STUDY OBJECTIVES: A novel method for acoustic imaging of the human respiratory system is proposed and evaluated.
DESIGN: The proposed imaging system uses simultaneous multisensor recordings of thoracic sounds from the chest wall, and digital, computer-based postprocessing. Computer simulations and recordings from a life-size gelatin model of the human thorax are used to evaluate the system in vitro. Spatial representations of thoracic sounds from 8-microphone and 16-microphone recordings from five subjects (four healthy male adults and one child with lung consolidation) are used to evaluate t

OBJECTIVES: To measure the incidence of postoperative headaches after retrosigmoid resections of acoustic neuromas and to evaluate the impact of cranioplasty on the prevention and management of these headaches.
STUDY DESIGN: A prospective evaluation was performed on 30 consecutive patients who underwent a cranioplasty after retrosigmoid excision of their acoustic neuroma. The results were compared with 30 historical control patients who underwent the same procedure but did not have reconstruction with a cranioplasty. The patients were evaluated by review of office records and via telephone qu

Generally the main objective of acoustic neurinoma (AN) surgery is to totally remove the tumor associated with minimum morbidity. Nevertheless, in some cases residual tumor fragments are intentionally or accidentally left in place. These residues can lead to new growth. The present study provides a retrospective analysis of 14 cases of residual AN have undergone surgery at the Otology Group in Piacenza from 1987 to 1999. All these patients had previously undergone at least one retrosigmoidal exeresis although only one had been performed at the Otology Group. All patients except 1 were affected

A total of 311 patients with a unilateral acoustic neurinoma were operated on via the enlarged middle cranial fossa approach. A total tumor removal was achieved in 98% of cases. The mortality was 0.6%. Overall in acoustic neurinoma surgery, the percentage rates of meningitis (1.6%), cerebrospinal fluid fistula (1.6% requiring surgery), and neurological deficits were fairly low. The facial nerve could be preserved anatomically in 99%. A House I or House II classification was demonstrated in 91% of all tumors and in 98% of small- and medium-sized tumors. Preservation of the hearing function was

Between 1991 and 1996, 35 patients had an intracanalicular tumor: 26 patients were operated via middle cranial fossa approach (MFA: 23 cases) or translabyrinthine approach (TL: 3 cases), 9 cases were supervised by audiovestibular testing and MRI. For the operated patients through MFA the functional hearing (pure tone loss < 50 dB and 100% vocal discrimination) was preserved in 47.8% of cases, facial function was grade I and II in 83% of cases and grade III in 17% of cases. One recurrence was noted and reoperated by TL. For the TL operated patients, the postoperative facial function was normal

We report the case of a 52-year-old woman with a cerebellopontine angle tumor, which appeared to have arisen from the 8th nerve. Microscopically the tumor was proved to be an acoustic neurinoma and showed unusual findings such as inclusions of mature bone and bone marrow. The histogenesis and diagnostic relevance of the very rare heterotopic osteogenesis in acoustic neurinomas is discussed.

In articles and chapters on the subject of acoustic neuroma, it is almost invariably stated that they are well-encapsulated tumors. During surgical procedures, blunt mechanical dissection defines a natural subsurface cleavage plane that leaves intact a several millimeter thick rind of tumor surface. Occasionally, as a concession to neural integrity, less than complete resection is elected, leaving behind this "capsular" remnant. To clarify the nature of the surface of acoustic neuromas and to test whether this long held description is indeed correct, a microscopic analysis of 10 surgical speci

To determine whether acoustic reflection measurement of the upper airway can be used to identify tracheas that are difficult to intubate, we conducted a pilot study of adults with a documented history of unexpected failed endotracheal intubation (16 cases) and compared them with 16 controls with previous successful intubation. The two groups were matched by age, sex, height, and weight. Acoustic reflection measurements of airway cross-sectional area versus distance were made at six combinations of body (upright and supine) and neck (flexed, neutral, and extended) positions. A retrospective study was performed on 106 consecutive patients with an acoustic neuroma who underwent suboccipital surgery for the first time between 1980 and 1992. Complete follow-up data were available for all the patients, including regular imaging up to 1 January 1994. Incomplete removal of the tumours was chosen in preference to radical surgery in a large proportion of the patients, to avoid the risk of post-operative morbidity. The recurrence rate in this series was 20%. Comparison of the results of suboccipital surgery to those obtained using other surgical approaches, while taking the

The diagnostic findings in 31 patients (32 ears) with acoustic neuromas are reported. In patients suspicious of acoustic neuroma, case history and clinical examination are important. In our material of 31 patients with acoustic neuromas, hearing loss was presented in 94% (slow onset 87%, sudden onset 10% and fluctuating 3%), tinnitus in 91%, vestibular symptoms (dizziness or vertigo) in 62%. It should be stressed that in patients with unilateral hearing loss (or tinnitus) or vestibular symptoms, it is necessary to exclude the presence of an acoustic neuroma. In addition, the trigeminal never w

Stereotactic radiosurgery is an important alternative treatment for carefully selected patients with acoustic tumors. We perform radiosurgery under local anesthesia, and 91% of our patients have been discharged from the hospital within 24 hours after treatment. All returned to their preoperative level of function or employment within 5 to 7 days after treatment. Our current tumor control rate is 97%, but reduction in tumor size, judged by strict, objective criteria, was achieved in only 23%. Our actuarial rate of useful hearing preservation after radiosurgery is 38% at 1 year. Three tumors inc

The morphology of the acoustic cortex was studied in light and electron microscopy, in 6 post mortem cases of Alzheimer's disease. Silver impregnation techniques and routine stainings were applied for the study of the cytoarchitecture and the cellular morphology of the acoustic cortex. Samples from every part of the acoustic cortex were processed for electron microscopy. The morphological findings were correlated with those of normal controls of relevant age, as well as with the temporal isocortex of the anterior part of the superior temporal gyrus. Silver impregnation techniques revealed a ma

In patients with bilateral acoustic neuromas, it is not unusual for the tumors to be of unequal size. At the time of the first examination, the smaller tumor may have already destroyed the hearing, while the ear with residual hearing may harbor a large acoustic neuroma. The remaining hearing is then at great risk if surgical removal of the tumor is attempted. Chemotherapy of bilateral acoustic neuromas is here presented as a possible alternative to surgery in those patients whose prognosis for conservation of hearing is poor.
