Iatrogenic suprascapular neuropathy secondary to drilling for superior labrum anterior-posterior repair: a case report.

Suprascapular neuropathy is an uncommon complication following arthroscopic superior labrum anterior-posterior (SLAP) repair. The authors describe a patient who developed nerve entrapment at the spinoglenoid notch after surgery. Postoperatively, the patient reported persistent pain about the anterolateral aspect of the shoulder, with weakness affecting the superior and posterior rotator cuff muscles. Imaging suggested possible penetration of the posterior glenoid rim yet repeat arthroscopy revealed the nerve surrounded by a dense fibrous tissue. The authors propose that drill penetration durin

High Sensitivity and Specificity of Magnetic Resonance Arthrography for Labral Tears, Rotator Cuff Tears, Hill-Sachs Lesions, and Bankart Lesions: A Systematic Review and Meta-analysis.

PURPOSE: To compare the diagnostic value of magnetic resonance arthrography (MRA) in different shoulder lesions using arthroscopy as gold standard. METHODS: We performed a comprehensive search in Cochrane, Scopus, PubMed, and Web of Science databases for articles that reported the diagnostic value of MRA in diagnosing labral tears, rotator cuff tears (RCTs), Hill-Sachs, and Bankart injuries. We used arthroscopic surgery as a reference standard for comparison. The quality of the studies was evaluated using the Quality Assessment of Diagnostic Accuracy Studies tool. We pooled the collected data

The Burden of Craft in Arthroscopic Rotator Cuff Repair: Where Have We Been and Where We Are Going.

The rather turbulent history of arthroscopic rotator cuff repair went through stages of innovation, conflict, disruption, assimilation, and transformation that might be anticipated when a new and advanced technology (arthroscopic cuff repair) displaces an entrenched but outdated discipline (open cuff repair). The transition from open to arthroscopic rotator cuff repair has been a major paradigm shift that has greatly benefited patients. However, this technical evolution/revolution has also imposed a higher "burden of craft" on the practitioners of arthroscopic rotator cuff repair. Technologica

Surgical treatment of symptomatic superior labrum anterior-posterior tears in patients older than 40 years: a systematic review.

BACKGROUND: Successful arthroscopic repair of symptomatic superior labral tears in young athletes has been well documented. Superior labral repair in patients older than 40 years is controversial, with concerns for residual postoperative pain, stiffness, and higher rates of revision surgery. PURPOSE: To analyze the published data on the surgical treatment of superior labral injuries in patients aged ≥40 years, including those with concomitant injuries to the rotator cuff. STUDY DESIGN: Systematic review. METHODS: A systematic review of the literature was performed using the Preferred Report

Axillary nerve injury in young adults--an overlooked diagnosis? Early results of nerve reconstruction and nerve transfers.

An injury to the axillary nerve from a shoulder trauma can easily be overlooked. Spontaneous functional recovery may occur, but occasionally reconstructive surgery is required. The time frame for nerve reconstruction procedures is from a neurobiological view crucial for a good functional outcome. This study presents a group of operatively and non-operatively treated young adults with axillary nerve injuries caused by motorcycle accidents, where the diagnosis was set late. Ten young men (median age at trauma 13 years, range 9-24) with an axillary nerve injury were diagnosed by examination of sh

Infrared imaging of the anterior deltoid during overhead static exertions.

Infrared imaging has been used to detect the presence of neuromuscular disorders of the cervical spine and upper extremities. Despite diagnostic uses, evaluative or prognostic uses of thermography are limited. The objective of this study was to quantify surface temperature changes over the anterior deltoid and evaluate efficacy of thermography as an assessment tool. Surface temperature, discomfort ratings and endurance time were quantified during overhead static exertions until exhaustion at two work loads (15 and 30% maximum voluntary contraction) and shoulder angles (90 degrees and 115 degre

Rupture of the pectoralis major: a meta-analysis of 112 cases.

Of about 150 cases reported in the literature on pectoralis major ruptures, 108 were selected as presenting enough data to be analyzed for cause, rupture site, injury mechanism, and treatment outcome. We added data on four of our own cases reported here. All patients yet reported have been men. Rupture of the PM occurs most commonly in sports during weight training, weight lifting, or wrestling when the arm is externally rotated and abducted. Most reported ruptures are complete and are located at the insertion to the humerus. Work-related injuries occur more often at the musculo-tendinous junc

Arthroscopic shoulder stabilization. Overview of indications, technique, and efficacy.

The importance of patient selection for arthroscopic shoulder stabilization cannot be overemphasized. A significant learning curve involving both proper patient selection and correct surgical technique exists for arthroscopic Bankart repair using the Suretac device. Nevertheless, with appropriate selection criteria, knowledge of the anatomic limitations of the procedure and mastering of the operative technique, one can expect excellent outcomes with results approaching those found with open capsulorrhaphy. In addition, patients can anticipate a reliable return of motion that is nearly symmetri

Risk of suprascapular nerve injury in open Trillat procedure: an anatomical study.

