Bad split during bilateral sagittal split osteotomy of the mandible with separators: a retrospective study of 427 patients.

An unfavourable fracture, known as a bad split, is a common operative complication in bilateral sagittal split osteotomy (BSSO). The reported incidence ranges from 0.5 to 5.5%/site. Since 1994 we have used sagittal splitters and separators instead of chisels for BSSO in our clinic in an attempt to prevent postoperative hypoaesthesia. Theoretically an increased percentage of bad splits could be expected with this technique. In this retrospective study we aimed to find out the incidence of bad splits associated with BSSO done with splitters and separators. We also assessed the risk factors for b

[Hybrid osteosynthesis in orthognathic surgery: 28 cases of Le Fort I osteotomy].

INTRODUCTION: The use of resorbable materials in orthognathic surgery is presently the object of numerous clinical and experimental studies. Their main drawbacks are their lack of rigidity and induced chronic inflammatory reactions. The purpose of this study was to propose and to test an original system of hybrid osteosynthesis associating a single titanium plate with three resorbable plates in Le Fort I osteotomies in the absence of maxillary expansion, or two titanium plates with two resorbable plates in case of maxillary expansion. METHODS: Our retrospective study concerned 28 patients ope

Incidence and risk factors of alveolar bone dehiscences and fenestrations after clear aligner therapy with Class II elastics: a retrospective study.

BACKGROUND: In clear aligner therapy (CAT), Class II elastics are widely used to reinforce the anchorage during the distalization of upper molars, however, their association with alveolar bone dehiscences (ABDs) and fenestrations (ABFs) in the anterior region remains unclear. The aim of this research is to assess the incidence of ABDs/ABFs in adult patients undergoing non-extraction CAT with Class II elastics, and to explore risk factors associated with the occurrence of ABDs/ABFs. METHODS: Thirty adult patients with Class II malocclusion who underwent non-extraction CAT with Class II interma

Is significant mandibular advancement possible after the peak of puberty? Dento-osseous palatal expansion and the STM4 technique (Skeletal Therapy Manni Telescopic Herbst 4 miniscrews): A case report.

INTRODUCTION: Treatment of skeletal class II growing patients often requires the use of functional appliances, aimed at promoting mandibular advancement. Among these, Herbst appliance is recommended for its effectiveness, efficiency, and reduced need for compliance. Despite its skeletal favourable effects, well-known dental compensations can occur, especially when the appliance is not used close to the pubertal peak: upper incisors retroclination, lower incisors proclination, upper molars distalization and lower molars mesialization could reduce the overjet needed for a proper mandibular advan

Modification of Sagittal Split Osteotomy in Class II Asymmetry: Optimizing Bone Contact between Proximal and Distal Segments.

BACKGROUND: Computer-assisted surgical simulation (CASS) allows more precise orthognathic surgery. However, few studies have evaluated associations between CASS-designed bilateral sagittal split osteotomy (SSO) and bone contact surface in class II mandibular asymmetry. This study aims to evaluate the effects of using computer-assisted simulation and design modification of SSO to improve bony contact in skeletal class II asymmetry. METHODS: This retrospective analysis reviewed 28 patients with class II asymmetry who underwent orthognathic surgery, including 15 with modified SSO (group CS) and

Nonsurgical treatment of Brodie bite assisted by 3-dimensional planning and assessment.

This case report describes the nonsurgical treatment of an adolescent patient with a severe transverse discrepancy presented as a Brodie bite and retrognathic mandible. Distraction osteogenesis has been often used for similar cases in the literature. However, in this patient, a fixed appliance with 1 maxillary extraction combined with a functional appliance was used to resolve the transverse discrepancy with natural growth. After the orthodontic treatment, the impinging teeth and Brodie bite were corrected with a favorable occlusion and profile. Retention at the 3-year follow-up showed improve

Timing of Surgical Removal of Odontoma in a Growing Orthodontic Patient: A Case Report.

In patients with skeletal malocclusion and odontoma, the prioritization of treatment is ofgreat importance. Ifsurgical removal of odontoma is postponed, the probability of adjacent teeth impaction increases. In this case, skeletal treatment was performed before odontoma rgery due to concerns about facial appearance and the patient's fear of surgery, and therefore adjacent tooth failed to erupt.

First case of bilateral coronoid hyperplasia in monozygotic twin sisters-a new aetiological perspective?

BACKGROUND: Mandibular coronoid hyperplasia is a rare condition associated with gradual reduction in mouth opening. Its aetiology is unknown but increased temporalis activity, endocrine stimulus, trauma and familial causes have been proposed. CASE REPORT: In this article, we present a case of bilateral coronoid hyperplasia presenting with limited mouth opening in 16-year-old Caucasian twin sisters. DISCUSSION: This report gives a new perspective to the aetiology of coronoid hyperplasia since it describes its occurrence in monozygotic twins and hence provides a strong argument in support of a

A case report of multidisciplinary treatment of an adult patient with bilateral cleft lip and palate.

This is a case report about the successful orthodontic treatment of a bilateral cleft lip and palate patient by using a combination of bone grafting and subsequent prosthodontic rehabilitation. An adult patient with a bilateral cleft lip and palate presented with a concave profile, anterior and lateral crossbite, a markedly deep overbite, and residual bilateral alveolar clefts. His jaw movement patterns were unstable and irregular due to his collapsed bite. Orthodontic treatment with bilateral bone grafting improved his concave profile by downward and backward rotation of the mandible within t
