Occult brodifacoum poisoning presenting as a massive intramuscular hematoma.

INTRODUCTION: Brodifacoum induces coagulopathy after absorption, causing severe systemic hemorrhage. Occult hemorrhage may delay diagnosis and treatment. CASE SUMMARY: A 56-year-old man was referred to our hospital with worsening right thigh swelling for nine days. He had no recent trauma or medications affecting his INR. On admission, laboratory tests revealed an elevated PT of 75 s and INR of 9.05, and serum brodifacoum concentration of 25 µg/L (0.0478 µmol/L). Imaging confirmed a large right thigh hematoma. Forty-three days prior to this presentation, he had presented with life-threatening

Managing Methotrexate Intolerance in Rheumatoid Arthritis: The Role of Oral-to-Subcutaneous Switching.

Methotrexate (MTX) remains the anchor drug in the management of rheumatoid arthritis (RA). However, its clinical use is frequently limited by intolerance, leading to dose reduction or discontinuation. In addition, oral MTX exhibits pharmacokinetic limitations, including dose-dependent absorption and reduced bioavailability at higher doses, which may compromise therapeutic efficacy. Subcutaneous (SC) MTX has emerged as a strategy to overcome these limitations by providing more consistent bioavailability and potentially improved tolerability. Evidence suggests that switching from oral-to-SC MTX

A self-triggering and fully sustainable gastric device enabling long-acting oral delivery of macromolecules.

Long-acting oral delivery of macromolecular therapeutics remains difficult, as most oral delivery devices focus on improving gastrointestinal absorption but do not adequately address sustained drug release and frequent dosing. In parallel, many gastric-retentive systems rely on nondegradable structural components, raising concerns regarding retained-device burden and postuse medical waste. Here, we report a biodegradable oral capsule device for long-lasting drug delivery, termed the sustainable gastric self-triggered device (SGSTD), inspired by the stinging mechanism of Apis mellifera. After r…

Gastrointestinal dysfunction after brain injury: Mechanisms and the role of the brain-gut axis.

Brain injury (BI) encompasses traumatic BI and stroke, and is a leading cause of mortality and morbidity worldwide. In addition to primary brain damage, BI can cause a series of sequelae, of which gastrointestinal (GI) dysfunction is one of the most important affecting patient outcomes. GI dysfunction, including delayed gastric emptying, increased intestinal permeability, and gut dysbiosis, is common among patients with BI. GI dysfunction not only impairs nutrient absorption and increases the risk for infection, but also enhances secondary BI by aggravating the systemic inflammatory response.

Nanocurcumin in Oral Disease Therapy: A Review of Innovations and Future Prospects.

Curcumin exhibits strong anti-inflammatory, antioxidant, and anticancer activities. However, its clinical use is limited due to poor solubility, low gastrointestinal absorption, and rapid systemic metabolism. Nanocurcumin offers enhanced solubility, bioavailability, and targeted delivery through systems such as nanoparticles, liposomes, micelles, and nanoemulsions. In oral health, nanocurcumin has shown significant therapeutic promise. In periodontitis models, it attenuates pro-inflammatory cytokines and oxidative stress markers. In radiation- and chemotherapy-induced oral mucositis, randomize…

Apigenin derivatives as promising norovirus RdRp inhibitors: insights from in silico docking and molecular dynamics studies.

Norovirus is a leading cause of acute gastroenteritis worldwide, yet no approved antivirals currently exist. In this study, we employed in silico approaches to evaluate apigenin derivatives as potential inhibitors of norovirus RNA-dependent RNA polymerase (RdRp). Seventy-three compounds were initially retrieved from the ZINC database, of which 36 satisfied Lipinski's Rule of Five and were advanced for detailed analysis.Pharmacokinetic predictions revealed high intestinal absorption and favourable drug-likeness profiles for most derivatives, with limited central nervous system penetration, desir

Transcriptomic Alteration in the Brain and Gut of Offspring Following Prenatal Exposure to Corticosterone.

Maternal stress during pregnancy can profoundly affect offspring health, increasing the risk of psychiatric disorders, metabolic diseases, and gastrointestinal problems. In this study, the effects of high prenatal corticosterone exposure on gene expression in the brain and small intestine of rat offspring were investigated via RNA-sequencing analysis. Pregnant rats were divided into two groups: Corti.Moms were injected with corticosterone daily, while Nor.Moms were given saline injections. Their offspring were labeled as Corti.Pups and Nor.Pups, respectively. The brain tissue analysis of Corti

Flexibility-powered Pickering emulsion enhances mucus permeability to alleviate ulcerative colitis.

