Pre-existing fractures and bone mass predict vertebral fracture incidence in women.

OBJECTIVE: To determine the independent contributions of bone mass and existing fractures as predictors of the risk for new vertebral fractu … s. Stature, body mass index, arm span, and spinal conditions such as scoliosis, osteoarthritis, and sacroiliitis did not predict fracture incidence (P greater than 0.05). Weight was marginally predictive (P = 0.04) of fracture incidence but became nonpredictive after adjusting for bone mass (P greater than or equal to 0.05). CONCLUSIONS: Both bone mass and pr …

Age- and Sex-dependent Frequency of Fat Metaplasia and Other Structural Changes of the Sacroiliac Joints in Patients without Axial Spondyloarthritis: A Retrospective, Cross-sectional MRI Study.

OBJECTIVE: To determine the prevalence of fat metaplasia and other structural lesions of the sacroiliac joints associated with axial spondyloarthritis in a nonrheumatological patient population. METHODS: Magnetic resonance imaging examinations that included the pelvis and were performed in patients without known rheumatological disease were used for this retrospective cross-sectional study. These images were evaluated for sacroiliac fat metaplasia, sclerosis, osteophytes, and joint space alterations such as erosions or ankylosis. Patients were divided into 7 age groups (15-24 to ≥ 75 yrs). Pr

The paper deals with the estimation of the capabilities and role of different radiation techniques in the detection of late postradiation therapy local tissue responses that are evident as radionecrosis. To specify the nature of changes, it is necessary to take into account clinicoanamnestic data, results of laboratory and instrumental studies (standard X-ray study, multislice computed tomography, and magnetic resonance imaging) at any examination stage. A set of the studies can provide the means of significantly analyzing the bone structures and soft tissues and the degree of injury and ident

HLA-B27 and gender independently determine the likelihood of a positive MRI of the sacroiliac joints in patients with early inflammatory back pain: a 2-year MRI follow-up study.

OBJECTIVES: To describe how inflammation on MRI of the sacroiliac joints in patients with recent-onset inflammatory back pain (IBP) evolves over time, and to study determinants of activity on MRI of the sacroiliac joint. METHODS: A 2-year follow-up study with annual MRI of the sacroiliac joints was conducted in patients with IBP of less than 2 years' duration. Images were scored for bone marrow oedema on short τ inversion recovery and enhancement after administration of gadolinium on T1. RESULTS: Of the 68 patients (38% male; mean age 34.9 ± 10.3 years) enrolled, 44 had a negative baseline M

Diagnosis and Treatment of Ankylosing Spondylitis.

Ankylosing spondylitis (AS) is a chronic inflammatory condition primarily affecting the axial bone and sacroiliac joints. Its etiology is co … lammation of the aorta), and spinal ankylosis that impacts posture and fatigue. The dagger sign and sacroiliitis on radiographs, in particular, are crucial for diagnosis. Early inflammatory alterations can be found using modern diagnostic tools such as MRI, and the HLA-B27 gene can help confirm the diagnosis. Overall, 80-95% of people with AS have the HLA-B27 …

Single-Centre Analysis of Magnetic Resonance Imaging of Sacroiliac Joints in a Paediatric Population.

Background: Sacroiliitis in children is usually connected with one of the subtypes of juvenile idiopathic arthritis (JIA), such as enthesitis-related arthritis, psoriatic arthritis, or undifferentiated arthritis. The main diagnostic method is magnetic resonance imaging (MRI) of the sacroiliac joints, which can reveal bone marrow edema (BME) as a sign of an active inflammation process. This research aimed to retrospectively investigate the associations between the clinical presentation, laboratory test results, and MRI results of the sacroiliac joints of children. Methods: A total of 152 paedia…

MRI provides moderate-to-high sensitivity and high specificity for nr-axSpA. Acute signs of sacroiliitis on MRI are not specific and have been observed in the absence of spondyloarthritis. Initial management should involve NSAIDs and a regular exercise program, while TNF and IL-17 inhibitors can be used for high disease activity unresponsive to these i …

HLA-B27 predicts a more extended disease with increasing age at onset in boys with juvenile idiopathic arthritis.

OBJECTIVE: Juvenile idiopathic arthritis (JIA) is a heterogeneous condition with very few clinical and laboratory signs that can help predict the course and severity of the disease in the individual patient. The cell-surface antigen HLA-B27 is well known to be associated with spondyloarthropathies, reactive arthritis, and enthesitis. HLA-B27 plays an important role in the classification of JIA, since evidence of sacroiliitis most often evolves after years of arthritis in other joints. We investigated the associations of HLA-B27 and the clinical manifestations of JIA using a method as close to …

Radiologic changes of cervical spine in ankylosing spondylitis.

Ankylosing spondylitis (AS) is characterised by its effects on the axial skeleton. The cervical spine is also vulnerable to the disease proc … l spine. The BASRI-cervical spine score correlated with the BASRI-lumbar spine and hip joint score, sacroiliitis, disease duration, and duration of inflammatory back pain and cervical symptoms. Our data suggest that radiographic changes to the cervical spine are frequent in AS, and can be predicted in the patients with old age, long duration of disease and inf …

Influence of age and gender on quantitative sacroiliac joint scintigraphy.

UNLABELLED: The value of quantitative sacroiliac joint scintigraphy for detecting sacroiliitis is controversial. Age and gender may contribute to this discordance. In previous reports, the number of control groups has been small and might not exactly reflect the change of sacroiliac/sacral (SI/S) ratios related to different age. In addition, the selection of control subjects was not strict. In most studies, care was not taken to ensure that control subjects did not have a history of back pain or any other relevant conditions. In addition, there was no requirement for a normal radiograph as a c…
