Cost-effectiveness of amiodarone in cardiac surgery.

Atrial fibrillation is the most common complication after open-heart surgery. The incidence rate is 25-50%. This review summarizes findings of economic studies of amiodarone prophylaxis in cardiac surgery patients. Data were pooled from more than 15 studies. Four meta-analyses combining six to 19 amiodarone trials indicated a significant decrease in the incidence of postoperative atrial fibrillation (odds ratio range: 0.50-0.54). Other studies (individual randomized controlled trials, naturalistic studies and economic models) supported this finding. A majority of studies showed that amiodarone…

[Amiodarone and thyroid dysfunction: a pending problem].

Amiodarone is an antiarhytmic drug used in many clinical situations for its probed effect; it is also preferred in particular groups of patients (heart failure, post-ischemical) for its safe and its prognostic benefits. However, a substantial proportion of amiodarone treated patients develop either hypothyroidism or thyrotoxicosis. Both abnormalities may occur in apparently normal glands or in glandes with pre-existed abnormalities. It may be difficult to recognize the dysfunction because of the many changes in thyroid function test results that occur in euthyroid patients who are receiving am…

Amiodarone-induced thyrotoxicosis: not a benign condition.

Amiodarone is a heavily iodinated drug effective in the control of various tachyarrhythmias. Despite its undoubted benefit, treatment with amiodarone is not without risk, because a number of potentially serious side-effects may develop during therapy. Thyrotoxicosis is most commonly recognised but its potential consequences tend to be underestimated, and its treatment can be challenging and difficult. We report several cases of amiodarone-induced thyrotoxicosis which serve to highlight the potential severity of the condition.

The various effects of amiodarone on thyroid function.

Amiodarone, a benzofuranic-derivative iodine-rich drug used mostly for tachyarrhythmias, often causes changes in the peripheral metabolism of thyroid hormones mainly due to the inhibition of 5'-deiodinase activity: an increase in serum thyroxine and reverse triiodothyronine, and a decrease in serum triiodothyronine concentrations. Overt thyroid dysfunction, either amiodarone-induced thyrotoxicosis (AIT) or amiodarone-induced hypothyroidism (AIH), occurring in 14% to 18% of patients receiving long-term treatment, may develop both in apparently normal thyroid glands and in glands with preexistin…

Amiodarone -- waxed and waned and waxed again.

Amiodarone has been used as an anti-arrhythmic drug since the 1970s and has an established role in the treatment of ventricular tachyarrhythmias. Although considered to be a class III anti-arrhythmic, amiodarone also has class I, II and IV actions, which gives it a unique pharmacological and anti-arrhythmic profile. Amiodarone is a structural analogue of thyroid hormone and some of its anti-arrhythmic properties and toxicity may be attributable to interactions with nuclear thyroid hormone receptors. The lipid solubility of amiodarone gives it an exceptionally long half-life. Oral amiodarone ta…

[Therapy of amiodarone-induced thyrotoxicosis and hypothyroidism].

Pathogenesis, diagnostic procedures and therapy of amiodarone-induced thyrotoxicosis (AIT) and hypothyroidism (AIH) are briefly discussed. Diagnosis of AIT and AIT is based on the classical signs and symptoms of thyroid dysfunction, although oligosymptomatic cases may occur, and on laboratory tests such as TSH, fT4 und fT3. In AIT, radioiodine therapy usually is no option due to the high iodine content of amiodarone. Besides withdrawal of amiodarone, medical and surgical treatment remain the only modalities. If arrhythmia can be controlled by an alternative treatment, amiodarone should be disc…

Amiodarone-induced hyperthyroidism and a thyroid nodule.

OBJECTIVE: To report two cases of thyrotoxicosis associated with amiodarone therapy and to discuss how the presence of a thyroid nodule alters management. METHODS: We describe two patients with amiodarone-induced hyperthyroidism, review the mechanisms of disease, and outline a strategy for decision making relative to treatment in the presence of a thyroid nodule. RESULTS: Amiodarone-induced thyrotoxicosis occurs by two mechanisms: a direct toxic effect and iodine loading. This condition may be treated medically or surgically, but when discontinuation of amiodarone therapy is not possible and…

Can pancreatitis be associated with amiodarone hepatotoxicity?

We report a patient with classical features of amiodarone hepatotoxicity who died of progressive liver failure. Throughout the course of his illness, he had epigastric pain, nausea, vomiting, and persistent mild to moderate elevation of amylase and lipase in his serum and peritoneal fluid. Pancreatitis due to amiodarone has not been reported. We raise the question of whether or not the pancreas is yet another organ subject to amiodarone toxicity and speculate as to possible pathogenesis. We suggest that patients on amiodarone who develop abnormal liver enzymes, nausea, vomiting, or abdominal p…

Pathology of the thyroid in amiodarone-associated thyrotoxicosis.

