Effects of maternal bleeding problems on the fetus and neonate.

Maternal bleeding disorders directly affect fetal and neonatal outcome. This article will review how maternal bleeding may cause polycythemia, anemia, or hypovolemic states in the fetus, which will have an impact on the neonatal period. If problems are anticipated, identified, and treated aggressively, outcomes can be improved.

Neonatal manifestations of inherited bone marrow failure syndromes.

The inherited bone marrow failure syndromes (IBMFS) are a rare yet clinically important cause of neonatal hematological and non-hematological manifestations. Many of these syndromes, such as Fanconi anemia, dyskeratosis congenita and Diamond-Blackfan anemia, confer risks of multiple medical complications later in life, including an increased risk of cancer. Some IBMFS may present with cytopenias in the neonatal period whereas others may present only with congenital physical abnormalities and progress to pancytopenia later in life. A thorough family history and detailed physical examination are

Hereditary red cell defects as an underrecognized cause of neonatal jaundice.

BACKGROUND: Neonatal jaundice is most commonly caused by blood group incompatibility but may also result from hereditary elliptocytosis (HE) associated with heterozygous SPTB mutations. METHODS: A retrospective descriptive study was conducted among 1,584 neonates presenting with jaundice over a 3-year period. RESULTS: Seventy-six neonates (4.8%) were diagnosed with HE, all carrying heterozygous SPTB mutations (Providence variant, n = 67; Buffalo variant, n = 9). Five had coexisting hemoglobinopathies. Early-onset jaundice occurred in 71 patients, with a median onset of 38 h. Peak total bilir

Diamond-Blackfan anaemia presents as pathological foetal CTG and severe neonatal anaemia.

Diamond-Blackfan anaemia (DBA) is a rare congenital disorder characterised by pure red cell aplasia and often associated with various congenital anomalies. This chronic condition has a heterogeneous genetic profile, typically inherited in an autosomal dominant manner, though rare cases of autosomal recessive inheritance exist. Most patients (90%) exhibit symptoms before 12 months of age, with the median age of presentation and diagnosis being approximately 2 months. This report presents a unique case of DBA identified at birth, resulting in a preterm delivery. The patient continues to be manag

Anemia in Infants and Children: Evaluation and Treatment.

Anemia affects more than 269 million children globally, including 1.2 million children in the United States. Although anemia can present with numerous symptoms, children are most often asymptomatic at the time of diagnosis. Anemia in infants and children most often arises from nutritional iron deficiency but can also be a result of genetic hemoglobin disorders, blood loss, infections, and other diseases. In the United States, newborn screening programs assess for various genetic causes of anemia at birth. The US Preventive Services Task Force notes insufficient evidence to recommend universal

Infantile pyknocytosis, a rare cause of hemolytic anemia in newborns: report of two cases in twin girls and literature overview.

Infantile pyknocytosis is a rare cause of neonatal jaundice and hemolytic anemia. We report on two cases in twin girls that were diagnosed on peripheral blood smear reading. Pyknocytosis should be considered in cases of early unexplained severe hemolytic anemia, and systematic peripheral smear review performed. Its management consists of phototherapy and RBC transfusion.

Infantile Pyknocytosis in a Premature Dichorionic Diamniotic Twin.

Infantile pyknocytosis is a rare and self-limiting cause of hemolytic anemia in neonates. It can result in severe anemia and hyperbilirubinemia. The pathogenesis is unknown: a genetic origin has been discussed; however, based on the current literature it is not clear which genetic mutations should be considered. We present a case of a premature twin, in whom genetic screening was performed. Genetic mutations in 46 genes associated with hereditary hemolytic anemia and dyserythropoietic anemia were tested. No mutations were found. In infantile pyknocytosis, a genetic defect in these genes is unl

Nutritional anemias in infancy.

Anemia in the 1st year of life should be an important warning sign to the clinician of a potential underlying nutritional deficiency. Prompt diagnosis and treatment are warranted in many cases, not only to treat the anemia but also, more important, to prevent other long-term sequelae associated with the underlying, causative deficiency. As more information about the impact of nutrition on various aspects of health is collected and newer therapies for the treatment of disease are developed, it will be important to understand and consider the role of vitamins and minerals in a light beyond the r

Successful Management of the Fetal Severe Anemia Associated with Jra Alloimmunization by Intrauterine Transfusion of Jr(a+) Red Blood Cells.

OBJECTIVE: We present a case of fetal severe anemia associated with Jra alloimmunization, which was managed using Doppler measurement of the peak systolic velocity of the fetal middle cerebral artery (MCA-PSV) and intrauterine transfusion (IUT) of Jr(a+) red blood cells (RBCs). We also review the previous case reports on fetal or neonatal anemia associated with Jra alloimmunization. CASE REPORT: A woman with Jra alloimmunization was referred to our department at 29 weeks of gestation. As fetal MCA-PSV exceeded 1.55 multiples of the median, fetal blood sampling was performed and demonstrated s

Congenital Thrombotic Thrombocytopenic Purpura With a Novel ADAMTS13 Gene Mutation.

