Total hip arthroplasty in above-knee amputees: a case report.

There are previous case reports in the literature that describe total knee and total hip arthroplasty (THA) in below-knee amputees, but we could find no case reports on above-knee amputees (AKAs) who have severe osteoarthritis of the hip. We present a case involving an AKA who developed severe osteoarthritis of the ipsilateral hip. Out patient underwent THA with a satisfactory postoperative outcome. Technical considerations for AKAs undergoing THA are also reviewed.

Spatial, temporal and kinematic characteristics of traumatic transtibial amputees' gait.

Presented study gives findings of temporal-spatial kinematic gait analysis in 12 male adults, war trauma transtibial (TT) amputees fitted with prosthesis compared to 12 able-bodied persons. Results disclose asymmetries between the amputated, prosthetic and healthy legs of amputees, as well as between TT amputees and non-disabled persons. Amputees in comparison with able-bodied persons, generally, showed significantly increased swing-time (ms) (p < 0.01). Prosthetic, right legs of amputees showed decreased stance-phase (ms and % GC) (p < 0.01) and increased swing-time (ms) (p < 0.05), compared …

Anticipatory postural adjustments during standing in below-the-knee amputees.

OBJECTIVE: We studied the role of adaptive changes within the central nervous system in anticipatory postural adjustments seen in unilateral below-the-knee amputees. DESIGN: Changes in electromyographic and mechanical variables were compared during standardized tasks performed by standing subjects. BACKGROUND: Anticipatory postural adjustments represent an important mechanism of postural control which was expected to be changed in amputees because of both mechanical and secondary, neurological reasons. METHODS: Six patients after a below-the-knee amputation and six control subjects stood on a …

Dynamic electromyography in active and limited walking below-knee amputees.

Walking electromyography was performed in 12 below-knee amputees, who were amputated for peripheral vascular insufficiency. All were at least 1 year post-surgery and had no problems with the fit of their prostheses. Seven were community walkers and five were limited household ambulators. The quadriceps and hamstring muscles exhibited electrical activity during 60% and 64.7% of the gait cycle, respectively, in the active walkers, compared with 44.6% and 53.8% in the limited walkers. Quadriceps muscle activity was initiated almost 10% earlier in the active walkers, while hamstring muscle initiat…

Prostheses and the forward reach of sitting lower-limb amputees.

The hypothesis that prostheses improve the forward reach of sitting lower-limb amputees was tested. While sitting with only ischial support, ten unilateral below-knee (BK) amputees could reach farther when they wore their prostheses than when they did not, when reaching at 45 degrees toward or away from the side of the amputation, with mean differences (+/- 1 SD) of 10.7cm (+/- 6.9, p less than 0.0001) and 4.4cm (+/- 4.1, p less than 0.01), respectively. With ten unilateral above-knee (AK) amputees, the differences were not statistically significant. Seven bilateral amputees (mixed levels) wer…

Non-invasive brain stimulation reveals reorganized cortical outputs in amputees.

EMG responses to non-invasive electromagnetic brain stimulation (EMS) were recorded from arm muscles of congenital amputees. Responses were obtained with lower thresholds on the amputated than on the intact side and were evoked from a larger cortical area. Contracting muscles showed increased responses to EMS; the increase was more pronounced on the amputated side. Similar findings were obtained in one traumatic amputee who suffered an early amputation, but not in another patient with a late amputation. We conclude that in congenital amputees there is substantial reorganisation of the corticos…

Insulin as a potential factor influencing blood pressure in amputees.

War-injured, bilateral above-knee amputees are known to be at increased risk for cardiovascular mortality. To evaluate possible risk factors, we compared blood pressures and plasma glucose and insulin responses to orally administered glucose in 19 above-knee amputees from the Vietnam War (mean age, 36 +/- 1 years) with those of 12 age-matched unilateral below-elbow amputees. Body composition by densitometry and maximal oxygen consumption during arm or leg exercise were also determined. Nine of 19 leg amputees were hypertensive compared with one of 12 arm amputees. Their 3-hour average insulin …

Performance evaluation of BK amputees through graded load carrying tests.

The ergonomic approach to performance evaluation in orthopaedic rehabilitation presents problems because of the cumbersome and time-consuming measurement and test procedures involved. This paper describes a method of performance evaluation through graded load carrying tests which makes it possible to set up regression equations which can be used in routine clinical practice for prediction of the performance of below-knee amputees.

Flexible & Stretchable EMG Sensor for Lower Extremity Amputee.

EMG signals can be widely used for indicators of muscle activity, and it can be used for robot control. However, the practical use of the EMG sensor for the amputee has been limited due to harsh conditions in the socket where strong pressure and friction exist. In this paper, thus we suggested a flexible and stretchable EMG Sensor. It is designed to withstand the pressure of the socket and to be used repeatedly with soft adhesive material. The performance of mechanical and electrical properties is investigated, and the muscle signals are recorded in static and dynamic (jump and gait) condition

Referred Sensation Areas in Bilateral Upper Limb Amputee.

Phantom limb pain (PLP) following amputation considerably reduces the quality of life, given a difficult to treat pain of highly variate profile. The loss of sensory input induces a complex pattern of neuroplastic changes of the sensory neural pathways and their central projections. Referred sensation areas (RSAs) may occur on the stump as a consequence of amputation, providing a direct path towards the altered central sensory projections. Modulated electrical stimulation of RSAs was investigated in a long-term experiment in the case of a 62 years-old participant with bilateral upper limb ampu

Prosthetic options available for the diabetic lower limb amputee.

Although the rate of lower limb amputation in patients with diabetes is decreasing, amputation still remains a major complication of diabetes. Prosthetics have been long used to help amputees ambulate. The last decade has seen many advances in prostheses with the enhanced understanding of the mechanics of ambulation and improved use of technology. This review describes the different types of prosthetic options available for below knee, ankle, and foot amputees, emphasizing the latest advances in prosthetic design.
