Link between decisions regarding resuscitation and preferences for quality over length of life with heart failure.

AIMS: Survival preferences, ascertained from time-trade-off utilities, have not been studied in heart failure patients who designate a 'do not resuscitate' (DNR) status. Therefore, the aim of this study was to determine the association of heart failure patients' resuscitation preferences with survival preferences and mortality in the ESCAPE trial. METHODS AND RESULTS: We analysed the association of resuscitation orders at 1 month with time-trade-off utilities and 6-month mortality. There were 26 and 349 patients with a DNR order and Full Code order, respectively. DNR patients were older, had

Missed opportunities for accessing HIV care among Tshwane tuberculosis patients under different models of care.

OBJECTIVE: To compare access to human immunodeficiency virus (HIV) care for tuberculosis (TB) patients in settings with antiretroviral treatment (ART) and TB care under one roof ('semi-integrated sites') and in settings with geographically separately rendered care in Tshwane, South Africa. METHODS: Historical cohort study of patients registered with TB at 46 TB treatment points, with follow-up until the end of anti-tuberculosis treatment. ART initiation for HIV-positive TB patients was established through linkage of TB register patient identifiers to the electronic ART register. Data analysis

Factors related to tumor size of breast cancer at treatment in Taiwan.

METHODS. Personal information and clinical and pathological records were collected for 178 of 310 new cases of breast cancer in 1991 Cancer Registry of Taiwan. RESULTS. Among the subjects studied, 89.3% of the breast cancers were found incidentally. Delayed treatment, doctor shopping, and taking traditional medicine were common. Only 19.7% of the tumors were not more than 2 cm in diameter when the patients received treatment. The most important factors related to tumor size at treatment were occupation, family economic status, and action after detection. Those who were employed, had higher fam

Assessing Secondary Traumatic Stress in Burn Care Clinicians: Feasibility, Prevalence, and Related Factors.

INTRODUCTION: There is growing awareness of the indirect trauma exposure experienced by healthcare providers and the resulting development of secondary traumatic stress (STS). However, STS in burn care clinicians has not been examined thoroughly. The current study aimed to examine feasibility of study procedures in this population, estimate the prevalence of STS, and examine the relationship between STS and additional factors. MATERIAL AND METHODS: Cross-sectional data were collected from 103 burn clinicians from 15 different occupations working in a large civilian burn center in the United S

Toxic leadership behaviours of nurse managers and the co-occurrence of anxiety and depression in nurses: Propensity score matching.

OBJECTIVE: This study investigates the association between nurses' exposure to toxic leadership and the co-occurrence of anxiety and depression, applying both categorical and transdiagnostic analytical frameworks. METHODS: A cross-sectional design was employed among 2337 nurses from 20 tertiary hospitals in China. Toxic leadership was assessed using the Toxic Leadership Behaviours of Nurse Managers scale. Anxiety and depression symptoms were measured with the Generalized Anxiety Disorder-7 and the Patient Health Questionnaire-9. To minimize potential confounding, propensity score matching was

Development of the Attitude Scale Toward Nonpharmacological Methods in Postoperative Pain Management.

PURPOSE: Nurses' attitudes toward nonpharmacological methods are critical to effective postoperative pain management. However, no validated scale specifically assesses nurses' attitudes toward these methods in surgical wards and surgical intensive care units. This study aimed to develop the Attitude Scale Toward Nonpharmacological Methods for Postoperative Pain Management and to evaluate its psychometric properties. DESIGN: An exploratory sequential mixed-methods design was used. METHODS: In the qualitative phase, an item pool was generated based on input from 30 nurses working in surgical s

"I'm not a volunteer; I'm an equal player": the social factor of quality of life in mixed ability basketball players.

INTRODUCTION: Despite regulatory and conceptual progress in the field of inclusion, people with disabilities continue to face significant barriers which limit their full participation in mainstream sports and affect key social factors of quality of life. Within this context, the Mixed Ability (MA) model represents an alternative where people with disabilities can be included into grassroots sports settings without the need to modify rules or implement classification or identification systems. This study aimed to analyse perceived changes in the social factor of MA basketball players, as well a

Associations of Compassion Competence, Nursing Work Environment, Organizational Communication Satisfaction, and Missed Nursing Care on Patient Safety Among Nurses in Comprehensive Nursing Care Service Wards: A Path Analysis.

BACKGROUND/OBJECTIVES: This study aims to identify the factors associated with patient safety for nurses working in comprehensive nursing care service wards, based on the Structure-Process-Outcome model. As comprehensive nursing care service wards expanded in South Korea, the number of older and more severely ill inpatients increased, raising the demand for nursing care. In these wards, nurses play a crucial role in ensuring patient safety. Therefore, a thorough identification of the factors associated with patient safety, based on theoretical frameworks, is necessary. METHODS: A cross-sectio

Qualitative Analysis of Barriers and Facilitators to Tobacco Treatment Engagement Among Hispanic Patients with Cancer in New York City.

BACKGROUND: Hispanic patients with cancer who smoke are less likely to quit after diagnosis and underutilize evidence-based tobacco treatment. Understanding the complex factors influencing their engagement is crucial for developing effective and tailored interventions. METHODS: Semi-structured individual interviews were conducted with 12 adult patients with cancer who self-identified as Hispanic or Latino referred between June 1, 2023, and June 30, 2024, to a tobacco treatment program embedded within a cancer center. Interviews were conducted via phone or video in English or Spanish. Transcrip

Factors influencing the strengthening of nursing regulatory systems: A comparative study of Lao PDR and Mongolia.

Access to high-quality healthcare requires a qualified health workforce supported by a robust regulatory framework, an overarching legal structure governing regulation of health professionals. Its effectiveness depends on how well its regulatory functions are carried out. Implementing these functions requires regulatory systems that provide organizational arrangements for each health profession. This study focuses on the nursing regulatory systems in Lao PDR and Mongolia to identify key factors influencing their strengthening and extract policy insights to inform efforts to strengthen nursing

Closing the quality gap: analysis of implementation barriers and interventions for postoperative exercise rehabilitation for patients with radiofrequency ablation for atrial fibrillation.

BACKGROUND: Atrial fibrillation patients often experience issues such as recurrence and reduced exercise tolerance following radiofrequency catheter ablation, which adversely affect postoperative recovery and long-term prognosis. Although current guidelines recommend exercise-based rehabilitation for these patients, its implementation in clinical practice remains suboptimal. OBJECTIVE: This study aims to summarize the best evidence regarding post-ablation exercise rehabilitation for atrial fibrillation, establish structured quality evaluation metrics, and identify barriers in clinical practic

Biomolecular Condensates Integrate Transcriptional and Epigenetic Responses to Hypoxia.

Hypoxia is a defining feature of physiological stress and the core of solid tumors, where aberrant vascularization limits oxygen delivery; cells respond through mechanisms that extend beyond the canonical stabilization of hypoxia-inducible factors (HIFs). Recent studies suggest that hypoxia can promote the formation of specific biomolecular conden-sates, membraneless compartments generated through liquid-liquid phase separation in which regulatory proteins and RNAs become locally enriched at genomic regions, while chromatin mainly serves as an organizational scaffold.