Congenital thrombotic thrombocytopenic purpura (TTP) is an autosomal recessive disorder characterized by thrombocytopenia, microangiopathic hemolytic anemia (MAHA), and thrombosis. Congenital TTP should also be considered while investigating neonatal hyperbilirubinemia, hemolytic anemia, or isolated thrombocytopenia. This case is of an 8-year-old male child who presented with prolonged and recurrent history of thrombocytopenia and MAHA, first identified when he was seven weeks of age preceding neonatal hyperbilirubinemia. Peripheral blood smear examination showed thrombocytopenia and schistocy

Clinical and molecular characterization of novel deletions causing epsilon gamma delta beta thalassemia: Report of two cases.

Epsilon gamma delta beta (εγδβ)0 - thalassemia is a very rare disorder that results from large deletions in the β-globin gene cluster which abolish all regional globin chain gene expression from that allele. Since it is an exceedingly rare cause of neonatal anemia and is not detected by routine newborn screening, it is usually not suspected clinically and commonly undiagnosed or misdiagnosed. In this study, we describe two patients diagnosed in our hospital with (εγδβ)0-thalassemia based on the results obtained from DNA microarray analysis of their peripheral blood. The first patient of mixed

Hemolytic Jaundice and Anemia with Methemoglobinemia in Dizygotic Newborn Twins with Hb Evans: Case Report.

INTRODUCTION: It is highly unusual for a newborn infant to present in the first days after birth with cyanosis secondary to methemoglobinemia and with hemolytic anemia and jaundice. This combination suggests the possibility of an unstable hemoglobin variant. CASE PRESENTATION: We describe dizygotic twins, both with this presentation, where whole genome sequencing identified hemoglobin Evans (a point missense substitution in HBA2, the alpha2-globin gene) and found no other co-inherited variants known to be associated with neonatal methemoglobinemia, jaundice, or anemia. Herein we report on fou

Ascites, anemia and (intestinal) atresia.

We report a neonate who presented antenatally with fetal ascites and fetal anemia. The cause of this remained uncertain until 2 weeks later when bowel dilatation was noted on antenatal ultrasound. Clinical signs of intestinal obstruction became evident after delivery, and ileal atresia was found at laparotomy, which was resected with primary anastomosis. Recent reports in the literature have suggested a possible connection between fetal ascites, anemia and dilated bowel with neonatal intestinal obstruction. In these cases and ours, the likely sequence of events was that of in utero midgut volv

Cord blood iron profile and breast milk micronutrients in maternal iron deficiency anemia.

BACKGROUND: Micronutrient deficiencies among pregnant women are widespread in low-income countries, including Egypt. Iron deficiency anemia (IDA) is the most frequent nutritional deficiency during pregnancy, with an impact on maternal and fetal morbidity and mortality. We aimed to evaluate the effect of maternal IDA and nutritional status on birth anthropometry, cord blood iron profile and breast milk micronutrients in 50 anemic (hemoglobin <11 g/dl) and 30 healthy pregnant women. PROCEDURE: Maternal and neonatal anthropometric measures were recorded. Hemoglobin, red blood cell (RBC) indices,

Evaluating eosin-5-maleimide binding as a diagnostic test for hereditary spherocytosis in newborn infants.

OBJECTIVE: Neonates with undiagnosed hereditary spherocytosis (HS) are at risk for developing hazardous hyperbilirubinemia and anemia. Making an early diagnosis of HS in a neonate can prompt anticipatory guidance to prevent these adverse outcomes. A recent comparison study showed that a relatively new diagnostic test for HS, eosin-5-maleimide (EMA)-flow cytometry, performs better than other available tests in confirming HS. However, reports have not specifically examined the performance of this test among neonates. STUDY DESIGN: We compared EMA-flow cytometry from blood samples of healthy con

Maternal anemia effects during pregnancy on male and female fetuses: are there any differences?

OBJECTIVE: Sideropenic anemia is a common pregnancy disorder. The relationship between anemia and adverse pregnancy outcome are contradictory, and it is related to the severity of the hemoglobin deficit. The aim of the study was to evaluate the relationship between maternal mild anemia at third trimester of pregnancy, fetal birth weight and fetal gender. STUDY DESIGN: A retrospective study including 1131 single physiological term pregnancies was conducted. According to maternal Hb levels during the third trimester, pregnant women enrolled were divided in two groups: Group A (n = 156) with Hb

Breast milk induced immune haemolytic disease of newborn due to anti-c: A case report.

BACKGROUND: Hemolytic disease of fetus and newborn is a major risk factor for anemia and hyperbilirubinemia in newborns. Early identification and diagnosis can significantly improve neonatal health. CASE REPORT: This report documents a case of hemolytic disease of fetus and newborn presenting as persistent neonatal anemia requiring frequent transfusion support. The underlying cause was determined to be the passive acquisition of hemolytic alloantibodies (anti-c) via breast milk. CONCLUSION: Importance of antenatal screening for red cell antibodies is gradually being recognized and adopted in
