Physical symptoms and illness attributions in agoraphobia and panic.

The cognitive model of panic disorder proposes that panic patients interpret physical symptoms in a catastrophic way, thus precipitating panic attacks. This study describes the development of a questionnaire measure designed to assess beliefs about the health implications of various symptoms. Two groups, agoraphobics and normals, were compared on a variety of measures. Agoraphobics showed much higher panic frequency, fear of anxiety, and prevalence of frightening cognitions when anxious, but did not score higher than normals when assessing seriousness of symptoms. The implications of these res

Cognition and coping in agoraphobia and depression: a multivariate approach.

A multivariate strategy for the cognitive study of overlapping symptom disorders is presented and exemplified by an empirical investigation. … The externalizing of mental health locus of control to chance was the only significant predictor of agoraphobia factor score. Attributional variables were the only consistent predictors of depression factor score.

Neuroticism and age of onset for agoraphobia with panic attacks.

The study of the age of onset of psychiatric conditions can provide some clues to the aetiology of these disorders. A number of studies have examined the age of onset in agoraphobia but results have varied. This may be associated with small sample sizes or differences in populations. There has been very little work examining the factors determining age of onset. The present study examines age of onset in relation to sex and personality. Results indicate no sex differences in age of onset, but an association with age and high levels of neuroticism.

CSF prostaglandin-E in agoraphobia with panic attacks.

Prostaglandins are thought to act as neuromodulators of both central catecholamine and endocrine systems. Abnormalities of these systems have been described in affective disorders, in general, and in agoraphobia with panic attacks, in particular. This study measured basal prostaglandin-E (PGE) cerebrospinal fluid (CSF) levels in 20 patients with agoraphobia with panic attacks and 10 nonpsychiatric controls. In a subgroup of patients and controls, CSF levels of adrenocorticotrophic hormone (ACTH) and corticotropin-releasing factor (CRF) were also measured. There was no significant difference in…

Three cases of panic disorder with agoraphobia in children.

The authors report three cases of panic disorder with agoraphobia in children, with characteristic panic attacks, separation anxiety, and fear and avoidance of crowds and public places. The panic and agoraphobic symptoms responded to medications effective with agoraphobic adults, i.e., imipramine and alprazolam.

Obsessive-compulsive symptoms in agoraphobia: changes with treatment.

Ten patients suffering from agoraphobia with panic attacks reported significantly more obsessive-compulsive symptoms on the Comprehensive Psychopathological Rating Scale and Marks' Compulsion Checklist than healthy control subjects matched to the patients on sociodemographic variables. Behavioral treatment directed toward agoraphobia was associated with significant improvement in obsessive-compulsive complaints. On retesting, there were no significant differences in such symptoms between patients and controls. The results should alert the physician to inquire about the presence of agoraphobia …

Discrete dimensions in agoraphobia: a factor analytic study.

One hundred and one patients meeting DSM-III criteria for agoraphobia with panic rated their avoidance of 21 situations. Factor analysis of the ratings revealed five factors: two that were purely agoraphobic, involving travel or transportation and shopping; one that was agoraphobic but with social features; another that was agoraphobic with claustrophobic features; and a somewhat heterogeneous factor. The results were compared with an earlier analysis by Johnston, Johnston, Wilkes, Burns & Thorpe (1984) of ratings of the same situations and with the Fear Questionnaire of Marks & Mathews (1979)…

Diazepam in a patient with chronic schizophrenia complicated by agoraphobia.

A case of chronic schizophrenia complicated by agoraphobia expressed as an exacerbation of negative symptoms is presented. The patient responded to diazepam combined with behavior therapy. The need to recognize treatable factors contributing to negative symptoms of schizophrenia is discussed.

Inpatient treatment of complicated agoraphobia and panic disorder.

Despite recent advances in the treatment of agoraphobia and panic disorder, some patients do not respond to standard outpatient regimens of biological and psychosocial intervention and may require more intensive, closely supervised care. The authors describe a specialized inpatient program that integrates pharmacotherapy, intensive levels of exposure and other behavioral therapies, a structured and strategically reinforcing environment, panic and anxiety management strategies, and other interventions designed specifically for patients with complicated panic-based disorders. Outcome data for 25…

Immunological response to stress in agoraphobia and panic attacks.

The authors studied in vitro immune function as measured by lymphocytic proliferative response to mitogen stimulation in 36 patients with agoraphobia and panic attacks who were participating in a double-blind placebo-controlled psychopharmacological study of anxiety disorder. No significant difference in immune status was observed between panic patients and healthy controls.

Imipramine in the treatment of agoraphobia: dose-response relationships.

The authors examined dose-response relationships in 62 agoraphobic patients receiving either imipramine or placebo under double-blind conditions in conjunction with behavioral interventions. At the end of 12 weeks, patients treated with imipramine had improved significantly more than placebo patients. Results revealed that the beneficial therapeutic effect of imipramine was dose dependent and suggested that optimal response in agoraphobia may require doses of 150 mg/day or more. The results also indicated that side effects can significantly interfere with the buildup of optimal dose in agoraph…
