The penis is an organ of enormous importance and is vital for both excretory and reproductive function. The anatomy of the penis itself can lead to many of its emergent conditions and a thorough understanding of the anatomy and physiology is central to recognition of these conditions. Physicians should have a high clinical suspicion for penile emergencies and perform a thorough physical examination to make a proper diagnosis. Prompt diagnosis and proper management are essential for minimizing dysfunction of this vital organ.

BACKGROUND: A 45-year-old man presented with repeated awakenings at night caused by nocturnal painful erections, in sharp contrast with normal erections at other times, 2 years after surgical removal of a thoracic (Th6-7) ependymoma.
INVESTIGATIONS: Physical examination, pharmaco-sensitized penile Doppler ultrasound, spinal and brain MRI, neurophysiological work-up (tibial and perineal somatosensory-evoked potential; hand, foot and perineal sympathetic skin response assessment), polysomnography with recording of nocturnal penile tumescence.
DIAGNOSIS: Sleep-related painful erections, charact

We analyzed the incidence of prolonged erection (PE) in 495 impotent subjects following intracavernous papaverine chlorhydrate injection to evaluate sexual dysfunction. Patient course was individually assessed. The overall the incidence of PE was 5.05%. The etiology of sexual dysfunction was psychogenic in all cases. No evidence of dose- or age-related PE was observed. Fibrosis of the corpus cavernosum, although more prevalent in PEs greater than 6 hours, was also observed in PEs of a shorter duration. This suggests that a probable predisposing factor might previously be present in some patien

Cavernous body radiography is of great importance in the diagnosis of penile disorders. An exact localisation of structural changes of the cavernous bodies is possible and also identification of venous leaks causing erectile dysfunction.

Penile pathology encompasses a wide spectrum of conditions. Among the most common lesions are vascular abnormalities (Mondor's disease, thrombosis of the corpus cavernosum, erectile dysfunction and priapism), traumatic injuries (penile fracture, intracavernous and extraalbugineal haematomas), inflammatory and infectious processes (abscess and gangrenous cavernositis), benign lesions (Peyronie's disease and others) and malignant lesions (both primary and metastatic). A radiologist's knowledge of these conditions is fundamental for accurate diagnosis and appropriate treatment, with a significant…

A cavernosal abscess is a rare condition. It can be idiopathic or with an underlying cause such as intracavernosal injection therapy, foreign bodies, perineal abscesses extension, priapism or trauma. The most common presenting symptoms were penile pain and swelling. Standard treatment consists of drainage via an incision, followed by broad-spectrum antibiotics where some authors describe less invasive techniques such as image-guided aspiration. We present a case of spontaneous left cavernosal abscess diagnosed on MRI in a 60-year-old patient with uncontrolled diabetes mellitus that was treated…

Our patient suffered a perineal straddle injury, resulting in right cavernosal artery pseudoaneurysm in combination with a cavernosal-urethral fistula. The urethra failed to heal after several weeks, and the patient presented with severe intermittent urethral bleeding. The pseudoaneurysm was successfully treated by coil embolization, with resolution of the bleeding. The patient recovered completely, with normal erectile and voiding function. This type of injury is very rare in the literature: traumatic cavernosal arterial pseudoaneurysm is known to cause high flow priapism, but in this case ad…

Intracavernosal phenylephrine injection is commonly used during penile plication for Peyronie's disease due to its desirable vasoconstrictive effect in achieving detumescence. Although systemic absorption of medications following intracavernosal administration may occur, high-dose intracavernosal phenylephrine has been reported to be safe and effective in treating priapism with a rare incidence of morbidity or significant haemodynamic changes. We describe a 68-year-old man who underwent ambulatory penile plication with intra-operative intracavernosal phenylephrine injection, which led to hyper…

Catastrophic antiphospholipid syndrome (CAPS) is a rare disorder characterized by acute, multi-system dysfunc- tion due to small-vessel thrombosis related to anti-phospholipid antibodies. Here we present an unusual case of CAPS presenting with genitourinary manifestations. A 72-year-old male developed a series of symptoms over the course of two weeks. His symptoms included testicular inflammation, scrotal edema, priapism, hematuria, penile eschar, elbow eschars, and acute kidney injury. He was found to have anti-phospholipid antibodies and treated with anticoagulation, high-dose steroids and p…
