Three-dimensional finite element assessment of grid-type plate in 2 different designs of sagittal split osteotomy of the mandibular ramus.

OBJECTIVE: To evaluate stress distribution in the grid plate-screw system and cortical bone in 2 sagittal split ramus osteotomy (SSRO) designs, with linear advancements of 5.0 mm and 7.0 mm, using finite element analysis. STUDY DESIGN: A three-dimensional mandibular model was reconstructed from computed tomography (CT) scan and processed into finite element meshes. Grid plates with 4 monocortical screws were virtually positioned in the tension zone. Two osteotomy designs were tested: conventional Epker technique (Group I) and short split technique (Group II). Each design underwent advancement

A Surgical Approach to Root Coverage and Correction of Mucogingival Conditions and Deformities in Mandibular Incisors with Isolated Gingival Recession: Free Mucogingival Graft. A Pilot Prospective Cohort Study.

The aim of this study was to present a periodontal plastic surgery approach to treat gingival recessions (GRs) and correct mandibular incisor mucogingival conditions and deformities. Isolated deep GRs (≥ 3 mm) in the mandibular incisors (n = 24 teeth) were treated: 66.6% of sites were recession types 2 or 3, and 58.3% of teeth were malpositioned. Recessions were treated using free mucogingival grafts (FMGs) harvested from the buccal aspect of donor teeth with altered passive eruption or healthy periodontal support, with < 3 mm between the cementoenamel junction and the buccal alveolar crest. Lytic brown tumour of the mandible as the initial presentation of hyperparathyroidism in a 7-year old girl: A case report.

INTRODUCTION AND IMPORTANCE: Maxillomandibular involvement with brown tumours is rare, especially in the paediatric population. We present a rare case of a young girl with brown tumour (BT) occurring in the mandible as the first manifestation of hyperparathyroidism. CASE PRESENTATION: A 7-year-old black female, presented with a 6-month history of a swelling on the left mandible. Patient had a history of intractable gastrointestinal symptoms such as nausea, vomiting and abdominal cramps. Biopsy confirmed the lesion as a giant cell lesion. Elevated parathyroid hormone (PTH) levels confirmed the

Relationship between different skeletal facial types and anterior alveolar bone thickness with cone-beam computed tomography in an Asian population.

BACKGROUND: To investigate the relationship between different skeletal facial types and anterior alveolar bone thickness with cone-beam computed tomography (CBCT) in an Asian population. METHODS: A total of 130 patients with 1,560 healthy anterior teeth were enrolled. On three-dimensional reconstructed images, Frankfurt-mandibular plane angle (FMA) value and angle formed by subspinale, nasion, and supramental (ANB) value were measured, and subjects were categorized into different groups based on their vertical skeletal patterns as well as sagittal jaw relationships. For each tooth, the thickn

Lateral pterygoid muscle: a three-dimensional analysis of neuromuscular partitioning.

The lateral pterygoid (LP) has been implicated in temporomandibular joint (TMJ) pathology. Few studies have examined muscle architecture of the superior (SLP) and inferior (ILP) heads of LP; moreover, the pattern of intramuscular innervation is poorly defined. The purpose of this study was to determine patterns of intramuscular innervation of LP using 3D modeling. The superior and lateral aspects of LP were exposed in 10 embalmed cadaveric specimens. Nerves entering the muscle, all branches of the mandibular nerve (V(3) ), were followed intramuscularly in short segments and sequentially digiti

Artificial Intelligence in Occlusion-Oriented Digital Reconstruction of Maxillofacial Fractures: Current Applications and Translational Challenges.

Maxillofacial fractures require reconstruction of a functional craniofacial unit rather than isolated realignment of fractured bone. Stable occlusion, mandibular movement, temporomandibular joint position, facial contour, and fixation-device adaptation should be considered as interdependent treatment targets. Digital workflows incorporating CT or CBCT reconstruction, virtual surgical planning, CAD/CAM, 3-dimensional printing, patient-specific implants or plates, and navigation have improved visualization and surgical transfer. However, comminution, bilateral injury, loss of anatomic references

Extracranial Carotid artery aneurysm Attigah Type I Open Surgery Repair.

