Assessment of auditory and vestibular functions after vestibular neurectomy for Menière's disease.

Fourteen patients (12 Meniere's disease, one delayed endolymphatic hydrops and one labyrinthitis) suffering from intractable vertigo were treated with retrosigmoid vestibular neurectomy (RSVN) in our institute. Dizziness was controlled completely in all patients. Hearing was preserved in 91.7% of the cases with pure tone threshold deteriorating by more than 10 dB in one case, remained unchanged in 10 cases and improved in one case during long term observation. Substantial decrease of tinnitus was observed in 78.6%. Vestibular compensation was chronologically analyzed with a stabilometer, ENG a

Autonomic nervous system disturbance as etiological background of vertigo and dizziness.

In order to investigate whether dysautonomia lies behind vertigo and/or dizziness in the neurotological clinic, an electrocardiogram (ECG) R-peak to R-peak test (R-R test) along with the Schellong test were simultaneously performed in 136 subjects. ECG data were analyzed by a microcomputer, and R-R intervals were calculated. In a dynamic test performed during postural changes from the supine to the standing position, there were 23 of 55 (41.8%) positive test results in patients with peripheral vestibular disorders, 28 of 51 (54.9%) with central vestibular disorders, and 15 of 18 (83.3%) with s

[Peripheral vestibular vertigo and functional disorders of the craniovertebral joint].

Some vertiginous lesions producing nystagmus (e.g. acute vestibulopathy, benign paroxysmal postural vertigo and Ménière's disease) are well-defined diseases of purely labyrinthine origin. The possible place of a functional disorder of the vertebral joints on their etiology and pathogenesis, has been discussed, but is purely hypothetical. Since the lesion almost always reaches with reduced function of the vestibular apparatus, the course of the disease depends crucially on central compensation of the peripheral disorder of vestibular function. Investigations of the effect of a functional disord

Recovery of turning speed in patients after vestibular schwannoma resection.

BACKGROUND: Individuals after a vestibular schwannoma resection (VSR) experience significant vestibular symptoms that can be provoked with turning. Vestibular rehabilitation assists in recovery of function and symptom relief, however turning response is unknown. OBJECTIVE: Examine peak turning speed response to surgery and rehabilitation. METHODS: Eight participants with a vestibular schwannoma (PwVS) and five healthy controls (HC) participated in this study. Peak turning speed (PTS) was captured with inertial measurement units (IMU) at the head and/or trunk during turning tasks at a pre-ope

Utility of 1 Measurement Versus Multiple Measurements of Near Point of Convergence After Concussion.

CONTEXT: Increased near point of convergence (NPC) distance is a common finding after concussion and is associated with physical symptoms and worsened neurocognitive performance. Vestibular/Ocular Motor Screening measures NPC distance across 3 trials and uses the average measurement to inform clinical care. However, whether 3 trials are necessary, are consistent, or add clinical utility is unknown. OBJECTIVE: To investigate the consistency across 3 trials of NPC and establish the classification accuracy (ie, clinical utility) of 1 or 2 trials compared with the standardized average of 3 trials

Subtle alterations of vestibulomotor functioning in conductive hearing loss.

INTRODUCTION: Conductive hearing loss (CHL) attenuates the ability to transmit air conducted sounds to the ear. In humans, severe hearing loss is often accompanied by alterations to other neural systems, such as the vestibular system; however, the inter-relations are not well understood. The overall goal of this study was to assess vestibular-related functioning proxies in a rat CHL model. METHODS: Male Sprague-Dawley rats (N=134, 250g, 2months old) were used in a CHL model which produced a >20dB threshold shift induced by tympanic membrane puncture. Auditory brainstem response (ABRs) recordi

Athletes Who Test Positive on Multiple Domains of the Vestibular-Ocular Motor Screen Take Longer to Return to Play.

BACKGROUND: The Vestibular-Ocular Motor Screen (VOMS) is a validated tool that is used to assess symptom provocation with vestibular and ocular testing following sport-related concussion (SRC). It is unknown if the involvement of multiple domains on the VOMS effects time to return to play (RTP) following SRC. HYPOTHESIS: Athletes who tested positive on >3 domains of the VOMS would take longer to RTP than athletes who tested positive on ≤3 domains of the VOMS. STUDY DESIGN: Retrospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: A total of 928 athletes were enrolled from a concussi

Recurrence in patients with benign paroxysmal positional vertigo of the lateral semicircular canal.

OBJECTIVES: There are 3 subtypes of lateral canal benign paroxysmal positional vertigo. Persistent geotropic positional nystagmus is due to a light cupula, heavy cupula produces apogeotropic positional nystagmus, and canalolithiasis accounts for transient geotropic positional nystagmus. The aims of this study were to determine the recurrence rate and to examine the number of times of recurrence in each of the 3 subtypes. METHODS: Subjects were patients with light cupula (n = 47), heavy cupula (n = 48), and canalolithiasis (n = 45). We investigated the number of times of recurrence and determi

A case of bilateral parietal cortical laminar necrosis with a loss of vertiginous sensation.

BACKGROUND: Animal experiments demonstrated that there are vestibular cortical areas at the parietal cortex. Moreover, in humans, recent functional neuroimaging studies revealed that caloric stimulation activated the parietoinsular vestibular cortex and optokinetic stimulation activated the parieto-occipital cortex. These activations indicate that the parietal vestibular areas play some role in nystagmus generation or in spatial information processing in the eye movement tasks. AIMS OF THE STUDY: The aim of this communication was to present a patient giving some information about parietal cor

Patients with CHARGE association: a model to study saccular function in the human.

The term CHARGE association refers to a combination of congenital malformations, the mnemonic CHARGE designating the most frequently occurring anomalies in the constellation. "C" indicates coloboma of the retina, "H" heart defects, "A" choanal atresia, "R" retarded growth and/or central nervous system anomalies, "G" genital hypoplasia, and "E" ear anomalies and/or deafness. The inner ear anomaly consists of a specific form of labyrinthine dysplasia that includes Mondini dysplasia of the pars inferior (cochlea and saccule) and complete absence of the pars superior (utricle and semicircular cana

[Neuro-otologic diagnosis after long-term exposure to wood preservatives containing pentachlorophenol].

We examined the possible vestibulotoxicity of 16 patients subject to chronic exposure to wood preservatives containing pentachlorophenol. A vestibular test battery showed no abnormal findings except in one patient with pre-existing Menière's disease. The discrepancy between a clear history of exposure contrasted with the normal vestibular finding, and illustrates the diagnostic problems in patients chronically exposed to wood preservative.

Vestibular stimulation after head injury: effect on reaction times and motor speech parameters.

Earlier studies by other authors indicate that vestibular stimulation may improve attention and dysarthria in head injured patients. In the present study of five severely head injured patients and five controls, the effect of vestibular stimulation on reaction times (reflecting attention) and some motor speech parameters (reflecting dysarthria) was investigated. After eight weeks with regular stimulation, it was concluded that reaction time changes were individual and consistent for a given subject. Only occasionally were they shortened after stimulation. However, reaction time was lengthened

Study of the effects of simultaneous exposure to noise and carbon disulfide on workers' hearing.

The aim of this study was to explore the effects of simultaneous exposure to noise and carbon disulfide on workers' hearing and balance. The study was conducted by interviews and by audiometric and balance tests on workers in a rayon factory in the city of São Paulo, Brazil (n = 258). The workers studied had a history of exposure both to excessive noise levels (86-89 dBA) and to excessive levels of carbon disulfide (89.92 mg/m3). The percentage of hearing loss found was much higher than expected for this occupational activity, which reinforced the possible connection between the exposure to no