The physicochemical properties of oral drug delivery carriers significantly influence their interactions with the intestinal barrier, which, in turn, affect their in vivo fate and therapeutic efficacy. As a delivery system with a hierarchical structure and high stability, Pickering emulsions have demonstrated significant advantages in enhancing the bioavailability of encapsulated bioactive compounds. However, the specific role of their inherent flexibility in modulating interactions with the intestinal barrier and influencing delivery efficiency remains poorly understood. In this study, a flex

Management of Gastrointestinal Symptoms in Parkinson's Disease.

Gastrointestinal (GI) dysfunction is a common and often underappreciated aspect of Parkinson's disease (PD), with symptoms manifesting at multiple levels of the digestive tract, from swallowing difficulties to challenges with defecation. These non-motor symptoms can be more debilitating than the hallmark motor impairments of PD, profoundly affecting patients' quality of life. The burden of GI issues in PD extends beyond discomfort, contributing to malnutrition, weight loss, and impaired medication absorption, which can exacerbate both motor and non-motor symptoms. Despite their clinical signif

Clinical features and management strategies of acute soluble barium poisoning: a review of case reports.

Soluble barium salts including barium carbonate, nitrate, acetate, and chloride are commonly used in the manufacture of ceramics, insecticides, and rodenticides. Soluble barium salts are highly toxic to humans if ingested. Acute soluble barium poisoning is an uncommon but life-threatening problem. The fatal dose of barium chloride for man has been reported to be only 0.8 to 0.9 gram. Currently, there have been no systematic studies on acute soluble barium poisoning. Based on the retrospective analysis of 55 global cases, toxicological characteristics, clinical features and management of acute

Exocrine Pancreatic Insufficiency: Postoperative Complication Emerging Decades after Surgery?

Exocrine pancreatic insufficiency (EPI) arises from a deficiency in pancreatic enzymes, leading to malabsorption and varied symptoms such as gastrointestinal discomfort, weight loss and steatorrhea. Though EPI commonly follows chronic pancreatitis or pancreatic surgery, its delayed onset is rarely discussed. We report a unique case of EPI manifesting 29 years after pancreaticoduodenectomy. A 71-year-old female, who underwent pancreaticoduodenectomy for carcinoma in 1992, presented with fatigue, weight loss, edema, diarrhea and paresthesias. Physical examination revealed mucocutaneous pallor, a

Use of novel taurine-chitosan mediated liposomes for enhancing the oral absorption of doxorubicin via the TAUT transporter.

Our research aimed to enhance the oral bioavailability of doxorubicin hydrochloride (DOX·HCl) while minimizing the potential for myocardial toxicity. To achieve this goal, we developed a new method that utilizes a coating material to encapsulate the drug in liposomes, which can specifically target intestinal taurine transporter proteins. This coating material, TAU-CS, was created by combining taurine with chitosan. We characterized TAU-CS using various methods, including 1H NMR, FT-IR, and scanning electron microscopy (SEM). The resulting liposomes exhibited a regular spherical morphology, wit

Mechanisms and Functions of Sweet Reception in Oral and Extraoral Organs.

The oral detection of sugars relies on two types of receptor systems. The first is the G-protein-coupled receptor TAS1R2/TAS1R3. When activated, this receptor triggers a downstream signaling cascade involving gustducin, phospholipase Cβ2 (PLCβ2), and transient receptor potential channel M5 (TRPM5). The second type of receptor is the glucose transporter. When glucose enters the cell via this transporter, it is metabolized to produce ATP. This ATP inhibits the opening of KATP channels, leading to cell depolarization. Beside these receptor systems, sweet-sensitive taste cells have mechanisms to r

Artificial digestion represents the worst-case scenario for studying nanoplastic fate in gastrointestinal tract.

Humans may inevitably be exposed to nanoplastics (NPls) through ingestion. The size of NPls significantly influences their absorption efficiency, so understanding behaviors of NPls during digestion is vital for risk assessment. In this study, fluorescent polystyrene (PS) and melamine-formaldehyde resin (MF) NPls were characterized by different techniques after the in vitro digestion process both with and without a standard food model, or with and without pH adjustment in the absence of the proteins. Results derived from the in vitro method were compared to those using human and porcine digesti
