in Colorectal Cancer: Current Perspectives and Future Considerations.

The invasive front and peritumoral region of colorectal cancers are replete with powerful prognostic biomarkers that can predict oncologic outcomes with far greater precision than achievable by tumor, node, metastasis (TNM) stage alone. This article considers both established and emerging invasive front/peritumoral prognostic factors with an emphasis on recent developments, practical considerations, diagnostic challenges, and the potential role of automated assessment in the integration of emerging prognostic factors into future clinical practice.
