PURPOSE: To compare the torsional stability of 6 fixation techniques for intercarpal fixation including a 4-tine, in-line staple.
METHODS: We chose the scapholunate interval as the test site for rotational stability for various constructs. We used 42 pairs of embalmed scaphoids and lunates for the comparisons and created 6 construct groups. Fixation of the scapholunate gap was performed with a 3-mm screw, 2 1.1-mm (0.045-inch) K-wires spaced 2 mm apart, or 2 1.1-mm K-wires spaced 5 mm apart. One member of each pair was fixed with either a standard 2-tine peripherally placed staple or a custom

PURPOSE: Scaphoid excision with 4-corner arthrodesis is a safe procedure that relieves pain while also preserving the functional movements of the wrist. Different techniques have been described with successful results. In this study, we present the midterm clinical outcomes of 17 patients who underwent treatment with our arthroscopic 4-corner arthrodesis technique.
METHODS: Patients with at least 1 year of follow-up were included in our study. Preoperative and postoperative radiologic, functional, and clinical results were compared. Radiologic and clinical union were achieved in all patients.

BACKGROUND: Computed tomography (CT) has been used to understand the deformity of scaphoid nonunion, but no standard protocol for the reformatting of scaphoid CT imaging exists. The purpose of this study was to compare the reliability of measurements of the deformity of scaphoid waist nonunion between CT-scans reformatted in line with the scaphoid long axis and CT-scans reformatted in line with the wrist axis.
HYPOTHESIS: We hypothesized that CT-scan which was reformatted along the scaphoid long axis is more reliable for understanding the deformity of scaphoid waist nonunion.

The purpose of this study was to establish a normal measure of scaphoid position in the radioulnar plane in standard neutral, radial and ulnar deviation posteroanterior radiographs. This measurement may allow indirect evaluation of the radiocarpal ligaments and comparison between normal and pathologic states (following radius fractures, perilunate dislocations). Measurements were trialed on 74 normal wrist radiographs and 25 cadaver wrists. We evaluated the distance between the radial styloid and the scaphoid and corresponding scaphoid width. The ratio of distance/width at the mid styloid leve

The idiopathic avascular necrosis of the scaphoid or Preiser's disease is condition about which few cases have been described. In the majority of cases, it is debatable whether it is actually a spontaneous osteonecrosis, or a pathological process secondary to a systematic disease, or a result of chemotherapy treatment, or associated with taking steroids. We present the case of a 20-year old woman, with no previous trauma, with pain in her right wrist, which progressed over a period of four years. It was wrongly diagnosed as a tendinitis, and was treated conservatively without any improvement.

It is my belief that all acute proximal pole fractures should be treated by open reduction and internal fixation, via a dorsal approach. There is no longer any place for conservative treatment of these fractures, because a lengthy period of plaster immobilization is required and there is an unacceptably high risk of nonunion with conservative management. Unfortunately, nonunion of the proximal pole remains a common and disabling problem which demands careful evaluation and treatment. Internal fixation combined with limited cancellous bone grafting produces very satisfactory results in terms of

Background Three-dimensional (3D) printing has developed as a revolutionary technology with the capacity to design accurate physical models in preoperative planning. We present our experience in surgeon-based design of 3D models, using home 3D software and printing technology for use as an adjunct in vascularized bone transfer. Methods Home 3D printing techniques were used in the design and execution of vascularized bone flap transfers to the upper extremity. Open source imaging software was used to convert preoperative computed tomography scans and create 3D models. These were printed in the

A fracture that does not unite in nine months is defined as nonunion. Nonunion is common in fragmented fractures and large bone defects where vascularization is impaired. The distal third of the tibia, the scaphoid bone or the talus fractures are furthermore prone to nonunion. Open fractures and spinal fusion cases also need special monitoring for healing. Bone tissue regeneration can be attained by autografts, allografts, xenografts and synthetic materials, however their limited availability and the increased surgical time as well as the donor site morbidity of autograft use, and lower probab…

BACKGROUND: Scaphoid fracture fixation using a single headless compression screw (HCS) may permit unacceptable rotation at the fracture site. This study aimed to assess the biomechanical stability of a single HCS construct to fix scaphoid waist fractures.
QUESTIONS/PURPOSES: (1) Does a single HCS provide rotational stability? (2) What degree of rotation is found at the central axis of a scaphoid without fixation?
METHODS: In eight fresh frozen cadavers, two parallel K-wires were placed in the scaphoid to mark rotation and an osteotomy was created at the scaphoid waist. To determine whether r

PURPOSE: To study the angle of screw placement in relation to the scaphoid fracture plane and its effect on union after percutaneous fixation of scaphoid waist fractures.
METHODS: Twenty-four consecutive scaphoid waist fractures were retrospectively evaluated for the orientation of screws in relation to the fracture plane using a method in which the sum-of-smaller angles (SSA) in 3 different radiographs were used to correlate with time to fracture union.
RESULTS: All but one patient achieved union after percutaneous fixation of the scaphoid. Another patient required revision surgery within t

