Circumstances surrounding non-fatal opioid overdoses attended by ambulance services.

INTRODUCTION AND AIMS: Opioid overdose fatalities are a significant concern globally. Non-fatal overdoses have been described as a strong predictor for future overdoses, and are often attended by the ambulance services. This paper explores characteristics associated with non-fatal overdoses and aims to identify possible trends among these events in an urban area in Norway. DESIGN AND METHODS: This is a retrospective analysis of non-fatal overdoses from Bergen ambulance services from 2012 to 2013. Demographic, temporal and geographic data were explored. RESULTS: During the two years, 463 non-

Training Persons with Spinal Cord Injury to Ambulate Using a Powered Exoskeleton.

Powered exoskeletons have become available for overground ambulation in persons with paralyses due to spinal cord injury (SCI) who have intact upper extremity function and are able to maintain upright balance using forearm crutches. To ambulate in an exoskeleton, the user must acquire the ability to maintain balance while standing, sitting and appropriate weight shifting with each step. This can be a challenging task for those with deficits in sensation and proprioception in their lower extremities. This manuscript describes screening criteria and a training program developed at the James J. P

Conus medullaris stroke. Does F wave predict return of ambulation?

Absent F wave in the stage of spinal shock has been described in cases of traumatic spinal cord injury. The role of F wave in predicting prognosis after conus medullaris infarct has not been described. We describe herein a middle aged-man with a conus medullaris infarct. Both tibial and peroneal F waves were absent on day 4. The left tibial F wave reappeared in the following study on day 18. All F waves reappeared on day 56 at which time the patient was still wheelchair bound. He regained walking on day 105. We hypothesize that reappearance of initially absent F waves post conus medullaris inf

Back to the future: preserved hippocampal network activity during reverse ambulation.

During movement, there is a transition of activity across the population, such that place-field centers ahead of the rat are sequentially activated in the order that they will be encountered. Although the mechanisms responsible for this sequence updating are unknown, two classes of models can be considered. The first class involves head-direction information for activating neurons in the order that their place fields will be traversed. An alternative model contends that motion and turn-related information from the posterior parietal cortex shift the subset of active hippocampal cells across th

Trends in quetiapine use and non-fatal quetiapine-related ambulance attendances.

INTRODUCTION AND AIMS: Concern about the non-medical use of quetiapine and related acute harms is growing. Case series cite quetiapine as a potential drug of misuse, while recent research questions its relative safety in comparison with other atypical antipsychotic preparations. This paper explores population-level patterns of quetiapine-related ambulance attendances over time, identifying associated risk factors and potential subpopulations at-risk of acute harms. DESIGN AND METHODS: A retrospective analysis of quetiapine-, olanzapine- and risperidone-related ambulance attendances in metropo

A biomechanical and subjective assessment and comparison of three ambulance cot design configurations.

Effects of ambulance cot design features (handle design and leg folding mechanism) were evaluated. Experienced ambulance workers performed tasks simulating loading and unloading a cot to and from an ambulance, and a cot raising task. Muscle activity, ratings of perceived exertion, and performance style were significantly affected by cot condition (p < 0.05). Erector Spinae activity was significantly less when using Cot-2's stretcher-style handles. Shoulder muscle activity was significantly less when using Cot-2's loop handle. During loading and unloading, operators allowed the cot to support i

Acute hip pain and inability to ambulate: a rare presentation for perirectal abscess.

A patient with acute hip pain out of proportion to physical findings and inability to weight bear despite negative plain films warrants further in-depth evaluation. Correctly diagnosing the cause of hip pain, a common emergency department (ED) complaint, may be a challenge in the geriatric population (Perron A, Miller M, Brady W. [A case of brugada syndrome presenting with syncope transferred by ambulance to a neurosurgical hospital].

