Electronic integration of glottic closure and circopharyngeal relaxation for the control of aspiration: a canine study.

Aspiration can result from muscular weakness or paralysis of laryngopharyngeal muscles after lower motor neuron disorders (e.g., stroke) or unchecked gastroesophageal reflux. We submit that rehabilitation of the finely tuned swallowing mechanism should provide at least restoration of the normal dynamic relationships between glottic closure and cricopharyngeal relaxation. In three dogs under general endotracheal anesthesia, the recurrent laryngeal nerves and the pharyngeal musculature were exposed through a midline cervical incision. A tracheotomy was performed to allow unhindered laryngoscopic…

The management of advanced subglottic carcinoma with stomal invasion: a case report.

Primary subglottic carcinoma is rare. The initial stage is usually asymptomatic and the carcinoma is advanced when the symptoms are in evidence. Because the trachea and thyroid gland adjoin to the subglottic area, the tumor invades them along with the superior mediastinum and lung by the way of paratracheal lymph drainage. To resuscitate a severe airway obstruction, an emergency tracheotomy is required, which, however, results in a higher frequency of stomal recurrence later. We treated a case with advanced subglottic carcinoma and performed extended radical neck dissection with the resection …

Pulmonary function studies in a newborn with congenital laryngeal atresia.

Congenital laryngeal atresia is a rare cause of upper airway obstruction that leads to death unless a surgical airway is immediately established. We were able to resuscitate a baby boy with laryngeal atresia by the placement of an 18-gauge plastic intravenous cannula into the trachea, connected in turn to a 3-mL syringe without the plunger, and then to the connector to a 7.0-mm endotracheal tube. This arrangement allowed hand ventilation and sufficient gas exchange until a formal tracheotomy was established minutes later. The baby boy had deficient abdominal musculature (without cryptorchidism…

[Laryngocele: analysis of 18 cases and review of the literature].

18 cases of laryngocele (8 combined, 6 external, 4 internal) treated at the E.N.T. Clinic from January 1968 to December 1989 are reported. T … of small internal laryngoceles can endoscopic CO2 laser excision be considered a valid alternative. Tracheotomy is usually not indicated.

Early and late dilatation for acquired subglottic stenosis.

There has been widespread use of periodic dilatations in the management of subglottic stenosis. However, some authorities have questioned th … nosis developed in all twelve experimental animals after 8 weeks of dilatation, but none required a tracheotomy. These twelve animals were then subjected to 8 additional weeks of dilatation and antibiotics, and supplemental steroids were used in some animals from this study. It can be concluded that early periodic dilatation and granulation removal in the acu …

Airway Management Following Tracheal Surgery.

The ability to remove longer segments of airway and to extend resections into the larynx proper has managed to create novel situations that will require attention to postoperative management. This article deals with prophylactic measures to prevent the requirement of assisted ventilation. It, however, also emphasizes various bronchoscopic and intubation techniques, which if required, will help to avoid trauma to the airway anastomosis. In addition, a variety of ventilator modalities are discussed that were developed by the author over many years at the Toronto General Hospital.

The comorbidities and risk factors in children with congenital airway anomalies: A nationwide population-based study in Taiwan.

The comorbidities and risk factors associated with congenital airway anomalies (CAAs) in children are undecided. This study aimed to investigate the comorbidities commonly associated with CAA and to explore the prognosis and risk factors in CAA children. This nationwide, population-based cohort study was conducted between 2000 and 2011 with children aged 0 to 5 years assigned to either a CAA group (6341 patients) that diagnosed with CAA or an age- and gender-matched control group (25,159 patients) without CAA, using the Taiwan National Health Insurance Research Database (NHIRD). Descriptive, lo

[A case of post-radiation therapy patient with difficulty in intubation unexpected preoperatively].

We experienced a post-radiation therapy patient who had a narrow trachea, and presented with unexpected difficulty for intubation. His trachea was narrowest at 2 cm below the glottis. The fiberscope barely could pass the narrowest part of the trachea. We speculate that the radiation therapy induced his tracheal constriction. Careful attention is necessary when examining the post-radiation therapy patients. Multiple cervical radiographs are necessary in such a case.

Hemicricoidectomy as the primary diagnosis and treatment for cricoid chondrosarcomas.

OBJECTIVES/HYPOTHESIS: The objective was to present a new approach for the diagnosis and treatment of chondrosarcoma involving the cricoid c … ade tumor. Primary treatment included hemicricoidectomy in all cases. In six (75%) of the patients, tracheotomy was required at the initial surgery. The mean time for decannulation was 3.2 months (range, 5 d-1 y). In four cases a second procedure was required because of recurrence. The second procedure included two total laryngectomies and two endoscopic carb …

[Preoperative nasal continuous positive airway pressure treatment as substitute for protective tracheostomy in severe obstructive sleep apnea syndrome].

OBJECTIVE: Preoperative nasal continuous positive airway pressure(NCPAP) treatment was used as a substitute for protective tracheostomy before UPPP surgery in 42 patients with severe obstructive sleep apnea syndrome(OSAS) from 1994 to 1996. METHODS: Fifty-two patients operated on three years before this period served as control group. All cases were diagnosed as severe OSAS by polysomnography (PSC). RESULTS: There were no statistical difference in main parameters between two groups. Seven cases in the control group underwent protective tracheostomy whose apnea and hypopnea index(AHI) was 28.

Peristomal recurrence.

INTRODUCTION: Peristomal recurrence following laryngectomy is a very serious and fatal complication. To treat this complication, extensive surgical resection has been advocated. However, when peristomal recurrence develops, the prognosis is dismal and the mortality is nearly 100%. Therefore, attention should focus on prevention. METHODS: Treatment of patients with tumors of the subglottis should include attention to the paratracheal region and superior mediastinum. For this reason pretracheal, paratracheal, and retrosternal dissection has been performed in patients with subglottic lesions sin

Supracricoid hemilaryngopharyngectomy. Analysis of 240 cases.

The supracricoid hemilaryngopharyngectomy consists of resection of the supracricoid hemilarynx and ipsilateral pyriform sinus. Two hundred forty patients underwent this procedure from 1964 through 1983. Two hundred twenty-two patients had no airway impairment, and 204 recovered normal deglutition. The local recurrence rate was 5.2%. Indications for the procedure are carcinoma of the supracricoid upper part of the pyriform sinus and carcinoma of the lateral laryngeal margin with normal vocal cord mobility.

Pediatric home care: an ambulatory "special care unit".

A pediatric home care program is described which has operated as an ambulatory special care unit in an urban hospital center since 1970. The program provides care for seriously or chronically ill children, who would not be adequately served through traditional ambulatory services because of either the severity and complexity of their medical problems or the inability of their families to use such services. An interdisciplinary PHC team provides comprehensive pediatric care and integrates medical and psychosocial services for the child and family. This form of special ambulatory care appears to
