OBJECTIVE: Idiopathic spinal cord herniation (ISCH) is a rare cause of progressive thoracic myelopathy, for which surgical management is wid … associated with a higher probability of stable/worsened sensory outcome (p = 0.015). Intraoperative neurophysiological monitoring showed significant associations in both motor (p = 0.020) and sensory (p = 0.007).
CONCLUSIONS: Surgical treatment of ISCH resulted in neurological improvement in a substantial proportion of patients. This IPD analysis identified preoperative neuro …

Surgical correction of spinal deformities in children presents a challenge to the anaesthetist due to the extensive nature of the surgery, the co-morbidities of the patients and the constraints on aesthetic techniques of intraoperative neurophysiological monitoring of the spinal cord. Patients undergoing scoliosis surgery are considered to suffer severe pain, which may lead to a negative impact on patient psychology and physical well-being. By using effective postoperative pain regimens to enhance recovery after surgery, pain can be significantly reduced, leading to patient satisfaction, facil…

BACKGROUND: Influenza-associated encephalopathy and acute necrotizing encephalopathy (ANE) are rare but devastating complications of pediatric influenza, and early differentiation from severe pneumonia without neurological involvement is challenging at admission. We aimed to develop and internally validate an early prediction model to distinguish severe influenza A pneumonia from influenza-associated central nervous system (CNS) involvement using routine admission laboratory tests. The intended clinical use is early triage: to prioritize neurological evaluation (e.g., EEG/neuroimaging when ind

BACKGROUND: In the past four to five decades, the field of swallowing science has made significant strides in the evaluation and treatment of swallowing disorders (dysphagia). Despite these strides, several gaps in knowledge remain and optimal approaches for dysphagia management have yet to be established. Part of this hindrance stems from our relatively limited understanding of the complex underlying swallowing mechanisms which further limits our ability to examine how these mechanisms may be altered in patients with dysphagia and how to optimally target them in therapy. To overcome this hind

HIV-1 infection of brain may be associated with multiple treatment targets, only the most severe of which is represented by HAD. Focusing on earlier treatment targets such as MCMD and cognitive-motor impairment in the absence of any clinical disorder (as well as neuroprotection) may prove to be of greater clinical utility in the treatment and prevention of such impairment than a focus on later-stage cognitive-motor disease, when neuronal cell death is already extensive. This may be especially important now that improvements using the protease inhibitors in triple-drug combination regimens have

Survival with a good quality of life after cardiac arrest continues to be abysmal. Coordinated resuscitative care does not end with the effective return of spontaneous circulation (ROSC)-in fact, quite the contrary is true. Along with identifying and appropriately treating the precipitating cause, various components of the post-cardiac arrest syndrome also require diligent observation and management, including post-cardiac arrest neurologic injury and myocardial dysfunction, systemic ischemia-reperfusion phenomenon with potential consequent multiorgan failure, and the various sequelae of criti

BACKGROUND: Early-life stress (ELS) is a key risk factor for adolescent depression. Si-Ni-San (SNS), a classic traditional Chinese medicine formula, has shown antidepressant potential, yet its effects on the dorsal raphe nucleus (DRN)-nucleus accumbens (NAc) serotonergic circuit remain unclear.
PURPOSE: This study aimed to investigate whether SNS alleviates adolescent depression by restoring DRN-NAc serotonergic circuit function and to identify the serotonin receptor mediating its synaptic effects in the NAc.
STUDY DESIGN: Firstly, the antidepressant efficacy of SNS was evaluated in a mouse

BACKGROUND: Stroke is a leading cause of long-term disability, often resulting in cognitive and upper limb motor impairments that significantly impact activities of daily living. While conventional rehabilitation approaches have shown effectiveness, challenges such as high cost, limited accessibility, and poor adherence hinder their widespread implementation. Calligraphy training has emerged as a promising rehabilitation strategy, offering a multimodal intervention that integrates motor and cognitive processes. However, the underlying neural mechanisms and the combined effects of calligraphy t

BACKGROUND: Parkinson's disease (PD) is a movement disorder caused by dopaminergic neurodegeneration. Both Levodopa (L-dopa) and Subthalamic Deep Brain Stimulation (STN-DBS) effectively alleviate symptoms, yet their cerebral effects remain under-explored. Understanding these effects is essential for optimizing treatment strategies and assessing disease severity. Magnetoencephalogram (MEG) data provide a continuous time series signal that reflects the dynamic changes in brain activity. The hidden Markov model (HMM) can capture and model the temporal features and underlying states of the MEG sig

Approximately 3.4 billion people are at risk of malaria, a disease caused by infection with Plasmodium spp. parasites, which are transmitted by Anopheles mosquitoes. Individuals with severe falciparum malaria often exhibit changes in circulating blood levels of biogenic amines, including reduced serotonin or 5-hydroxytryptamine (5-HT), and these changes are associated with disease pathology. In insects, 5-HT functions as an important neurotransmitter for many behaviors and biological functions. In Anopheles stephensi, we show that 5-HT is localized to innervation in the head, thorax, and midgu

Focused research on pain and pain control in children has developed primarily in the last 10 years and even now is woefully inadequate in relation to the magnitude of the problem. The available research, inferences from the adult literature, and anecdotal information all indicate the elusive nature of pain. Pain is not solely a fixed neurophysiologic response to a noxious stimulus but a product of the interaction of many variables such as age, cognitive set, personality, ethnic background, and emotional state. These factors exert a tremendous influence on the suffering which surrounds the pain

In the era of information overload, understanding the brain's adaptive responses to prolonged cognitive tasks is critical. This study investigates the neural compensatory mechanisms that sustain cognitive performance under mental fatigue, offering insights into dynamic resource allocation and practical applications in high-demand settings. Twenty healthy participants performed a Stroop-based cognitive interference task while prefrontal hemodynamic activity was monitored using functional near-infrared spectroscopy (fNIRS). Subjective fatigue was assessed via the Multidimensional Fatigue Invento
