Peak alpha frequency and spectral exponent in postoperative delirium: a high-density EEG cohort study.

BACKGROUND: Present concepts around the neurophysiology of delirium suggest impairments in excitatory/inhibitory balance, leading to EEG slowing, altered synaptic activity and integration of information. We aim to clarify the mechanisms involved, hypothesising that, in postoperative delirium, the EEG peak alpha frequency (PAF; decreased peak frequency indicating slowing in alpha frequency) would slow and the spectral exponent (SE; a higher slope indicating an increased neural inhibitory tone) would be steeper compared with a baseline collected before surgery. The primary aim was to determine t

Correlation of final evoked potential amplitudes on intraoperative electromyography of the recurrent laryngeal nerve with immediate postoperative vocal fold function after thyroid and parathyroid surgery.

IMPORTANCE: Thyroid and parathyroid surgery are among the most common operations in the United States. Recurrent laryngeal nerve (RLN) injury is an infrequent but potentially detrimental complication. OBJECTIVE: To correlate the final evoked potential amplitudes on intraoperative electromyography (EMG) after stimulation of the RLN with immediate postoperative vocal fold function after thyroid and parathyroid surgery. DESIGN, SETTING, AND PARTICIPANTS: Retrospective observational study at a tertiary academic medical center. We included 674 patients (with 1000 nerves at risk) undergoing thyroi

Analysis of sleep disorders under pain using an optogenetic tool: possible involvement of the activation of dorsal raphe nucleus-serotonergic neurons.

BACKGROUND: Several etiological reports have shown that chronic pain significantly interferes with sleep. Inadequate sleep due to chronic pain may contribute to the stressful negative consequences of living with pain. However, the neurophysiological mechanism by which chronic pain affects sleep-arousal patterns is as yet unknown. Although serotonin (5-HT) was proposed to be responsible for sleep regulation, whether the activity of 5-HTergic neurons in the dorsal raphe nucleus (DRN) is affected by chronic pain has been studied only infrequently. On the other hand, the recent development of opto

Symptomatic epilepsy in children with poroencephalic cysts secondary to perinatal middle cerebral artery occlusion.

BACKGROUND: Perinatal cerebral artery occlusion is responsible for ischemic cerebral infarction leading to brain cavitation and gliosis; the territory of the middle cerebral artery is most frequently involved. The resulting poroencephalic cysts are frequently associated with hemiplegia and epilepsy; that can be managed medically in most cases, only 6-7% of them being refractory to medical treatment. This particular subset of congenitally hemiplegic children will be possible candidates for electrophysiological investigation and eventually for resective surgery. Whatever the kind of surgical tre

Artificial intelligence in headache medicine: between automation and the doctor-patient relationship. A systematic review.

BACKGROUND: Headache disorders, particularly migraine, are highly prevalent, but often remain underdiagnosed and undertreated. Artificial intelligence (AI) offers promising applications in diagnosis, prediction of attacks, analysis of neuroimaging and neurophysiology data, and treatment selection. Its use in headache medicine raises ethical, regulatory, and clinical questions, including its impact on the doctor-patient relationship. METHODS: A systematic literature search was conducted on April 10, 2025, across PubMed, Cochrane Library, Scopus, Web of Science, and DOAJ, following PRISMA guide

Neurophysiological signals as predictive translational biomarkers for Alzheimer's disease treatment: effects of donepezil on neuronal network oscillations in TgF344-AD rats.

BACKGROUND: Translational research in Alzheimer's disease (AD) pathology provides evidence that accumulation of amyloid-β and hyperphosphorylated tau, neuropathological hallmarks of AD, is associated with complex disturbances in synaptic and neuronal function leading to oscillatory abnormalities in the neuronal networks that support memory and cognition. Accordingly, our recent study on transgenic TgF344-AD rats modeling AD showed an age-dependent reduction of stimulation-induced oscillations in the hippocampus, and disrupted long-range connectivity together with enhanced neuronal excitability

Restoring walking ability in older adults with arm-in-arm gait training: study protocol for the AAGaTT randomized controlled trial.

CONTEXT: Falls are a significant problem among older adults. While balance and functional exercises have been shown to be effective, it remains unclear whether regular walking has specific effects on reducing the risk of falls. RATIONALE: Older people who fall frequently have impaired gait patterns. Recent studies have suggested using interpersonal synchronization: while walking arm-in-arm, an older person synchronizes steps with a younger person to reinstate a better gait pattern. This method of gait training may reduce the risk of falls. OBJECTIVE: The aim is to assess the efficacy of an a

Cervical vestibular-evoked myogenic potential in vestibular schwannoma after gamma-knife surgery.

OBJECTIVE: Gamma-knife radiosurgery (GKS) for vestibular schwannomas (VSs) has become popular during the last two decades, and a promising tumor control rate has been reported. Therefore, the evaluation and preservation of auditory-vestibular nerve function after GKS have become more and more important in these patients with long-term survival. We have traditionally used pure-tone audiometry (PTA) for evaluation of auditory nerve function, and the caloric test for superior vestibular nerve function. Vestibular-evoked myogenic potential (VEMP) has recently emerged from various neurophysiologica

Near-infrared spectroscopy (NIRS): a non-invasive in vivo methodology for analysis of brain vascular and metabolic activities in real time in rodents.

Near infrared spectroscopy (NIRS) was first used as a tool for the in vivo monitoring of tissue oxygenation in the late seventies. Today, NIRS instruments are more and more used in clinical environments since they are now easy to use, sensitive, robust, provide rapid analysis and could be complementary to other non invasive methodologies such as functional magnetic resonance imaging (fMRI) and positron emission tomography (PET). The feasibility of non-invasive analysis of brain activities is studied in the attempt to overcome the major limitation of invasive in vivo methodologies. In the prese

Investigating the Relationship Between Voice Characteristics and Presbyphagia in Geriatric Population.

PURPOSE: Aging is a significant risk factor for dysphagia. Speech and swallowing share the same anatomical and neurophysiological mechanisms and are closely related functions. Therefore, speech assessment can serve as a valuable tool for predicting the risk of dysphagia. Presbyphagia is a term that describes the impact of aging on swallowing function. This study aims to investigate the relationship between acoustic and aerodynamic parameters of voice and speech and presbyphagia in the normal geriatric population. METHODS: A total of 96 individuals aged 65 and above participated in the study.

[Female sexual dysfunction: a systematic overview of classification, pathophysiology, diagnosis and treatment].

Sexual dysfunction is defined as "disturbances in sexual desire and in the psychophysiological changes that characterize the sexual response cycle and cause marked distress and interpersonal difficulty". The female sexual response cycle consists of three phases: desire, arousal, and orgasm. Various organs of the external and internal genitalia, e.g. vagina, clitoris, labia minora, vestibular bulbs, pelvic floor muscles and uterus, contribute to female sexual function. During sexual arousal, genital blood flow and sensation are increased. The vaginal canal is moistened (lubrication). During org