BACKGROUND: Extracranial carotid artery aneurysm (ExCAA) is a very rare pathology. However, due to local compression symptoms, risk of CNS ischemia, and the potential risk of rupture, it constitutes a significant clinical problem. The small number of cases, substantial differences in etiology and morphology, and various treatment methods of ExCAA disturb the analysis of this pathology. METHODS: We analyzed patients with ExCAA involving exclusively internal carotid artery (ICA) - morphologically Attigah class I, resulting from dysplasia or atherosclerosis and surgically treated with aneurysm r

Technique for secondary modification after maxillary resection and reconstruction for soft tissue flap fixation before prosthesis addition: a case report.

BACKGROUND: The removal of maxillary carcinoma causes various types of tissue defects, which can be corrected by free flap reconstruction. In flap reconstruction after maxillary cancer resection, ensuring prosthesis stability is frequently difficult owing to the flap's weight. Therefore, a second modification technique is required for improvement of configuration. This case where flap suspension and flap modifying surgery were performed using anchor system for the extensive complete maxillectomy case. CASE PRESENTATION: The patient was a 56-year-old male, who underwent an extensive total maxi

Mandibular extracellular vesicles mediate morphogenesis and mineralization of tooth germs in miniature swine through the miR-206/HDAC4 signaling axis.

BACKGROUND: Reciprocal communication between odontogenic tissues underpins the complexity of tooth morphogenesis. Despite mandible serving as the developmental niche and functional platform for tooth germs, their reciprocal signaling mechanisms remain underexplored. Histone acetylation plays a pivotal role in maintaining long-term regulatory equilibrium and physiological homeostasis by establishing stable gene expression patterns. However, whether stable histone acetylation signatures exist during tooth germ morphogenesis and how they might ensure developmental fidelity remain unreported. MET

A Compromised Maxillofacial Wound Healing Model for Characterization of Particulate Bone Grafting: An In Vivo Study in Rabbits.

Preclinical testing of tissue engineering modalities are commonly performed in a healthy wound bed. These conditions do not represent clinically relevant compromised oral wound environments due to radiation treatments seen clinically. This study aimed to characterize the bone regeneration outcomes in critical-sized mandibular defects using particulate grafting in an irradiated preclinical model of compromised wound healing. Sixteen New Zealand white rabbits were divided into two groups (n = 8/group), namely (i) irradiated (experimental) and (ii) non-irradiated (control). The rabbits in the exp

Dual-Innervated Free Gracilis Muscle Transfer for Facial Reanimation in Children.

BACKGROUND: Facial palsy may have deleterious effects for pediatric patients. The most common reconstruction is 2-stage free gracilis muscle transfer (FGMT) after cross-face nerve graft (CFNG). This requires a prolonged period from time of surgery to smile. New techniques using both a CFNG and motor nerve to masseter (MNM) as dual power sources in a single-stage surgery have been described in adults. Here, we examine our experience with this technique in children. METHODS: A retrospective study was performed examining patients who underwent dual-innervated single-stage FGMT at 2 pediatric hos

The Craniofacial and Upper Limb Management of Nager Syndrome.

INTRODUCTION: Nager syndrome is a rare condition characterized by craniofacial and upper limb abnormalities. It is commonly mistaken for Treacher Collins syndrome, with which it shares the same craniofacial phenotype. However, patients with Treacher Collins do not exhibit hand anomalies, which are seen in patients with Nager syndrome. This paper reviews the multidisciplinary management of patients with Nager syndrome who were treated at the Australian Craniofacial Unit, Adelaide and the Erasmus Medical Centre, Rotterdam. METHODS: The database of both units was scrutinized and the case-notes o
