Anterior Locking Plate Osteosynthesis of Patellar Factures - Analysis of Complications and Functional Outcome.

BACKGROUND: Tension band wiring is the standard procedure for patellar fractures, but is associated with a high rate of implant related complications and implant failure. Tension band wiring may fail, especially with multifragmentary and comminuted fractures. Plate fixation of complex patellar fractures seems to be superior to wiring, both clinically and biomechanically. The aim of this study was to evaluate complications after locking plate fixation in patellar fractures two years after surgery and to access the functional outcome. MATERIAL AND METHODS: As part of a prospective case series,

Outcomes of Medializing Tibial Tubercle Osteotomy With Medial Reefing and Vastus Medialis Obliquus Advancement Coupled With Lateral Retinacular Z-Lengthening for Recurrent Patellar Instability.

PURPOSE: Patella-femoral dislocations, although rare, can result in functionally limiting symptoms and limited return to prior activity levels. The purpose of this study is to report outcomes of a three-in-one patellar realignment surgery for treatment of recurrent patellar instability in a young, active duty military population. METHODS: We conducted a retrospective chart review of all patients who underwent our senior author's three-in-one patellar realignment surgery for recurrent patellar instability in the setting of underlying anatomic malalignment. Our primary outcome measure was the r

Osteochondral Allograft Transplantation for Femoral Trochlear Dysplasia.

BACKGROUND: The risk factors for patellofemoral joint instability include laxity of medial patellar restraints, abnormal limb geometry, femoral and tibial malrotation, patella alta, and trochlear dysplasia. Femoral trochlear dysplasia is characterized by a hypoplastic or shallow trochlear groove. CASE REPORT: We report the case of a 31-year-old female with trochlear dysplasia and recurrent patella dislocations, laxity of the medial patellofemoral ligament (MPFL), and high-grade chondromalacia of the trochlea and the patella. Surgical treatment goals were to re-create a trochlear groove, resto

Biomechanical comparison of different fixation techniques for reconstruction of tibial avulsion fractures of the anterior cruciate ligament.

PURPOSE: The most common fixation techniques for tibial avulsion fractures of the anterior cruciate ligament (ACL) described in the literature are screw and suture fixation. The fixation of these fractures with the TightRope® device might be an alternative. Up to now it has been commonly used in other injuries, such as acromioclavicular joint or syndesmosis ruptures. The purpose of this study was to evaluate the biomechanical properties of different fixation techniques for the reconstruction of tibial avulsion fractures. METHODS: Type III tibial avulsion fractures were simulated in 40 porcine

Patients with lateral tracking patella have better pain relief following CT-guided tuberosity transfer than patients with unstable patella.

In patients with either lateral tracking patella or unstable patella the pathological lateral position of the tuberosity can be corrected by a medial transfer. This study compared the results of subtle CT-guided correction of the tuberosity for objective unstable patella (n=27) with the results for lateral tracking patella (potential instability) as described by Dejour (n=16). Follow-up was 37 months. CT revealed a pathological lateralization of the tibial tuberosity-trochlear groove greater than 15 mm in 41 knees. These patients underwent medialization of the tibial tuberosity up to 10-12 mm

The Elmslie-Trillat procedure for recurrent subluxation of the patella. One to five year follow-up.

Thirty-four patients who had undergone the Elmslie-Trillat procedure from 1985 to 1989 were examined in order to evaluate the effectiveness of this operation in the treatment of recurrent subluxation of the patella. The result of an increased Q angle accompanied by patella alta, femoral sulcus dysplasia, or vastus medialus obliquus dysplasia was also studied. Only those patients with recurrent subluxation of the patella took part in this study; cases of habitual or permanent dislocation, potential instability, traumatic dislocation, and degenerative arthritis were excluded. The patients were r

Modular megaprostheses in the treatment of periprosthetic fractures of the femur.

BACKGROUND: Periprosthetic fractures (PPF) of the femur remain challenging, especially in patients with previous multiple revisions. Modular megaprostheses (mMPs) are rarely used in this indication; however, in some cases mMPs seem to be the last chance for limb salvage. We aimed to evaluate the clinical outcome of PPFs of the femur treated by modular mMPs at our institution. PATIENTS AND METHODS: In this study 33 patients (27 female; mean age 79 years) with a PPF after total hip or total knee arthroplasty (no tumor indications) were treated using modular proximal (mPFR; n = 12), distal (mDFR

Osteochondral mosaicplasty of the femoral head.