PURPOSE: The open Trillat Procedure described to treat recurrent shoulder instability, has a renewed interest with the advent of arthroscopy. The suprascapular nerve (SSN) is theoretically at risk during the drilling of the scapula near the spinoglenoid notch. The purpose of this study was to assess the relationship between the screw securing the coracoid transfer and the SSN during open Trillat Procedure and define a safe zone for the SSN. METHODS: In this anatomical study, an open Trillat Procedure was performed on ten shoulders specimens. The coracoid was fixed by a screw after partial ost

Methods to analyse the long head of the biceps in the management of distal ruptures of the supraspinatus tendon. Part 1: the concept of the "biceps box": dynamic rotator interval approach. Incidence of lesions of the long head of the biceps tendon.

INTRODUCTION: The area encompassing the long head of the biceps (LHB) can be represented as a rectangular parallelepiped. This geometric view can be likened to a box, the "biceps box", where the sides are the extrinsic structures and the LHB is the intrinsic structure. Since these structures are mobile in relation to each other, a dynamic "biceps box" model can modify assessments of the LHB, in its healthy or pathological state, and make the therapeutic approach to treating LHB lesions less arbitrary. MATERIAL AND METHOD: In order to describe the different sides of the "biceps box", and to un

Deep learning classification of shoulder fractures on plain radiographs of the humerus, scapula and clavicle.

In this study, we present a deep learning model for fracture classification on shoulder radiographs using a convolutional neural network (CNN). The primary aim was to evaluate the classification performance of the CNN for proximal humeral fractures (PHF) based on the AO/OTA classification system. Secondary objectives included evaluating the model's performance for diaphyseal humerus, clavicle, and scapula fractures. The training dataset consisted of 6,172 examinations, including 2-7 radiographs per examination. The overall area under the curve (AUC) for fracture classification was 0.89, indica

Terminology and diagnostic criteria used in studies investigating patients with subacromial pain syndrome from 1972 to 2019: a scoping review.

INTRODUCTION: There is no recognised terminology, nor diagnostic criteria, for patients with subacromial pain syndrome (SAPS). This is likely to cause heterogeneity across patient populations. This could be a driver of misconceptions and misinterpretations of scientific results. We aimed to map the literature regarding terminology and diagnostic criteria used in studies investigating SAPS. MATERIALS AND METHODS: Electronic databases were searched from inception to June 2020. Original peer-reviewed studies investigating SAPS (also known as subacromial impingement or rotator cuff tendinopathy/i

Deep learning for automated measurement of CSA related acromion morphological parameters on anteroposterior radiographs.

BACKGROUND: The Critical Shoulder Angle Related Acromion Morphological Parameter (CSA- RAMP) is a valuable tool in the analyzing the etiology of the rotator cuff tears (RCTs). However, its clinical application has been limited by the time-consuming and prone to inter- and intra-user variability of the measurement process. OBJECTIVES: To develop and validate a deep learning algorithm for fully automated assessment of shoulder anteroposterior radiographs associated with RCTs and calculation of CSA-RAMP. METHODS: Retrospective analysis was conducted on radiographs obtained from computed tomogra

Editorial Commentary: Determination of Meaningful, Clinically Significant Outcome Thresholds for Superior Capsular Reconstruction of the Shoulder: Predicting Those Patients Who Improve and Those Who Don't!

Since the introduction of superior capsular reconstruction (SCR) for the treatment of irreparable rotator cuff tears nearly 10 years ago, we have witnessed widespread adoption by the arthroscopic shoulder community alongside an exponential increase in use within North America. To date, more than 42,000 acellular dermal allografts have been implanted in the United States alone, and our indications and surgical technique have been progressively refined in response to the evolving body of clinical outcomes research. However, with P values set aside, the real question still remains: how do our pat

Practicability of a Virtual Consultation to Evaluate the Shoulder Joint.

OBJECTIVE: In the course of the corona pandemic, resource conservation and the protection of further infections have made it necessary to break new ground in the organisation of orthopaedic and trauma surgery consultations. One solution is consistent digitisation and the offer of video consultation hours. In this study, non-contact examination of patients with shoulder disorders is described and critically examined. METHODS: Thirty patients who presented with pathologies of the shoulder joint in a university outpatient clinic were subjected to a physical examination in a conventional and cont

Global Excision of Severe Heterotopic Ossification of the Shoulder: A Case Report.

CASE: A 21-year-old man sustained a closed glenohumeral fracture/dislocation as a pedestrian struck by a motor vehicle. He was treated nonoperatively and developed severe post-traumatic heterotopic ossification (HO) with near-complete shoulder ankylosis. We present our technique for safe surgical excision. CONCLUSIONS: Excision led to improvements in motion and quality of life at 1 year postoperatively. Recommendations for successful HO excision around the shoulder include excision after at least 180 days, appropriate preoperative imaging to include cross-sectional imaging and a 3D model, int
