Endodermal sinus tumor of the paranasal sinuses.

BACKGROUND: We report a rare case of endodermal sinus tumor (EST) of the paranasal sinus in a 59-year-old man with a 6-week history of nasal obstruction. METHODS AND RESULTS: High-resolution MRI showed a right nasoethmoid mass with obstructive changes in the sphenoid, frontal, and right maxillary sinuses and orbital and intracranial extension. Histologic analysis showed 2 distinct histologic features: poorly differentiated carcinoma and EST. Immunohistochemical studies showed strong keratin staining in both components and restricted positivity of alpha-fetoprotein in the endodermal sinus-like

Hemangiopericytoma of the infratemporal fossa: progression toward malignancy in a 30-year history.

Hemangiopericytoma is a rare vascular tumor first described by Stout and Murray in 1942 and characterized by a proliferation of Zimmermann's pericytes, smooth muscle cells arranged around blood vessels. This tumor presents as a slowly enlarging painless mass. Diagnosis with certainty is often a difficult one because of the close likeness with other spindle cell tumors; it requires the help of immunohistochemical techniques and sometimes ultrastructural techniques. Only 15% of hemangiopericytomas are localized in the cervicofacial region; in particular, occurrence in the infratemporal fossa is

Long-term results of stereotactic gamma radiosurgery of meningiomas.

BACKGROUND: The early effects at a mean of 30 months and long-term results at 7 years after gamma radiosurgery for meningiomas were evaluated. METHODS: Changes in tumor size were evaluated every 3 to 6 months after treatment using a five-point grading system, as well as changes in neurological signs and general status. RESULTS: Early effects in 87 cases of benign meningioma showed a minimal size reduction of 16.1% and a response rate of 8.0%, but a higher control rate of 93%. The cavernous sinus meningioma showed a size reduction of 23.2%, a response rate of 11.1%, and control of 100%. A gre

Outcome of craniofacial surgery in children and adolescents with malignant tumors involving the skull base: an international collaborative study.

BACKGROUND: The aim of the study was to characterize the distribution of anterior skull base (ASB) cancers in the pediatric population and to identify predictors of outcome. METHODS: This was an international study of 1307 patients undergoing craniofacial surgery for malignant tumors; 6.4% (n = 84) among these were <or=21 years old. RESULTS: The most common histologic type was sarcoma (40%), followed by squamous cell carcinoma (14%). Higher incidence of tumors was found in older children (7.5-21 years) compared to younger children. The 5-year disease-specific and overall survival rates were

Intensity modulated radiotherapy (IMRT) for recurrent, residual, or untreated skull-base meningiomas: preliminary clinical experience.

OBJECTIVE: To investigate the feasibility of using intensity modulated radiotherapy (IMRT) for complex-shaped benign meningiomas of the skull base and report clinical experience. METHODS: Twenty patients with benign skull-base meningiomas WHO degrees I (histopathologically proven in 16/20) were treated with IMRT between June 1998 and August 1999. Each tumor was complex in shape and adherent to, or encompassed, organs at risk (cranial nerves, optic apparatus, and brainstem). All patients, immobilized in a customized head mask integrated into a stereotactic system, were planned on an inverse tr

Stereotactic radiosurgery and stereotactic radiotherapy in the treatment of skull base meningiomas.

Meningiomas are the most common nonglial brain tumors. They tend to be slow growing and benign and can reach substantial sizes before becoming symptomatic. Complete surgical resection of intracranial meningiomas remains the treatment of choice. Location of a meningioma within the cranial vault may make complete surgical resection unlikely; tumors arising from the dura of the skull base are particularly challenging. Advances in radiation therapy, including stereotactic techniques, can expand the options for treatment available in these situations. They may be used as adjuncts to surgery or as a

Jugular Foramen Paraganglioma (Glomus Jugulare): A 10-Year Single-Center Experience.

BACKGROUND: Jugulotympanic paragangliomas (JTPs) are rare, hypervascular skull base tumors. Surgery and stereotactic radiosurgery (SRS) are often framed as competing options, yet outcome data from Latin American public referral centers are scarce. We evaluated a function-first surgical strategy and related tumor stage to outcomes. METHODS: Retrospective single-center study of 24 consecutive patients with histopathologically confirmed JTP who underwent 32 resections (2016-2025), analyzed at patient and procedure level. Fisch class (A/B vs. C/D) was related to extent of resection, cranial nerve

3D endoscopic endonasal craniectomy for intestinal type adeno-carcinoma (ITAC) of the nasal cavity.