Brugada syndrome is a known cause of sudden death. We report a case of Brugada syndrome who was transferred by ambulance to our neurosurgical hospital. An 18-year-old male suddenly lost consciousness and collapsed at his home. His mother urgently called for an ambulance because there was atypical absence of consciousness for several minutes. Because the Japan Coma Scale (JCS) and the consciousness level was about 10 on arrival by ambulance, the emergency services suspected brain concussion, so transported him to our neurosurgical hospital. However, the JCS reached a level of 1 in the emergency

Community ambulation in patients with chronic stroke: how is it related to gait speed?

OBJECTIVE: To explore the strength of the association between gait speed and community ambulation and whether this association is significantly distorted by other variables. DESIGN: Cross-sectional study conducted 3 years after stroke. SUBJECTS: A total of 102 patients after first-ever stroke following inpatient rehabilitation who are now living in the community. METHODS: Community ambulation was determined by a self-administered questionnaire with 4 categories. Gait speed was assessed by the 5-m walking test. Possible confounding factors included in the analyses were: age, hemisphere, livi

Ambulation with the reciprocating-gait orthosis. Experience in 15 children with myelomeningocele or paraplegia.

We reviewed 15 children with spina bifida or paraplegia who have used a reciprocating-gait orthosis between 1985 and 1995. All were nonfunctional ambulators. The level of the spinal lesions ranged from Th10 to L3. The mean age of fitting the orthosis was 5 years. 8 children have stopped using it at an average age of 10 years. The maximum ambulation level with the orthosis was a community ambulator in 4 children, household ambulator in 9 children and 2 remained nonfunctional ambulators. The average daily use was 6 hours by community ambulators to 0.5 hours nonfunctional ambulators. Bilateral di

Probabilities of the development of ambulation for mobile children with moderate to profound mental retardation.

Development of ambulation was studied among 1,744 children, ages 3 through 10 years, with moderate to profound mental retardation. The sample was limited to children with some degree of mobility at initial testing. Evaluations over 6 years indicated that improvements in basic skills, although not probable, do occur far past the normative age for development of speech and ambulation and across all levels of mental retardation. The probability of improvement declined with age. Degree of mental retardation and severity of motor impairment had the expected negative effects on initial skill level a

Acute epiglottitis in a rural area: experiences with an anesthesiologist-staffed ambulance helicopter.

The majority of fatalities due to acute epiglottitis (AE) result from prehospital airway problems. We reviewed the courses of 14 patients with AE treated by an aeromedical team consisting of an anesthesiologist and a paramedic. Eight patients were transported from a physician's office or from the patient's own home. One patient was intubated at the scene, and two received ventilatory support with mask and bag en route to the hospital. Two patients suffered cardiopulmonary arrest before arrival of the aeromedical team, both resulting in severe hypoxic encephalopathy. All six patients transporte

Interface shear stresses during ambulation with a below-knee prosthetic limb.

Shear stresses on a residual limb in a prosthetic socket are considered clinically to contribute to tissue breakdown in below-knee amputees. When applied simultaneously with normal stresses, they can cause injury within the skin or can generate an abrasion on the surface. To gain insight into shear stresses and parameters that affect them, interface stresses were recorded on below-knee amputee subjects during walking trials. On the tibial flares, resultant shear ranged from 5.6 kPa to 39.0 kPa, while on the posterior surface it ranged from 5.0 kPa to 40.7 kPa. During stance phase, anterior res

High-dose naloxone modifies cardiovascular and neuroendocrine function in ambulant subjects.

To determine the role of the opioids in the control of the cardiovascular system in awake ambulatory subjects, eight healthy men were infused with a high dose of naloxone (10 mg bolus plus 7 mg/hr), or saline placebo, for 12 hr. Ambulatory monitoring of blood pressure and heart rate every 10 min indicated no differences between trials for blood pressure (p greater than 0.80), but a significant difference for the maximal heart rate response during stair climbing or 1 km walks (p less than 0.02). Plasma cortisol concentration were increased during the naloxone trials (p less than 0.05), as was t
