[Treatment of congenital hip dislocation using Pavlik's harness. Long term results].

PURPOSE OF THE STUDY: After many years using Pavlik harness for treatment of congenital hips dislocation, it is important to evaluate long-term results, failure causes and factors responsible for avascular necrosis. MATERIAL: One hundred and forty one children were treated by this mean (159 dislocated hips, 30 acetabular dysplasia). The follow up was 74 months (12-175). METHODS: In this study, the severity of the dislocation was evaluated by antero-posterior radiographs of the pelvis. The importance of proximal dislocation was measured by the distance from the top of the proximal femoral met

Lumbar support thickness: effect on seated buttock pressure in individuals with and without spinal cord injury.

We compared the effects of different lumbar support thicknesses on seated buttock pressure in individuals with and without spinal cord injury (SCI). Eighteen subjects with SCI (SCI group) and 18 subjects without SCI (control group) were seated on a pressure-sensing transducer incorporated into an adjustable chair. The output was calibrated so that eight pressure intervals (colors) were displayed. The lumbar support thickness was adjusted to 0, 2.5, 5, and 7.5 cm, and the highest and lowest seated buttock pressures were analyzed. External measurements of the hip angle were taken for each lumbar

Diseases of the developing hip joint.

The best treatment for diseases of the hip can begin if one diagnoses the disorder as soon as possible after onset. In many cases, successful management of the disease will prevent permanent damage to the hip joint. If injury occurs to the developing hip joint, major hip surgery may be necessary in adult life.

Legg-Calvé-Perthes disease: treatment and prognosis.

Our experience in the treatment of Legg-Perthes disease using non-containment, containment, and surgical treatment supports the concept that containment is a key factor in the treatment of Legg-Perthes disease. Patients without excessive lateral extrusion or other signs of "head at risk" and half or more of the femoral head involved probably can be treated for short periods of time in an abduction orthosis. If less than half the femoral head is involved but lateral extrusion is minimal, a period of traction to obtain range of motion and decrease the synovitis may be sufficient treatment. In th

Comparing Patient and Provider Priorities Around Amputation Level Outcomes Using Multiple Criteria Decision Analysis.

BACKGROUND: Patients with chronic limb threatening ischemia may require a transmetatarsal amputation (TMA) or a transtibial amputation. When making an amputation-level decision, these patients face a tradeoff-a TMA preserves more limb and may provide better mobility but has a lower probability of primary wound healing and may therefore result in additional same or higher level amputation surgeries with an associated negative impact on function. Understanding differences in how patients and providers prioritize these tradeoffs and other outcomes may enhance shared decision-making. OBJECTIVES:

Effectiveness of high intensity laser therapy on pain, grip strength and median nerve conductivity in pregnant women with carpal tunnel syndrome: A randomized controlled trial.

STUDY DESIGN: Single-blind randomized controlled trial. INTRODUCTION: High-intensity laser therapy (HILT) was recently introduced to the physiotherapy field as a treatment option for patients with carpal tunnel syndrome (CTS). However, evidence about its effectiveness on pregnant women with CTS is sparse. PURPOSE OF STUDY: To test the effectiveness of HILT combined with a conventional physical therapy program to reduce pain intensity, sensory and motor nerve distal latency, and increase grip strength in pregnant women with CTS compared with the physical therapy alone. METHODS: Fifty-four pr

Chronic ankle instability. Which tests to assess the lesions? Which therapeutic options?

This paper purpose is to suggest an in-depth approach to diagnose the causes and lesions associated with and consecutive to chronic ankle instability due to ankle collateral ligament laxity. The different therapeutic and medicosurgical options adapted to this diagnostic approach are identified. The diagnostic aim is to precisely locate the ligamentous injuries of the tibiofibular, subtalar, talar and calcanean system, to identify the predisposing factors such as the hindfoot morphology, and any lesions associated with chronicity: anterolateral impingement, fibular injury, osteochondral lesions

Early treatment of coronal synostosis with endoscopy-assisted craniectomy and postoperative cranial orthosis therapy: 16-year experience.

OBJECT: The objective of this study was to present the authors' 16-year experience treating coronal craniosynostosis in infants using endoscopy-assisted techniques and postoperative cranial orthoses. METHODS: A total of 128 synostosed coronal sutures in 115 patients were treated between 1996 and 2012 by endoscopically resecting a strip of bone containing the stenosed suture via a 2-3 cm incision made at the ipsilateral stephanion. Data were obtained from a prospective database. Following surgery, patients were fitted with custom cranial orthoses to help correct preoperative craniofacial defor

Does the Powers™ strap influence the lower limb biomechanics during running?

