Management of tuberculomas of the craniovertebral junction.

Tuberculosis of the craniovertebral junction is extremely rare. However, recent evidence suggests that the incidence of this condition may be increasing in the United Kingdom. The diagnosis is often difficult despite advances in imaging using MRI. CT guided biopsy of lesions often yields inconclusive results. The transoral approach to the anterior craniovertebral junction provides excellent access to this region with a low operative morbidity and mortality, enabling biopsy of lesions and decompression of the neuraxis. Management of secondary atlantoaxial instability, regarding both timing and

Vertebral infection after kyphoplasty: distinguishing misdiagnosis from true postoperative infection and implications for management.

BACKGROUND: Spinal infection following percutaneous kyphoplasty (PKP) is rare but clinically devastating. However, its true incidence, diagnostic challenges, and optimal revision strategies remain poorly defined, particularly in referral-based cohorts. METHODS: We conducted a retrospective study comprising two components: (1) an institutional incidence analysis of 6,328 PKP procedures performed between 2015 and 2025, and (2) a pooled clinical cohort of 39 patients treated for post-PKP spinal infection, including 3 hospital-acquired cases and 36 referred cases. Patients were managed with eithe

Application of Metagenomic Next-Generation Sequencing in Suspected Spinal Infectious Diseases.

OBJECTIVE: This study aimed to explore the clinical efficacy of metagenomic next-generation sequencing (mNGS) in diagnosing and treating suspected spinal infectious diseases. METHODS: Between October 2022 to December 2023, a retrospective analysis was performed on patient records within the Department of Spinal Surgery at Guilin People's Hospital. The analysis included comprehensive data on patients with presumed spinal infectious diseases, incorporating results from mNGS tests conducted externally, conventional pathogen detection results, laboratory examination results, and imaging findings.

Posterior-only debridement, bone fusion, single-segment versus short-segment instrumentation for mono-segmental lumbar or lumbosacral pyogenic vertebral osteomyelitis: minimum five year follow-up outcomes.

BACKGROUND: Pyogenic vertebral osteomyelitis (PVO), which is a potentially life-threatening condition and is associated with significant morbidity and mortality, is a cause of back pain that can lead to neurologic deficits if not diagnosed in time and effectively treated. The objective of this study is to compare the efficacy of posterior single-segment and short-segment fixation combined with one-stage posterior debridement and fusion for the treatment of mono-segmental lumbar or lumbosacral PVO. METHODS: Charts of all patients with mono-segmental lumbar or lumbosacral PVO were treated by si

Tuberculosis of the cervical spine.

Tuberculosis of the cervical spine is a rare extra-pulmonary localization. We reported the case of a 59-year-old woman with no medical history. She consulted for a 9 weeks history of painful cervical stiffness. Neurological examination was normal. Plain radiography revealed an osteolytic lesion of the C3 body, with infiltration of adjacent soft tissues. MRI showed a compression fracture of the C3 body with a signal anomaly extending to the anterior epidural space and pre-vertebral soft tissues. First, tumoral extension assessment was done and was normal. BK's research into sputum was negative.

Direct internal kyphectomy for severe angular tuberculous kyphosis.

We describe a direct internal kyphectomy through a modified costotransversectomy, an extrapleural approach to the kyphus that does not jeopardize already compromised pulmonary function. A curved longitudinal incision is made 6 to 8 cm lateral to the midline. The posterior 5 cm of the two to three crowded ribs at the apex are resected. The segmental intercostal nerves are preserved as a guide into the spinal canal. Two to three pedicles at the apex are resected. The pleura are elevated with blunt dissection leading to the internal kyphus. Removal of the posterior half of the collapsed vertebrae

[Anterior thoracolumbar K-plate fixation system: design and clinical application].

OBJECTIVES: To develop an anterior thoracolumbar K-plate fixation system and to report its preliminary clinical application. METHODS: The K-plate system was developed with titanium alloy(TC4, Ti6A14V) and self-locking screw design was used. In 23 cases treated with this system, 13 had thoracolumbar fracture, 9 tuberculosis, and back syndrome after lumbar TFC operation. RESULTS: All the 23 cases were followed up for one to ten months, averaging 4.5 months. In the 13 cases of fracture, body height of injured vertebra were regained completely, and neurologic deficits improved 2.1 grades in Fran

[Pyogenic and non-pyogenic vertebral osteomyelitis: descriptive and comparative study of a series of 40 cases].

BACKGROUND: To describe the features of vertebral osteomyelitis (VO) in our area, and to compare pyogenic VO (PVO) and non-pyogenic VO (NPVO). METHODS: Retrospective study of patients with VO diagnosed in our hospital between january 1992 and december 1998. RESULTS: We identify 40 patients with VO: 17 (42.5%) with tuberculous VO, 12 (30%) with PVO, 10 (25%) with brucellar VO and 1 (2.5%) with a VO caused by Candida albicans. The mean age was 52.9 years, and 75% of patients were male. Only a 20% of cases had a known immunosuppression. Fever/febricule was present in 55% of patients and vertebr

Spinal epidural abscess complicating tuberculous spondylitis.

We report the case of a patient with tuberculous L1-L2 spondylo-discitis complicated by a spinal epidural abscess which extended anteriorly to the cord up to the low cervical level. Mild signs and symptoms of spinal cord involvement improved with antituberculous therapy; however, after seven months of therapy, the MRI appearance of the abscess findings was unchanged. An attempt at surgical decompression and drainage of the abscess was unsuccessful because of the presence of dense scar tissue.

Intradural extramedullary tuberculous spinal granulomas.

Four cases of intradural extramedullary tuberculous spinal granulomas without bony involvement are presented. Both, the rarity of the disease, as well as the successful microsurgical resection with good recovery prompted this report. The pathogenesis with the controversies therein, is discussed and pertinent literature is reviewed.

CT-Based Clinical-Radiomics for Differentiating Talaromyces marneffei Spondylitis From Tuberculous Spondylitis Before Pathogen Confirmation: A Multicentre Diagnostic Study in an Endemic Region.

BACKGROUND: Talaromyces marneffei spondylitis (TMS) is an uncommon but clinically important form of talaromycosis in endemic regions. Where talaromycosis and tuberculosis overlap, TMS may be worked up as tuberculous spondylitis (TBS), delaying fungal investigations and antifungal treatment. OBJECTIVES: To assess whether pretreatment CT, routine clinical variables and radiomics features can help differentiate TMS from TBS at the initial assessment stage. PATIENTS/METHODS: This retrospective multicentre study included 263 patients meeting the prespecified composite reference standard (83 TMS a