Young adults with osteochondral lesions of the femoral head are at risk of rapid progression to symptomatic arthritis of the hip joint. Between January 2008 and July 2009, 10 patients were treated for femoral cartilage damage by a osteochondral mosaicplasty of the femoral head through a trochanteric flap with dislocation of the hip. The consecutive series had the following exclusion criteria: acetabular chondropathy, age above 25 years, and femoral head osteonecrosis. Patients were followed up after surgery using the Oxford-12 score, Harris hip score and the Merle d'Aubigné score, and activity

Acceptable outcomes for isolated medial patellofemoral ligament reconstruction despite using higher thresholds for patella alta and regardless the value of tibial tubercle-trochlear groove distance and the degree of trochlear dysplasia-An international multicenter study.

INTRODUCTION: When planning surgery for treatment of recurrent patellar instability, one of the questions of interest is how far the indications for an isolated medial patellofemoral ligament (MPFL) reconstruction can be extended. This international multicenter study followed the outcomes of patients undergoing isolated MPFL reconstruction using higher thresholds for patella alta and tibial tubercle-trochlear groove (TT-TG) distance imaging measurements than in the current literature and in common practice algorithms. METHODS: One hundred ninety-nine patients from five countries (USA, Austral

Lateral retinacular release in concordance with medial patellofemoral ligament reconstruction in patients with recurrent patellar instability: A computational model.

BACKGROUND: The aim of this study was to develop and validate a finite element (FE) model of the patellofemoral joint to analyze the biomechanics of lateral retinacular release after medial patellofemoral ligament (MPFL) reconstruction in patellar malalignment (increased tibial tubercle-trochlear groove distance (TT-TG)). We hypothesized that lateral retinacular release is not appropriate in patellar instability addressed by MPFL reconstruction due to decreased lateral stability and inappropriate adjustment in patellofemoral contact pressures. METHODS: A FE in-silico model of the patellofemor

The Anatomy of the Medial Patellofemoral Ligament.

Recurrent patellar dislocation is observed in many patients treated nonoperatively following primary dislocation. Injury to the medial patellofemoral ligament (MPFL) is reported in the majority of patients following dislocation. There is an increased interest in repair or reconstruction of the MPFL for patients experiencing recurrent instability. The femoral attachment of the MPFL is critical in determining graft behavior following reconstruction. The femoral attachment can be determined by referencing local anatomy, fluoroscopic imaging or on the basis of desired graft-length changes. This ar

[MRI after patellar dislocation: assessment of risk factors and injury to the joint].

Patellar dislocation is the lateral displacement of the patella from the femoral trochlea. Affected individuals typically have underlying anatomic risk factors of variable magnitude, which, in conjunction with leg rotation, cause the event. Magnetic resonance imaging (MRI) permits straightforward diagnosis of the typical features of recent patellar dislocation: contusion edema of the inferomedial patella and the lateral femoral condyle as well as rupture of the medial patellofemoral ligament. In case of concomitant osteochondral injury, early surgical refixation may be indicated, depending on

Cartilage lesions in patellofemoral dislocations: incidents/locations/when to treat.

Patellofemoral (PF) dislocations are frequently associated with chondral injury. Chondral and osteochondral lesions are often associated with traumatic (high-energy) PF dislocations, whereas atraumatic (low-energy) PF dislocations in patients with significant PF risk factors have a much lower incidence of osteochondral damage. This article provides a historical overview and delineates the current state of radiographic and clinical outcomes of osteochondral lesions after PF dislocation. The importance of understanding risk factors of redislocation is emphasized, and the current treatment option

Outcomes of medial patellofemoral ligament reconstruction for patellar instability: a systematic review.

It has been suggested that in the majority of patellar dislocation cases the medial patellofemoral ligament (MPFL) is disrupted. Accordingly several authors over the past decade have recommended repair or reconstruction of the MPFL to reduce the high incidence of recurrent dislocation. The purpose of this review was to determine whether MPFL reconstruction is a suitable procedure with regards to clinical and radiological outcomes for patients with patellar instability. A literature search using the electronic databases AMED, British Nursing Index, CINAHL, the Cochrane database, EMBASE, ovid Me