Previous research has reported a prevalence of running related injuries in 25.9% to 72% of all runners. A greater hip internal rotation and adduction during the stance phase in running has been associated with many running related injuries, such as patellofemoral pain. Researchers in the USA designed a treatment device 'the Powers™ strap' to facilitate an external rotation of the femur and to thereby control abnormal hip and knee motion during leisure and sport activities. However, to date no literature exists to demonstrate whether the Powers™ strap is able to reduce hip internal rotation dur

Immediate weight bearing following modified lapidus arthrodesis.

First metatarsocuneiform arthrodesis (Lapidus procedure) can provide powerful correction of mild to moderate hypermobile or severe hallux valgus, although a period of non-weight bearing may be necessary. The purpose of this retrospective investigation was to compare patients who underwent a modified Lapidus arthrodesis with 2 screws plus an additional "neutralization" Kirschner-wire with immediate partial weight-bearing in a removable boot, with a control group without the Kirschner-wire who were non-weight bearing for 6 weeks in a short leg cast. A total of 41 consecutive patients from Januar

Posterior tibial tendon transfer: results of fixation to the dorsiflexors proximal to the ankle joint.

BACKGROUND: Most techniques for posterior tibial tendon transfer attach the posterior tibial tendon somewhere at the dorsum of the foot. Inadequate tendon length and difficulties in securing the tendon to bone or tendon can complicate these procedures. Performing the tenodesis proximal to the ankle joint can prevent these problems. The present study is the first to describe the results of posterior tibial tendon transfer through the interosseous membrane attaching only the posterior tibial tendon to the extensor tendons proximal to the ankle joint. METHODS: We examined 13 feet in 12 patients

Conservative treatment of fractures of the thoracolumbar spine.

We reviewed 48 patients with thoracolumbar fractures treated conservatively between 1988 and 1999. The average follow-up was 77.5 (31-137) months and average patient age (23 women, 25 men) was 46 (18-76) years. Twenty-nine patients suffered a fall from a height and 13 patients were injured in traffic accidents. Thirty-two patients had compression-type fractures and 16 burst-type fractures. There were no neurological deficits. Twenty-nine patients were treated by orthosis, 13 by body cast and six by bed rest. In addition to pain and functional scoring, we measured a number of radiographic param

[Pelvic injuries in the polytraumatized patient].

Pelvic injuries represent a thorny and stubborn therapeutic challenge. Because major forces are required to fracture the pelvis, pelvic ring disruption, more than any other fracture, can lead to life-threatening associated injuries such as massive bleeding, organ injuries, and open fractures including hemipelvectomy. The rapid diagnosis and effective treatment ("damage control") of those injuries play the key role in the patient's survival, inasmuch as the mortality of multiply injured patients with pelvic ring disruption remains high with 20-35%. Exsanguinating hemorrhage represents the most

Clinical examination and magnetic resonance imaging in the assessment of ankle sprains treated with an orthosis.

This is a prospective clinical study of treatment of ankle sprains with an ankle brace that permits ankle dorsiflexion and plantarflexion of 20 degrees, but limits inversion and eversion for 6 weeks. The ankle brace is followed by physiotherapy for another 6 weeks. Thirty patients were evaluated with clinical examination and magnetic resonance (MR) imaging before treatment and after 12 weeks of treatment. MR imaging revealed acute tears in the anterior talofibular ligament in all 30 ankles (100%) and tears in the calcaneofibular ligament in 25 of 30 ankles (83%). At 12 weeks after injury, MR e

Long-term evaluation of adolescents treated operatively for spondylolisthesis. A comparison of in situ arthrodesis only with in situ arthrodesis and reduction followed by immobilization in a cast.

The medical records and radiographs of forty-two adolescents (twenty-three male and nineteen female) who had had a posterolateral spinal arthrodesis for spondylolisthesis between 1950 and 1986 were reviewed to assess the long-term outcome of this form of treatment. The average age of the patients at the time of the operation was fourteen years (range, seven years and nine months to seventeen years and eleven months). The duration of the clinical and radiographic follow-up ranged from two years to twenty-seven years and seven months. All patients had an in situ arthrodesis of the involved verte