BACKGROUND: The aim of this study was to describe the potential advantages of the 3D endoscope-assisted craniectomy for tumor of the nasal cavity. METHODS: A 77-year-old man with a 6 month history of persistent progressive right nasal obstruction and iposmia is reported. Physical examination, including nasal endoscopy, revealed a large mass within the right nasal cavity. He had no associated symptoms such as visual complaints, paresthesia, and facial pain. He worked as a carpenter. Further imaging by CT and MRI revealed a large, expansive nasal-ethmoid lesion that almost completely occupies t

Chemotherapy associated dural sinus thrombosis presenting as a cerebrospinal fluid leak.

Despite the well documented increased risk of thrombosis in patients with cancer and during chemotherapy, cerebral venous sinus thrombosis (CVT) remains a rare entity. We present a rare case of cerebrospinal fluid (CSF) rhinorrhoea secondary to a left transverse sinus thrombus which occurred 2 years previously during chemotherapy for breast cancer. The patient underwent a three-layer repair using Neuro-Patch, septal cartilage and middle turbinate pedicle flap and was started on acetazolamide. There was no recurrence at 1-year follow-up. Raised intracranial pressure secondary to cerebral venous

CSF leak after endoscopic skull base surgery in children: A single institution experience.

INTRODUCTION: The endoscopic expanded endonasal approach (EEA) has been shown to be a safe and effective surgical technique in the resection of pediatric skull base lesions. Cerebrospinal fluid (CSF) leaks are among the most common complications of this approach. Here we review skull base resections using EEA in pediatric patients at our single institution, to identify potential risk factors for this surgical complication. METHODS: A retrospective chart review was conducted on pediatric patients at our single institution for patients 19 years-old and under, who underwent an EEA for resection

Hypernasality after using the endoscopic endonasal transsphenoidal approach for skull base tumors.

OBJECTIVES/HYPOTHESIS: The increasing number of endoscopic endonasal transsphenoidal approaches (EETSA) has resulted in several sinonasal complications, including voice changes. Here, we compared preoperative and postoperative voice changes according to age. METHODS: We retrospectively reviewed the medical records. Patients were divided into the following three groups according to age: ≤ 30 years, 31 to 60 years, and ≥ 60 years. Patients underwent preoperative voice evaluation using acoustic analysis, a nasometer, and the voice handicap index (VHI). A nasometer was used to measure the nasalan

A rare case of chordoma and craniopharyngioma treated by an endoscopic endonasal, transtubercular transclival approach.

Co-occurrence of different brain tumors is rarely observed, being more frequent in patients affected by genetic syndromes like phacomatoses. Different histological types of intracranial lesions may present at different times in the clinical history of the patient or, more rarely, they may occurr at the same moment. In these last cases, particularly for tumors located in adjacent areas of the brain, diagnostic difficulties may arise. Moreover, even when the correct diagnosis is established, treatment strategy becomes complex and a single staged approach could be ineffective in obtaining success

Radiosurgery for chordomas and chondrosarcomas of the skull base.

OBJECT: Chordomas and chondrosarcomas of the skull base are aggressive and locally destructive tumors with a high tendency for local progression despite treatment. The authors evaluated the effect of stereotactic radiosurgery (SRS) on local tumor control and survival. METHODS: Twenty-eight patients with histologically confirmed chordomas (18) or chondrosarcomas (10) underwent Gamma Knife SRS either as primary or adjuvant treatment. Their ages ranged from 17 to 72 years (median 44 years). The most common presenting symptom was diplopia (26 patients, 93%). In two patients, SRS was the sole trea

Transpterygoid transposition of a temporoparietal fascia flap: a new method for skull base reconstruction after endoscopic expanded endonasal approaches.

BACKGROUND: Endoscopic expanded endonasal approaches (EEAs) for the resection of lesions of the anterior and ventral skull base can create large defects that present a significant risk of postoperative cerebrospinal fluid (CSF) leak. These defects, especially in patients who received preoperative radiotherapy, are best reconstructed with vascularized tissue. The Hadad-Bassagasteguy flap, a pedicled nasoseptal flap, is our preferred method for reconstruction. This option is not available, however, in patients who underwent a previous posterior septectomy or in those with tumors that invade the