Among 83 consecutive patients operated on for thyrotoxicosis at the Mayo Clinic between January 1, 1980 and May 1, 1986, four had a combination of pathologic findings that were similar and unexpected: involutional changes; degenerative and destructive follicular lesions; and zones of fibrosis. These four, but none of the remaining 79, were being treated with amiodarone for cardiac tachyarrhythmias; this drug is known to be taken up by the thyroid gland. Characteristically, small groups of involuted follicles exhibited varying degrees of damage ranging from degenerative changes in a few lining …

Amiodarone in the management of refractory atrial fibrillation.

Thirty-eight patients were studied to evaluate amiodarone hydrochloride in the treatment of refractory atrial fibrillation. Among them were 25 with sustained atrial fibrillation and 13 with paroxysmal atrial fibrillation. All patients were symptomatic and refractory to therapeutic doses of at least two conventional drug trials, and patients with atrial fibrillation had relapsed from electroversion. Amiodarone hydrochloride was administered in doses of 5 mg/kg intravenously, then 600 to 800 mg/d for seven to ten days, followed by 200 to 400 mg/d. Holter recordings were obtained every one to thr…

Intravenous and oral amiodarone for arrhythmias in children.

Oral amiodarone was administered to 30 children (aged one week to 14 years) for treatment of resistant or life threatening tachycardias. Five children received initial intravenous medication. The mean duration of oral treatment ranged from two weeks to 64 months (mean 23 months). Infants required a higher oral dose than older children when this was calculated on the basis of body weight but not when it was calculated on the basis of body surface area, indicating that the prescribed dose of amiodarone for infants should be calculated on the basis of body surface area. Although plasma concentrat…

Amiodarone. The dilemma of hyperthyroxinaemia and the treatment of thyrotoxicosis.

The use of amiodarone, a drug which is prescribed increasingly as an anti-anginal and anti-arrhythmic agent, necessitates a high index of suspicion for the development of thyroid disorders, especially thyrotoxicosis. Two cases, which illustrate the diagnostic dilemma of hyperthyroxinaemia and the poor response to antithyroid medication, are described. During amiodarone therapy, the clinical features of thyrotoxicosis may be masked or atypical, and the choice of therapy is complicated by a delayed response to thioamide drugs and possible contraindication for beta-blocking agents which necessita…

Amiodarone: pharmacology and antiarrhythmic and adverse effects.

Amiodarone is a benzofuran derivative that has been effective for the treatment of both supraventricular and ventricular tachyarrhythmias. It has a large volume of distribution, moderate bioavailability and a long half-life. Its pharmacokinetics are not well understood and its tissue distribution is not typical of a 2-compartment model. Due to ocular, dermatologic, gastrointestinal, neurologic, cardiovascular, thyroid and pulmonary toxicity, amiodarone should be reserved for use in patients with refractory and/or life-threatening arrhythmias.

A study of cutaneous photosensitivity induced by amiodarone.

Amiodarone-induced cutaneous photosensitivity was studied in 12 subjects treated with the drug. The action spectrum for the abnormal response to sunlight was shown to be within the range of 335-460 (+/- 30) nm. The clinical features of the photosensitivity response suggested that it was most probably a phototoxic reaction, a conclusion supported by the results of in vitro studies which indicated activity mainly against cell membranes. Of the five in vitro models used, three--namely photohaemolysis, the inhibition of DNA synthesis in PHA stimulated lymphocytes and the killing of mouse peritonea…

[Antiarrhythmic effect and side effects of amiodarone].

UNLABELLED: Amiodarone (AM) is one of the most potent antiarrhythmic drugs, the value of which is limited by reversible and irreversible side-effects (SE). 59 patients, 50 male, 9 female (age 33 to 81 years) entered the study with ventricular tachycardia (VT, 68%), WPW-tachycardia (12%), non-sustained VT (12%) or untreatable paroxysmal atrial fibrillation or supraventricular tachycardia (8%). Prior to AM the patients had received 1 to 8 different antiarrhythmic drugs (m 3.5) and maximal 9 different combinations of antiarrhythmics. The drug regimen started with a loading dose of 1200 mg/d for 1…

Amiodarone for tachyarrhythmias: pharmacology, kinetics, and efficacy.

Amiodarone, although widely studied in Europe, is a recent addition to the investigational antiarrhythmics being used in the U.S. Pharmacologically, its primary cardiac effects are to increase coronary artery blood flow, increase the effective refractory period, and produce an atropine-resistant bradycardia. Amiodarone is incompletely (approximately 50 percent) and slowly (peak serum concentration approximately 6 h) absorbed. With chronic administration, it deposits both in adipose tissue and in organs with high blood perfusion. It has an apparent elimination half-life of 15-45 days, which pre…