PURPOSE: To determine whether a screw placed perpendicular to the fracture line in an oblique scaphoid fracture will provide fixation strength that is comparable with that of a centrally placed screw.
METHODS: Oblique osteotomies were made along the dorsal sulcus of 8 matched pairs of cadaveric scaphoids. One scaphoid from each pair was randomized to receive a screw placed centrally down the long axis. In the other scaphoid, a screw was placed perpendicular to the osteotomy. Each scaphoid underwent cyclic loading from 80 N to 120 N at 1 Hz. Cyclic loading was carried out until 2 mm of fractur

PURPOSE: To investigate computer-assisted surgery (CAS) for dorsal insertion of percutaneous scaphoid screws and to compare operative time, accuracy, and fluoroscopy time with the traditional mini C-arm method. We hypothesized that CAS techniques would improve accuracy, reduce actual K-wire insertion time, and decrease fluoroscopy time.
METHODS: Ten fresh cadaveric upper limbs were randomized to either CAS or traditional dorsal percutaneous scaphoid screw placement by a single surgeon. Custom orthoses were applied to the CAS arms followed by intraoperative computed tomography (CT) scan and na

UNLABELLED: Trans-scaphoid perilunate fracture-dislocation is the most common variety of fracture-dislocations of the wrist. Because of the carpal scaphoid fracture association, post-reduction stability is poor, often requiring surgery. Of the several surgical techniques used over time, percutaneous screw fixation for scaphoid fracture is currently preferred.
MATERIAL AND METHODS: Six cases of trans-scaphoid perilunate fracture-dislocation were surgically treated by percutaneous reduction of the scaphoid fracture and percutaneous insertion of Herbert screw in a proximal to distal manner. Post

BACKGROUND: : The fluoroscopic imaging technique that has been used in the dorsal percutaneous screw fixation of scaphoid fracture was modified for more consistent central screw placement according to the different fracture locations.
METHODS: : Eighteen scaphoids of the fresh-frozen cadaver were used. The computed tomographic data of scaphoid were reconstructed for 3D images. The reconstructed image was three dimensionally rotated to define the outlining images of scaphoid according to the three imaginary fracture lines (proximal 1/3, waist, distal 1/3). PURPOSE: Volar percutaneous screw fixation (PSF) of acute nondisplaced scaphoid waist fractures allows early mobilisation of the wrist and a faster return to work than prolonged cast immobilisation. Usually, placement of the wire which guides the definitive canulated screw is performed by hand. Nevertheless, correct placement of this wire is technically difficult. We designed a guidewire insertion device (GID) to facilitate this placement.
METHODS: We compared the hand held technique with the technique using the GID in a cadaveric study. The hand held technique was performed on 16 scaphoids a

We report a case of delayed post-traumatic volar midcarpal dislocation in a 39 year-old woman. The dislocation was a gradual process starting from the time of injury. Initial X-rays showed a normal midcarpal joint. By 6 weeks, lunocapitate subluxation was apparent radiologically and by the 18 week X-ray, midcarpal dislocation had occurred. This type of wrist injury has not been previously reported. Because of advanced degenerative changes in the lunocapitate joint, a partial wrist fusion was performed with a successful outcome.

PURPOSE: Extrinsic ligament injury is necessary for scapholunate (SL) diastasis to occur in SL ligament injury. Prior studies demonstrate an association between dorsal extrinsic ligament injury and SL diastasis, but it is unclear what is the importance of volar extrinsic injury. This study aimed to characterize injury to the dorsal intercarpal ligament (DIC), dorsal radicarpal ligament (DRC), volar extrinsic ligaments (LRL/SRL/RSC), and scaphotrapezotrapezial (STT) ligaments to determine what extrinsic ligament injuries are independently associated with SL diastasis.

ABSTRACT: Scapholunate ligament injuries are the most common ligamentous injuries of the wrist and typically result from high-energy trauma, most often following a fall onto the outstretched hand. This comprehensive review summarizes the current knowledge regarding scapholunate ligament injuries, including anatomy, pathophysiology, diagnostic approaches, and management strategies. Scapholunate ligament injuries primarily affect individuals aged 20-50 years, with male patients at higher risk due to participation in high-impact activities. Despite their prevalence, these injuries are challenging

Assessment of scaphoid fracture union on computed tomography scans is not currently standardized. We investigated the extent of scaphoid waist fracture union required to withstand physiological loads in a finite element model, based on a high-resolution CT scan of a cadaveric forearm. For simulations, the scaphoid waist was partially fused at the radial and ulnar sides. A physiological load of 100 N was transmitted to the scaphoid and the minimal amount of union to maintain biomechanical stability was recorded. The orientation of the fracture plane was varied to analyse the effect on biomechan
