Exercise-induced urticaria and anaphylaxis have become increasingly recognized during the past 2 decades as more people participate in physical activities. These syndromes can be categorized as cholinergic urticaria or exercise-induced anaphylaxis based on the clinical manifestation. An anaphylactic reaction to latex is reported in two patients. One occurred intraoperatively with exposure to the surgeon's latex gloves and the other during vaginal contact with a latex condom. Radioallergosorbent test (RAST) and scratch test with latex material identified the allergen. With the increasing use of latex gloves and condoms, one should be aware of the possibility of a severe immediate hypersensitivity reaction to latex products. Latex allergy should be considered when evaluating intraoperative and idiopathic anaphylaxis.

We report a case which illustrates occupational sensitivity to Papain with life-threatening anaphylaxis. In addition to urticaria and angioedema and glottic symptoms, the patient demonstrated annoying intermittent problems mimicking rheumatoid, gastrointestinal, cardiac, and pulmonary disease. Food-dependent exercise-induced anaphylaxis, first reported in 1983, is a subtype of exercise-induced anaphylaxis. A case of celery-dependent exercise-induced anaphylaxis is reported. The presentation and management of these and other exercise-related physical allergies, including classic and variant cholinergic urticaria, is reviewed. As the prevalence of strenuous physical activity increases, it is important for emergency physicians to recognize unusual, but potentially serious, complications of exercise.
